Temporal, spatial and diel variation in the distribution and abundance of organisms is an inherent property of ecological systems. The present study describes these variations and the composition of decapod larvae from the surface waters of St Paul`s Rocks. The expeditions to the archipelago were carried out in April, August and November 2003, March 2004 and May 2005. Surface plankton samples were collected during the morning and dusk periods, inside the inlet and in increasing distances around the archipelago (similar to 150, 700 and 1500 m). The identification resulted in 51 taxa. Seven species, six genera and larvae of the families Pandalidae and Portunidae were identified for the first time in the area. The mean larval density varied from zero to 150.2 +/- 69.6 individuals 100 m(-3) in the waters surrounding the archipelago and from 1.7 +/- 3.0 to 12,827 +/- 15,073 individuals 100 m(-3) inside the inlet. Significant differences on larval density were verified between months and period of the day, but not among the three sites around the archipelago. Cluster and non-metric multidimensional scaling analysis indicated that the decapod larvae community was divided into benthic and pelagic assemblages. Indicator species analysis (ISA) showed that six Brachyura taxa were good indicators for the inlet, while three sergestids were the main species from the waters around the archipelago. These results suggest that St Paul`s Rocks can be divided into two habitats, based on larval composition, density and diversity values: the inlet and the waters surrounding the archipelago.

This paper studies the average control problem of discrete-time Markov Decision Processes (MDPs for short) with general state space, Feller transition probabilities, and possibly non-compact control constraint sets A(x). Two hypotheses are considered: either the cost function c is strictly unbounded or the multifunctions A(r)(x) = {a is an element of A(x) : c(x, a) <= r} are upper-semicontinuous and compact-valued for each real r. For these two cases we provide new results for the existence of a solution to the average-cost optimality equality and inequality using the vanishing discount approach. We also study the convergence of the policy iteration approach under these conditions. It should be pointed out that we do not make any assumptions regarding the convergence and the continuity of the limit function generated by the sequence of relative difference of the alpha-discounted value functions and the Poisson equations as often encountered in the literature. An increasing number of women have been diagnosed with spondyloarthritis (SpA) in recent decades. While a few studies have analyzed gender as a prognostic factor of the disease, no studies have addressed this matter with a large number of patients in South America, which is a peculiar region due to its genetic heterogeneity. The aim of the present study was to analyze the influence of gender on disease patterns in a large cohort of Brazilian patients with SpA. A prospective study was carried out involving 1,505 patients [1,090 males (72.4%) and 415 females (27.6%)] classified as SpA according to the European Spondyloarthropaties Study Group criteria who attended at 29 reference centers for rheumatology in Brazil. Clinical and demographic variables were recorded and the following disease indices were administered: Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Bath Ankylosing Spondylitis Functional Index (BASFI), Bath Ankylosing Spondylitis Radiologic Index (BASRI), Maastricht Ankylosing Spondylitis Enthesitis Score (MASES), and Ankylosing Spondylitis Quality of Life (ASQoL). Ankylosing spondylitis (AS) was the most frequent disease in the group (65.4%), followed by psoriatic arthritis (18.4%), undifferentiated SpA (6.7%), reactive arthritis (3.3%), arthritis associated to inflammatory bowel disease (3.2%), and juvenile SpA (2.9%). The male-to-female ratio was 2.6:1 for the whole group and 3.6:1 for AS. The females were older (p<0.001) and reported shorter disease duration (p=0.002) than the male patients. The female gender was positively associated to peripheral SpA (p<0.001), upper limb arthritis (p<0.001), dactylitis (p=0.011), psoriasis (p<0.001), nail involvement (p<0.001), and family history of SpA (p=0.045) and negatively associated to pure axial involvement (p< 0.001), lumbar inflammatory pain (p=0.042), radiographic sacroiliitis (p<0.001), and positive HLA-B27 (p=0.001). The number of painful (p<0.001) and swollen (p=0.006) joints was significantly higher in the female gender, who also achieved higher BASDAI (p<0.001), BASFI (p=0.073, trend), MASES (p=0.019), ASQoL (p=0.014), and patient's global assessment (p=0.003) scores, whereas the use of nonsteroidal anti-inflammatory drugs (p<0.001) and biological agents (p=0.003) was less frequent in the female gender. Moreover, BASRI values were significantly lower in females (p<0.001). The female gender comprised one third of SpA patients in this large cohort and exhibited more significant peripheral involvement and less functional disability, despite higher values in disease indices.

Abstract Background A descriptive study was carried out in an area of the Atlantic Forest with autochthonous malaria in the Parelheiros subdistrict on the periphery of the municipality of São Paulo to identify anopheline fauna and anophelines naturally infected with Plasmodium as well as to discuss their role in this peculiar epidemiological context. Methods Entomological captures were made from May 2009 to April 2011 using Shannon traps and automatic CDC traps in four areas chosen for their different patterns of human presence and incidences of malaria (anthropic zone 1, anthropic zone 2, transition zone and sylvatic zone). Natural Plasmodium infection was detected by nested PCR based on amplification of the 18S rRNA gene. Results In total, 6,073 anophelines were collected from May 2009 to April 2011, and six species were identified in the four zones. Anopheles cruzii was the predominant species in the three environments but was more abundant in the sylvatic zone. Anopheles (Kerteszia) cruzii specimens from the anthropic and sylvatic zones were positive for P. vivax and P. malariae. An. (Ker.) bellator, An. (Nys.) triannulatus, An. (Nys.) strodei, An. (Nys.) lutzi and An. (Ano) maculipes were found in small numbers. Of these, An. (Nys.) triannulatus and An. (Nys.) lutzi, which were collected in the anthropic zone, were naturally infected with P. vivax while An. (Nys.) triannulatus from the anthropic zones and An. (Nys.) strodei from the transition zone were positive for P. malariae. Conclusion These results confirm that Anopheles (Kerteszia) cruzii plays an important role as a major Plasmodium vector. However, the finding of other naturally infected species may indicate that secondary vectors are also involved in the transmission of malaria in the study areas. These findings can be expected to help in the implementation of new measures to control autochthonous malaria in areas of the Atlantic Forest.

OBJETIVO: Medir e caracterizar a carga de trabalho de enfermagem em Unidade de Terapia Intensiva (UTI) por meio da aplicação do Nursing Activities Score (NAS). MÉTODOS: Estudo descritivo quantitativo, retrospectivo, realizado em uma das UTIs de um Hospital Filantrópico de Teresina- PI, de setembro a outubro de 2010, com amostra de 66 pacientes. Foram realizadas 285 medidas do escore NAS. RESULTADOS: Quanto à carga de trabalho de enfermagem, foi verificada uma média do escore total do NAS de 68,1% (51,5% e 108,3%), correspondendo à porcentagem de tempo gasto pelo profissional de enfermagem na assistência direta ao paciente nas 24 horas. Houve correlação estatística entre NAS e desfecho clínico (p= 0,001). Já entre NAS e tempo de internação (p= 0,073) e NAS e idade (p=0,952), não houve significância estatística. CONCLUSÃO: Os resultados mostraram que os pacientes apresentaram elevada necessidade de cuidados, refletida pela média elevada do NAS.

OBJETIVO: avaliar a memória operacional fonológica e relacionar com a impulsividade de pacientes em tratamento no Centro de Atenção Integrada à Saúde Mental. MÉTODO: 29 usuários: 21 do gênero masculino e 8 do feminino, usuários de substâncias psicoativas, com 37,9±10,5 anos de idade e 10,59±3,53 anos de escolaridade; e 30 voluntários: 19 do gênero masculino e 11 do feminino, com 32,4±11,9 anos de idade e 11,07±3,29 anos de escolaridade, sem histórico psiquiátrico ou de dependência química foram convocados à avaliação de: 1) memória operacional para palavras e pseudo-palavras; 2) impulsividade em seus fatores de segunda ordem (impulsividade atencional, motora e de não planejamento). RESULTADOS: o desempenho dos usuários de substâncias psicoativas na avaliação da memória em comparação ao grupo controle foi pior tanto no span auditivo de palavras e pseudo-palavras como também no número total de recordação de palavras e pseudo-palavras. Na avaliação da impulsividade, os usuários apresentaram escores elevados em contraposição aos sujeitos controle em todos os subtipos de impulsividade, inclusive no total. Na análise de correlação dos dados não foram encontradas relações entre os escores de impulsividade e memória. CONCLUSÃO: : este padrão de respostas indica comprometimento da memória operacional fonológica provavelmente independente do alto nível de impulsividade apresentado pelos usuários de drogas. Estas análises contribuem para propor estratégias de tratamento direcionadas às alterações detectadas.
