There is growing evidence that the course of Bipolar Affective Disorder (BAD) can be altered by psychotherapeutic approaches, such as Psychoeducation. Therefore, this study was performed with the objective of identifying the implications of a Psychoeducation group on the everyday lives of individuals with BAD. To do this, the authors chose to perform a qualitative case study. Participants included twelve individuals with BAD who had attended at least six meetings of the Psychoeducation Group held at the Sao Jose do Rio Preto Faculty of Medicine (FAMERP). Semi-structured interviews were performed, which were recorded and then transcribed and subjected to Thematic Analysis. The present study showed that the referred group experience promoted the individuals' knowledge acquisition; their awareness regarding the disease and adherence to treatment; their making positive changes in life; the possibility of helping other patients to benefit from the knowledge learned in the group; and their awareness regarding other realities and coping strategies, obtained by exchanging experiences with other participants.

Sports Exerc., Vol. 44, No. 7, pp. 1227-1234, 2012. Purpose: Exposure to diesel exhaust particles (DEP) results in lung inflammation. Regular aerobic exercise improves the inflammatory status in different pulmonary diseases. However, the effects of long-term aerobic exercise on the pulmonary response to DEP have not been investigated. The present study evaluated the effect of aerobic conditioning on the pulmonary inflammatory and oxidative responses of mice exposed to DEP. Methods: BALB/c mice were subjected to aerobic exercise five times per week for 5 wk, concomitantly with exposure to DEP (3 mg.mL (1); 10 mu L per mouse). The levels of exhaled nitric oxide, reactive oxygen species, cellularity, interleukin 6 (IL-6), and tumor necrosis factor alpha (TNF-alpha) were analyzed in bronchoalveolar lavage fluid, and the density of neutrophils and the volume proportion of collagen fibers were measured in the lung parenchyma. The cellular density of leukocytes expressing IL-1 beta, keratinocyte chemoattractant (KC), and TNF-alpha in lung parenchyma was evaluated with immunohistochemistry. The levels of IL-1 beta, KC, and TNF-alpha were also evaluated in the serum. Results: Aerobic exercise inhibited the DEP-induced increase in the levels of reactive oxygen species (P < 0.05); exhaled nitric oxide (P < 0.01); total (P < 0.01) and differential cells (P < 0.01); IL-6 and TNF-alpha levels in bronchoalveolar lavage fluid (P < 0.05); the level of neutrophils (P < 0.001); collagen density in the lung parenchyma (P < 0.05); the levels of IL-6, KC, and TNF-alpha in plasma (P < 0.05); and the expression of IL-1 beta, KC, and TNF-alpha by leukocytes in the lung parenchyma (P < 0.01). Conclusions: We conclude that long-term aerobic exercise presents protective effects in a mouse model of DEP-induced lung inflammation. Our results indicate a need for human studies that evaluate the pulmonary responses to aerobic exercise chronically performed in polluted areas.

Chronic tonsillar diseases are an important health problem, leading to large numbers of surgical procedures worldwide. Little is known about pathogenesis of these diseases. In order to investigate the role of respiratory viruses in chronic adenotonsillar diseases, we developed a cross-sectional study to determine the rates of viral detections of common respiratory viruses detected by TaqMan real time PCR (qPCR) in nasopharyngeal secretions, tonsillar tissues and peripheral blood from 121 children with chronic tonsillar diseases, without symptoms of acute respiratory infections. At least one respiratory virus was detected in 97.5% of patients. The viral co-infection rate was 69.5%. The most frequently detected viruses were human adenovirus in 47.1%, human enterovirus in 40.5%, human rhinovirus in 38%, human bocavirus in 29.8%, human metapneumovirus in 17.4% and human respiratory syncytial virus in 15.7%. Results of qPCR varied widely between sample sites: human adenovirus, human bocavirus and human enterovirus were predominantly detected in tissues, while human rhinovirus was more frequently detected in secretions. Rates of virus detection were remarkably high in tonsil tissues: over 85% in adenoids and close to 70% in palatine tonsils. In addition, overall virus detection rates were higher in more hypertrophic than in smaller adenoids (p = 0.05), and in the particular case of human enteroviruses, they were detected more frequently (p = 0.05) in larger palatine tonsils than in smaller ones. While persistence/latency of DNA viruses in tonsillar tissues has been documented, such is not the case of RNA viruses. Respiratory viruses are highly prevalent in adenoids and palatine tonsils of patients with chronic tonsillar diseases, and persistence of these viruses in tonsils may stimulate chronic inflammation and play a role in the pathogenesis of these diseases.

Nebulized l-epinephrine has been recommended for the treatment of viral croup. However, the few studies assessing its effect on post-extubation stridor (PES) have shown conflicting results. We compared the efficacy and safety of nebulized l-epinephrine at three different doses for the treatment of PES. We conducted a prospective, randomized, double-blind trial including all consecutive children with a PES score of a parts per thousand yen4 (Westley score). The primary efficacy outcome was change in PES score at 40 min. A reduction of a parts per thousand yen2 points in stridor score was defined as clinically significant. A total of 96 patients were randomly assigned to receive one of three doses of nebulized l-epinephrine upon achieving a PES score of 4 or more following extubation. Stridor score and vital signs were recorded before treatment, and at 20, 40, 60 and 180 min after nebulization. Baseline characteristics were similar among all study groups. No significant difference was detected among the treatments based on change in Westley score by intent-to-treat analysis. In addition, the difference in the number of patients who clinically improved among the treatment groups was not significant (p = 0.54). Patients receiving 5 ml nebulized epinephrine had a significant increase of systolic and diastolic blood pressure at 40 and 180 min. Nebulized l-epinephrine at doses of 0.5, 2.5 and 5 ml demonstrated a lack of dose response in effect on PES and a modestly clinically significant increase in undesired side effects (heart rate and blood pressure) at higher doses.

Context. The ESO public survey VISTA variables in the Via Lactea (VVV) started in 2010. VVV targets 562 sq. deg in the Galactic bulge and an adjacent plane region and is expected to run for about five years. Aims. We describe the progress of the survey observations in the first observing season, the observing strategy, and quality of the data obtained. Methods. The observations are carried out on the 4-m VISTA telescope in the ZYJHK(s) filters. In addition to the multi-band imaging the variability monitoring campaign in the K-s filter has started. Data reduction is carried out using the pipeline at the Cambridge Astronomical Survey Unit. The photometric and astrometric calibration is performed via the numerous 2MASS sources observed in each pointing. Results. The first data release contains the aperture photometry and astrometric catalogues for 348 individual pointings in the ZYJHK(s) filters taken in the 2010 observing season. The typical image quality is similar to 0 ''.9-1 ''.0. The stringent photometric and image quality requirements of the survey are satisfied in 100% of the JHK(s) images in the disk area and 90% of the JHK(s) images in the bulge area. The completeness in the Z and Y images is 84% in the disk, and 40% in the bulge. The first season catalogues contain 1.28 x 10(8) stellar sources in the bulge and 1.68 x 10(8) in the disk area detected in at least one of the photometric bands. The combined, multi-band catalogues contain more than 1.63 x 10(8) stellar sources. About 10% of these are double detections because of overlapping adjacent pointings. These overlapping multiple detections are used to characterise the quality of the data. The images in the JHK(s) bands extend typically similar to 4 mag deeper than 2MASS. The magnitude limit and photometric quality depend strongly on crowding in the inner Galactic regions. The astrometry for K-s = 15-18 mag has rms similar to 35-175 mas. Conclusions. The VVV Survey data products offer a unique dataset to map the stellar populations in the Galactic bulge and the adjacent plane and provide an exciting new tool for the study of the structure, content, and star-formation history of our Galaxy, as well as for investigations of the newly discovered star clusters, star-forming regions in the disk, high proper motion stars, asteroids, planetary nebulae, and other interesting objects.

Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Conversely, if the ART strategy is similar or inferior to the current evidence-based strategy (ARDSNet), this should also change current practice as many institutions routinely employ recruitment maneuvers and set PEEP levels according to some titration method.

Active tufas in the form of waterfalls and dams occur along drainage channels in the Serra do Andre Lopes region (State of Sao Paulo, southeastern Brazil) and are associated with the karst system that developed on a dolomitic plateau with a superhumid subtropical climate. The predominance of autogenic waters enables the groundwater to become enriched in calcium carbonate, with low terrigenous sediment content. The tufas that were studied are composed of calcite and have high calcium contents and low magnesium contents. Eroded tufa beds that originate from changes in the position of fluvial channels or river flow rates also occur in this region. In the Sapatu deposit, phytohermal tufas with complex morphologies are arranged in levels constituting various temporally repeated sequences that were deposited between 10,570 and 4,972 cal years BP. In the Frias deposit, distal fluvial deposits of tufa are massive with a relatively greater quantity of terrigenous material and show evidence of dissolution and reprecipitation. The base of this deposit is composed of a cemented breccia dated at 25,390 years BP, which is younger than the overlying tufas ([42,000 years BP). In the two deposits, the levels of terrigenous sediments (quartz sand and lithic pebbles) and terrestrial gastropod shells are interpreted as phases of increased flow rate of rivers during intervals of higher rainfall.

Abstract Background Hepatitis C chronic liver disease is a major cause of liver transplant in developed countries. This article reports the first nationwide population-based survey conducted to estimate the seroprevalence of HCV antibodies and associated risk factors in the urban population of Brazil. Methods The cross sectional study was conducted in all Brazilian macro-regions from 2005 to 2009, as a stratified multistage cluster sample of 19,503 inhabitants aged between 10 and 69 years, representing individuals living in all 26 State capitals and the Federal District. Hepatitis C antibodies were detected by a third-generation enzyme immunoassay. Seropositive individuals were retested by Polymerase Chain Reaction and genotyped. Adjusted prevalence was estimated by macro-regions. Potential risk factors associated with HCV infection were assessed by calculating the crude and adjusted odds ratios, 95% confidence intervals (95% CI) and p values. Population attributable risk was estimated for multiple factors using a case–control approach. Results The overall weighted prevalence of hepatitis C antibodies was 1.38% (95% CI: 1.12%–1.64%). Prevalence of infection increased in older groups but was similar for both sexes. The multivariate model showed the following to be predictors of HCV infection: age, injected drug use (OR = 6.65), sniffed drug use (OR = 2.59), hospitalization (OR = 1.90), groups socially deprived by the lack of sewage disposal (OR = 2.53), and injection with glass syringe (OR = 1.52, with a borderline p value). The genotypes 1 (subtypes 1a, 1b), 2b and 3a were identified. The estimated population attributable risk for the ensemble of risk factors was 40%. Approximately 1.3 million individuals would be expected to be anti-HCV-positive in the country. Conclusions The large estimated absolute numbers of infected individuals reveals the burden of the disease in the near future, giving rise to costs for the health care system and society at large. The known risk factors explain less than 50% of the infected cases, limiting the prevention strategies. Our findings regarding risk behaviors associated with HCV infection showed that there is still room for improving strategies for reducing transmission among drug users and nosocomial infection, as well as a need for specific prevention and control strategies targeting individuals living in poverty.

PO artigo demonstra as interações entre algumas bases de dados internacionais na disponibilização de periódicos da área de Ciência da Informação. Para isso, foram investigadas seis bases de dados com acesso a texto completo dentro da área de Ciência da Informação. Foram elas: Web of Science, Scopus, Library, Information Science & Technology Abstracts, Information Science & Technology Abstracts, Library Literature Information Science – full text e Library Information Science Abstract. No desenvolvimento da pesquisa foram utilizados conceitos, princípios e equações da ‘teoria ingênua dos conjuntos’, o que permitiu observar as bases de dados como conjuntos compostos por elementos, aqui considerados os títulos de periódicos, bem como conferir consistência aos resultados. A análise dos dados foi dividida em três etapas, a saber: a) a primeira priorizou o cruzamento entre as duas maiores bases de dados utilizadas para levantamento bibliográfico (Web of Science e Scopus), seguido do cruzamento com a mais utilizada na Ciência da Informação, a Library and Information Science Abstracts; b) a segunda etapa tratou somente do cruzamento das bases especialistas da área; c) a terceira e última etapa tratou da interseção de todas as bases de dados do estudo. O resultado mostra que 10 títulos de periódicos constituem a interseção dos seis conjuntos de bases de dados. Foi possível identificar, também, o número de periódicos disponibilizados exclusivamente por uma base de dados, demonstrando que as necessidades de pesquisas bibliográficas exaustivas passam, obrigatoriamente, pelo levantamento em todas as bases de dados aqui elencadas.

Este artigo tem por objetivo discutir o desenvolvimento territorial sustentável a partir da atuação de diferentes atores sob a ótica da gestão social e o processo de cidadania deliberativa. Analisa-se a construção do "Programa de Manejo Agroambiental da Bacia do Rio Almada" na região pertencente ao território de cidadania Litoral Sul da Bahia. Organizaram-se os dados, as informações e as entrevistas por meio de estudo de caso. Para construção do caso, foi utilizada a triangulação de técnicas da história oral e do sensemaking. O caso em tela contribui para a reflexão sobre os desafios e oportunidades referentes à gestão social, indicando que o processo de cidadania deliberativa pode se constituir em estratégia necessária para o desenvolvimento territorial sustentável. Assim, fica claro que o diálogo, a participação dos atores na elaboração e implantação de políticas públicas é algo complexo, porém imprescindível, quando se almejam transformações das realidades sociais.

Há evidências crescentes de que o curso Transtorno Afetivo Bipolar (TAB) pode ser modificado por abordagens psicoterápicas, tais como a Psicoeducação. Assim, o objetivo deste trabalho foi identificar as implicações do grupo de Psicoeducação no cotidiano dos portadores. Para tanto, optou-se pelo estudo qualitativo, do tipo Estudo de Caso. Foram incluídos doze portadores de TAB que tiveram pelo menos seis participações no Grupo de Psicoeducação desenvolvido na Faculdade de Medicina de São José do Rio Preto (FAMERP). Foram realizadas entrevistas semi-estruturadas, gravadas, transcritas e trabalhadas por meio da Análise Temática. Este estudo demonstrou que tal experiência grupal favoreceu a aquisição de conhecimento; a conscientização da doença e adesão ao tratamento; a realização de mudanças positivas na vida; a possibilidade de ajudar outros portadores a se beneficiarem do aprendizado construído no grupo; a descoberta de outras realidades e estratégias de enfrentamento, obtidas por meio da troca de experiências entre os participantes.

OBJETIVO: Identificar os acidentes de trabalho com exposição à material biológico ocorridos em um hospital universitário, discutindo os resultados com o processo de implementação das medidas de segurança e saúde dos trabalhadores, exigidas pela Norma Regulamentadora NR-32. MÉTODOS: Estudo exploratório de abordagem quantitativa dos dados. Foram realizados levantamento dos acidentes de trabalho, as entrevistas com o coordenador do Serviço de Segurança e Medicina do Trabalho e a análise de dados documentais do Programa de Prevenção de Riscos Ambientais e do Programa de Controle Médico de Saúde Ocupacional. RESULTADOS: O percentual de acidentes de trabalho reduziu ao longo do período, no qual várias exigências dessa norma foram sendo adotadas. Acidentes com material perfurocortante foram os mais frequentes, não havendo em todos os setores do hospital o oferecimento dos dispositivos de segurança exigidos pela NR-32. CONCLUSÃO: Houve redução de acidentes de trabalho com material biológico no hospital estudado entre 2007 e 2009. Contudo, não é quantitativamente significativa, apesar da implantação de várias diretrizes da NR-32 ao longo dos anos. É necessária a colaboração entre gestores, serviços de segurança e trabalhadores na promoção da saúde no trabalho.

OBJETIVO: este trabalho buscou analisar as percepções quanto à satisfação e expectativas dos usuários dos serviços de saúde de um território atendido pela Estratégia de Saúde da Família (ESF), do município de Bauru, Estado de São Paulo, e assim, promover um direcionamento no acolhimento e atenção dada aos indivíduos ali atendidos. MÉTODO: trata-se de uma pesquisa qualitativa que utilizou a análise do Discurso do Sujeito Coletivo. Fizeram parte da pesquisa 10 usuários da ESF selecionados por meio de amostragem oportunista. Foi utilizado um questionário semi-estruturado e as entrevistas foram gravadas em áudio para posterior análise do discurso. A referida pesquisa foi aprovada pelo Comitê de Ética em Pesquisa (CEP) da FOB/USP bem como pela Secretaria Municipal de Saúde, sob protocolo numero 167/2009.Para aprovação neste CEP, a pesquisa contou com o cadastro no Conselho Nacional de Saúde (CONEP), via Sistema Nacional de Ética em Pesquisa (SISNEP) RESULTADOS: verificou-se com os resultados obtidos que os usuários estão satisfeitos com o atendimento recebido. Além disso, notou-se a presença da participação popular quanto à divisão de responsabilidades no cuidado à saúde. No entanto, apresentaram queixas quanto à lentidão no referenciamento para a média e a alta complexidade. CONCLUSÃO: a Fonoaudiologia concentra parte das suas atividades na média e alta complexidade, níveis citados como insatisfação do usuário. Assim sendo, sugere-se o desenvolvimento desta pesquisa em locais cuja atuação fonoaudiológica esteja presente.
