Background: To ascertain the population rates and proportion of late entry into HIV care, as well as to determine whether such late entry correlates with individual and contextual factors. Methods: Data for the 2003-2006 period in Brazil were obtained from public health records. A case of late entry into HIV care was defined as one in which HIV infection was diagnosed at death, one in which HIV infection was diagnosed after the condition of the patient had already been aggravated by AIDS-related diseases, or one in which the CD4(+) T-cell count was <= 200 cells/mm(3) at the time of diagnosis. We also considered extended and stricter sets of criteria (in which the final criterion was <= 350 cells/mm(3) and <= 100 cells/mm(3), respectively). The estimated risk ratio was used in assessing the effects of correlates, and the population rates (per 100,000 population) were calculated on an annual basis. Results: Records of 115,369 HIV-infected adults were retrieved, and 43.6% (50,358) met the standard criteria for late entry into care. Diagnosis at death accounted for 29% (14,457) of these cases. Late entry into HIV care (standard criterion) was associated with certain individual factors (sex, age, and transmission category) and contextual factors (region with less economic development/increasing incidence of AIDS, lower local HIV testing rate, and smaller municipal population). Use of the extended criteria increased the proportion of late entry by 34% but did not substantially alter the correlations analyzed. The overall population rate of late entry was 9.9/100,000 population, specific rates being highest for individuals in the 30-59 year age bracket, for men, and for individuals living in regions with greater economic development/higher HIV testing rates, collectively accounting for more than half of the cases observed. Conclusions: Although the high proportion of late entry might contribute to spreading the AIDS epidemic in less developed regions, most cases occurred in large cities, with broader availability of HIV testing, and in economically developed regions.

In this work, we present an implementation of quantum logic gates and algorithms in a three effective qubits system, represented by a (I = 7/2) NMR quadrupolar nuclei. To implement these protocols we have used the strong modulating pulses (SMP) and the various stages of each implementation were verified by quantum state tomography (QST). The results for the computational base states, Toffolli logic gates, and Deutsch-Jozsa and Grover algorithms are presented here. Also, we discuss the difficulties and advantages of implementing such protocols using the SMP technique in quadrupolar systems.

Bioenergetic analysis may be applied in order to predict microbial growth yields, based on the Gibbs energy dissipation and mass conservation principles of the overall growth reaction. The bioenergetics of the photoautotrophic growth of the cyanobacterium Arthrospira (Spirulina) platensis was investigated in different bioreactor configurations (tubular photobioreactor and open ponds) using different nitrogen sources (nitrate and urea) and under different light intensity conditions to determine the best growing conditions in terms of Gibbs energy dissipation, number of photons to sustain cell growth and phototrophic energy yields distribution in relation to the ATP and NADPH formation, and release of heat. Although an increase in the light intensity increased the Gibbs energy dissipated for cell growth and maintenance with both nitrogen sources, it did not exert any appreciable influence on the moles of photons absorbed by the system to produce one C-mol biomass. On the other hand, both bioenergetic parameters were higher in cultures with nitrate than with urea, likely because of the higher energy requirements needed to reduce the former nitrogen source to ammonia. They appreciably increased also when open ponds were substituted by the tubular photobioreactor, where a more efficient light distribution ensured a remarkably higher cell mass concentration. The estimated percentages of the energy absorbed by the cell showed that, compared with nitrate, the use of urea as nitrogen source allowed the system to address higher energy fractions to ATP production and light fixation by the photosynthetic apparatus, as well as a lower fraction released as heat. The best energy yields values on Gibbs energy necessary for cell growth and maintenance were achieved in up to 4-5 days of cultivation, indicating that it would be the optimum range to maintain cell growth. Thanks to this better bioenergetic situation, urea appears to be a quite promising low-cost, alternative nitrogen source for Arthrospira platensis cultures in photobioreactors. (C) 2011 Elsevier Ltd.
