Acute promyelocytic leukemia (APL) is characterized by the presence of the t(15;17) and PML-RARa rearrangement, with good response to treatment with retinoids. However, few cases of variant APL involving alternative chromosomal aberrations have been reported, including t(11;17)(q23;q21) (Wells et al. in Nat Genet 17:109-113, 1; Arnould et al. in Hum Mol Genet 8:1741-1749, 2) t(5;17)(q35;q12-21), t(11;17)(q13;q21) (Grimwade et al in Blood 96:1297-1308, 3) and der(17) (Rego et al. in Blood (ASH Annual Meeting Abstracts)114:Abstract 6, 4), whereby RARa is fused to the PLZF, NPM, NuMA, and STAT5b genes, respectively, have been described. These cases are characterized by distinct morphology, clinical presentation, and in respect to PLZF, a lack of differentiation response to retinoids leading to the need of different approaches concerning diagnostic methods and therapeutics. This paper describes two cases of APL associated with the PLZF-RARA fusion gene enrolled in the IC-APL trial that is a non-randomized, multicenter study conducted in Brazil, Mexico, Chile and Uruguay with the aim to improve the treatment outcome of APL patients in developing countries. These cases, although rare, offer a challenge to its early recognition and proper conduction.

Background: In epidemiological surveys, a good reliability among the examiners regarding the caries detection method is essential. However, training and calibrating those examiners is an arduous task because it involves several patients who are examined many times. To facilitate this step, we aimed to propose a laboratory methodology to simulate the examinations performed to detect caries lesions using the International Caries Detection and Assessment System (ICDAS) in epidemiological surveys. Methods: A benchmark examiner conducted all training sessions. A total of 67 exfoliated primary teeth, varying from sound to extensive cavitated, were set in seven arch models to simulate complete mouths in primary dentition. Sixteen examiners (graduate students) evaluated all surfaces of the teeth under illumination using buccal mirrors and ball-ended probe in two occasions, using only coronal primary caries scores of the ICDAS. As reference standard, two different examiners assessed the proximal surfaces by direct visual inspection, classifying them in sound, with non-cavitated or with cavitated lesions. After, teeth were sectioned in the bucco-lingual direction, and the examiners assessed the sections in stereomicroscope, classifying the occlusal and smooth surfaces according to lesion depth. Inter-examiner reproducibility was evaluated using weighted kappa. Sensitivities and specificities were calculated at two thresholds: all lesions and advanced lesions (cavitated lesions in proximal surfaces and lesions reaching the dentine in occlusal and smooth surfaces). Conclusion: The methodology purposed for training and calibration of several examiners designated for epidemiological surveys of dental caries in preschool children using the ICDAS is feasible, permitting the assessment of reliability and accuracy of the examiners previously to the survey´s development.

Na Região Metropolitana de São Paulo, a rede de monitoramento da qualidade do ar registra frequentemente valores de concentração de ozônio superiores aos estabelecidos pela legislação em vigor no Brasil. Em algumas ocasiões, condições meteorológicas favoráveis ao processo de formação de ozônio possibilitam a ocorrência de altas concentrações de ozônio durante diversos dias consecutivos na região. A influência de condições meteorológicas no episódio de poluição, registrado entre os dias de 24 de fevereiro e 05 de março de 2003, foi avaliada através de simulações numéricas com o modelo SPM-BRAMS. Estudos observacionais mostram que a ocorrência de ventos fracos, principalmente, durante o período noturno e durante as manhãs, favorece a formação de altas concentrações de ozônio sobre a região. O horário de entrada e a atuação da brisa marítima são fundamentais para o transporte do ozônio e de seus precursores. Essas condições foram bem simuladas pelo modelo. Para os dias mais críticos, quando diversas estações de monitoramento registraram a ocorrência de violações ao padrão nacional de qualidade do ar, o modelo simulou adequadamente as condições atmosféricas determinantes para a manutenção de valores elevados nas concentrações de ozônio, sem a necessidade de nenhum procedimento artificial ao longo das integrações.
