Validade de equações antropométricas para estimar a massa muscular em idosos

O presente estudo objetivou validar equações preditivas para estimar a quantidade de massa muscular esquelética (MME) em idosos. A absorciometria radiológica de dupla energia (DXA) foi adotada como referência, e utilizada para estimar a MME apendicular, cujos valores foram comparados àqueles, obtidos por equações preditivas, que utilizam dados antropométricos, idade, etnia e sexo. A concordância entre os métodos foi verificada pelo teste t pareado, pelo coeficiente de correlação de Pearson e pela medida de dispersão dos erros, enquanto a comparação da prevalência de sarcopenia foi analisada pelo coeficiente de Kappa, pela sensibilidade e especificidade. Foram mensurados 180 idosos (120 mulheres e 60 homens), com idade entre 60 e 81 anos. A quantidade de MME, estimada pela equação preditiva de Lee et al., não diferiu daquela obtida pela DXA (p>0,05), e apresentou elevada correlação, tanto em homens (r=0,90; p<0,001), quanto em mulheres (r=0,86; p<0,001), com significância estatística. A prevalência de sarcopenia, também, não diferiu entre os métodos (DXA=33,3% e equação=36,1%) e apresentou elevados valores de concordância (k=0,74; p<0,001), bem como de especificidade (89%) e de sensibilidade (86%). Conclui-se que a equações preditivas, em particular a de Lee et al., são válidas para estimar a quantidade de MME e a prevalência de sarcopenia, em idosos.

Este artigo tem como objetivo analisar o tratamento conferido pelo Estado Novo brasileiro (1937-1945) a Machado de Assis, depois das celebrações oficiais do centenário do autor de Dom Casmurro em 1939. Se, no ano da efeméride, o governo se empenhou em alçar o romancista à condição de maior escritor brasileiro, no início dos anos 1940, no âmbito do Departamento de Imprensa e Propaganda (DIP), o tom passa a ser outro: num contexto de prevalência de certo caráter social e documental da obra de arte, o próprio Getúlio Vargas, o ideólogo Cassiano Ricardo e os principais periódicos estadonovistas fazem menção ao suposto absenteísmo e à falta de "cor local" do fundador da Academia Brasileira de Letras.

Influence of HLA-DRB-1 alleles on the production of antibody against CSP, MSP-1, AMA-1, and DBP in Brazilian individuals naturally infected with Plasmodium vivax

We evaluated the influence of allelic frequency of the human leukocyte antigen (HLA) -DRB1 on the acquisition of antibody response against malaria sporozoite and merozoite peptides in patients with Plasmodium vivax malaria acquired in endemic areas of Brazil. IgG antibodies were detected by enzyme-linked immunosorbent assay against four peptides of circumsporozoite protein (CSP) (amino, carboxyl, and VK210 and VK247 repeats) and peptides of merozoite surface protein 1 (MSP-1), apical membrane antigen 1 (AMA-1), and Duffy-binding protein (DBP). We found an association between HLA-DR3 and HLA-DR5 alleles and lack of antibody response to CSP amino terminal, as well as an association between HILA-DR3 and the highest antibody response to MSP1 (Pv200L). In conclusion, we suggest a potential regulatory role of the H1A-DRB1 alleles in the production of antibodies to a conserved region of P. vivax CSP and MSP1 in Brazilian population exposed to malaria. Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Conversely, if the ART strategy is similar or inferior to the current evidence-based strategy (ARDSNet), this should also change current practice as many institutions routinely employ recruitment maneuvers and set PEEP levels according to some titration method.
