A new species of phlebotomine sand fly found in Minas Gerais state, Brazil, is described and illustrated based on male and female morphological characteristics. The new species belongs to the rupicola series within the subgenus Evandromyia Mangabeira. Both sexes are morphologically close to Evandromyia rupicola (Martins, Godoy & Silva) and Evandromyia grimaldii Andrade Filho, Pinto, Santos & Carvalho and the characteristics for their classification are here presented.

Insulin-like growth factor type 1 (IGF1) is a mediator of growth hormone (GH) action, and therefore, IGF1 is a candidate gene for recombinant human GH (rhGH) pharmacogenetics. Lower serum IGF1 levels were found in adults homozygous for 19 cytosine-adenosine (CA) repeats in the IGF1 promoter. The aim of this study was to evaluate the influence of (CA)n IGF1 polymorphism, alone or in combination with GH receptor (GHR)-exon 3 and -202 A/C insulin-like growth factor binding protein-3 (IGFBP3) polymorphisms, on the growth response to rhGH therapy in GH-deficient (GHD) patients. Eighty-four severe GHD patients were genotyped for (CA) n IGF1, -202 A/C IGFBP3 and GHR-exon 3 polymorphisms. Multiple linear regressions were performed to estimate the effect of each genotype, after adjustment for other influential factors. We assessed the influence of genotypes on the first year growth velocity (1st y GV) (n = 84) and adult height standard deviation score (SDS) adjusted for target-height SDS (AH-TH SDS) after rhGH therapy (n = 37). Homozygosity for the IGF1 19CA repeat allele was negatively correlated with 1st y GV (P = 0.03) and AH-TH SDS (P = 0.002) in multiple linear regression analysis. In conjunction with clinical factors, IGF1 and IGFBP3 genotypes explain 29% of the 1st y GV variability, whereas IGF1 and GHR polymorphisms explain 59% of final height-target-height SDS variability. We conclude that homozygosity for IGF1 (CA) 19 allele is associated with less favorable short-and long-term growth outcomes after rhGH treatment in patients with severe GHD. Furthermore, this polymorphism exhibits a non-additive interaction with -202 A/C IGFBP3 genotype on the 1st y GV and with GHR-exon 3 genotype on adult height. Background: Caesarean section rates in Brazil have been steadily increasing. In 2009, for the first time, the number of children born by this type of procedure was greater than the number of vaginal births. Caesarean section is associated with a series of adverse effects on the women and newborn, and recent evidence suggests that the increasing rates of prematurity and low birth weight in Brazil are associated to the increasing rates of Caesarean section and labour induction. Methods: Nationwide hospital-based cohort study of postnatal women and their offspring with follow-up at 45 to 60 days after birth. The sample was stratified by geographic macro-region, type of the municipality and by type of hospital governance. The number of postnatal women sampled was 23,940, distributed in 191 municipalities throughout Brazil. Two electronic questionnaires were applied to the postnatal women, one baseline face-to-face and one follow-up telephone interview. Two other questionnaires were filled with information on patients' medical records and to assess hospital facilities. The primary outcome was the percentage of Caesarean sections (total, elective and according to Robson's groups). Secondary outcomes were: post-partum pain; breastfeeding initiation; severe/near miss maternal morbidity; reasons for maternal mortality; prematurity; low birth weight; use of oxygen use after birth and mechanical ventilation; admission to neonatal ICU; stillbirths; neonatal mortality; readmission in hospital; use of surfactant; asphyxia; severe/near miss neonatal morbidity. The association between variables were investigated using bivariate, stratified and multivariate model analyses. Statistical tests were applied according to data distribution and homogeneity of variances of groups to be compared. All analyses were taken into consideration for the complex sample design. Discussion: This study, for the first time, depicts a national panorama of labour and birth outcomes in Brazil. Regardless of the socioeconomic level, demand for Caesarean section appears to be based on the belief that the quality of obstetric care is closely associated to the technology used in labour and birth. Within this context, it was justified to conduct a nationwide study to understand the reasons that lead pregnant women to submit to Caesarean sections and to verify any association between this type of birth and it's consequences on postnatal health.

Objective To perform systematic assessment of ovarian reserve markers using a combination of tests in juvenile systemic lupus erythematosus (JSLE) patients without amenorrhoea. Methods Twenty-seven consecutive JSLE female patients and 13 healthy controls without amenorrhoea were evaluated for 6 months. Ovarian reserve was assessed during early follicular phase by serum levels of follicle stimulating hormone (FSH), luteinising hormone (LH), estradiol, inhibin A, inhibin B and anti-Mullerian hormone (AMH). Ovarian size was measured by abdominal ultrasonography. Demographic data, disease activity, damage and treatment were also analysed. Results The median of current age was similar in ISLE patients and controls (16.5 vs. 15years, p=0.31) with a significantly higher age at menarche (13 vs. 12years, p=0.03). A trend of lower median total antral follicle count was observed in JSLE compared to controls (9 vs. 14.5, p=0.062) with similar median of other ovarian reserve parameters (p>0.05). Further evaluation of patients treated with cyclophosphamide and those without this treatment revealed a higher median FSH levels (6.4 vs. 4.6 IU/L, p=0.023). Inhibin B, AMH levels and ovarian volume were also lower but did not reach statistical significance (10.8 vs. 27.6 pg/mL, p=0.175; 0.6 vs. 1.5 ng/mL, p=0.276; 3.4 vs. 5 cm(3), p=0.133; respectively). LH (2.7 vs. 2.9 IU/L, p=0.43), estradiol (50 vs. 38 pg/mL, p=0.337) and inhibin A (1.1 vs. 0 pg/mL, p=0.489) levels were comparable in both groups. Conclusions Our study suggests that ovarian reserve after cyclophosphamide treatment may be hampered in spite of the presence of menstrual cycles emphasising the relevance of gonadal protection during the use of this alkylating agent.

Aims and objectives. To evaluate the effectiveness of a low-level laser therapy for pain relief in the perineum following episiotomy during childbirth. Background. Laser irradiation is a painless and non-invasive therapy for perineal pain treatment and its effects have been investigated in several studies, with no clear conclusion on its effectiveness. Design. A double-blind randomised controlled clinical trial. Method. One hundred and fourteen women who underwent right mediolateral episiotomies during vaginal birth in an in-hospital birthing centre in Sao Paulo, Brazil and reported pain =3 on a numeric scale (010) were randomised into three groups of 38 women each: two experimental groups (treated with red and infrared laser) and a control group. The experimental groups were treated with laser applied at three points directly on the episiotomy after suturing in a single session between 656 hours postpartum. We used a diode laser with wavelengths of 660 nm (red laser) and 780 nm (infrared laser). The control group participants underwent all laser procedures, excluding the emission of irradiation. The participants and the pain scores evaluator were blinded to the type of intervention. The perineal pain scores were assessed at three time points: before, immediately after and 30 minutes after low-level laser therapy. Results. The comparison of perineal pain between the three groups showed no significant differences in the three evaluations (p = 0.445), indicating that the results obtained in the groups treated with low-level laser therapy were equivalent to the control group. Conclusions. Low-level laser therapy did not decrease the intensity of perineal pain reported by women who underwent right mediolateral episiotomy. Relevance to clinical practice. The effect of laser in perineal pain relief was not demonstrated in this study. The dosage may not have been sufficient to provide relief from perineal pain after episiotomy during a vaginal birth.

The main objective of this paper was to discuss the importance of the reverse planning as guidance to surgical procedures on immediate loading implant-retained prosthesis, presenting a case report. Such design aims to define anchorage orientation after determination of prosthetic design. This is quite important because the possibility of success of cases with this kind of planning may achieve high levels of satisfaction. Another important aspect is the possibility of fast and definitive restoration of patients’ function and aesthetics

This paper discusses the effects of the internationalization of the Colombian armed conflict, understood as the participation of foreign actors in programs regarding the resolution of the conflict. Through an analysis of the involvement of multilateral and state actors, the authors argue that this process involved the dilemma between unilateralism and multilateralism. The empirical analysis is centered on the intervention of conflict mediation mechanisms from the Organization of American States (OAS), specifically the Mission to Support the Peace Process (MAPP/OAS), whose scope is aimed at demobilizing paramilitary groups in Colombia.

Objetivou-se, com este trabalho, avaliar a influência das variáveis ambientais nos níveis de ruídos emitidos por suínos e quantificar as faixas em dB comparativamente às condições de conforto térmico estabelecidas pela literatura. O experimento foi conduzido em câmara climática, onde foram alojados cinco leitões em fase de creche, submetidos à variação na temperatura ambiente de 20°C a 38°C e umidade relativa de 50% a 80%. Decibelímetros foram instalados para o registro dos níveis de ruídos e sensores dataloggers para os dados de temperatura e umidade relativa. O nível de atividade foi utilizado para quantificar a movimentação dos animais por intermédio de análise de imagens. Análises de correlação e regressão foram aplicadas nos dados para análise estatística. As variáveis ambientais influenciam na emissão de ruídos pelos leitões quando expostos a diferentes condições térmicas. Os níveis de ruídos foram estabelecidos em faixas de acordo com a condição térmica a que animais foram submetidos. Para a condição de conforto (20 a 23°C), níveis de ruídos na faixa de 70 a 75dB; condição de alerta (23 a 30°C), níveis de ruídos na faixa de 60 a 70dB e para condição de estresse térmico (acima de 30°C), na faixa de 55 a 60dB.

The β-adrenergic agonist ractopamine is increasingly used in the swine industry due to higher consumer demand for leaner pork products. Redirecting nutrients to favor leanness rather than fat deposition, ractopamine improves growth and carcass traits of finishing pigs. However, the impact of this agonist on pork quality is not clearly defined. Understanding the biological effects of dietary ractopamine dose, treatment period, lysine levels, and the lysine to metabolizable energy ratio will help pork producers achieve improvements in animal performance, carcass leanness, and economic efficiency in swine production systems.

Analisa-se a formação de nossas fronteiras apresentando inicialmente a questão da demarcação da linha de Tordesilhas e os problemas que levaram ao abandono desse traçado para adotar uma configuração que respeitasse tanto a posse efetiva do território (uti possidetis) como as divisas naturais: rios e divisores de água. A seguir examina-se o Mapa das Cortes que, tendo servido como base para o Tratado de Madri, determinou na prática a configuração atual de nosso país. É feita uma análise desse documento cartográfico, com o auxílio da Cartografia digital, que permitiu quantificar as distorções existentes, afirmar seu caráter propositado e descobrir a forma como foi construído.

INTRODUÇÃO: As cirurgias bariátricas são cada vez mais frequentes no tratamento dos pacientes portadores de obesidade mórbida. A grande perda ponderal decorrente desses procedimentos causa excesso de pele e de tecido subcutâneo em todo o corpo, especialmente na região abdominal. O objetivo deste trabalho é apresentar a técnica de abdominoplastia vertical como alternativa para a ressecção do excesso de pele nesses pacientes. MÉTODO: Foram avaliados, retrospectivamente, os prontuários de 40 pacientes submetidos ao procedimento entre 2004 e 2009. O grau de satisfação dos pacientes foi avaliado através de escala subjetiva, com notas variando de 0 a 10. RESULTADOS: Vinte e cinco por cento dos pacientes apresentaram pequenas complicações, sendo 3 seromas, 3 pequenas deiscências e 5 cicatrizes hipertróficas, todas tratadas ambulatorialmente. Dois terços dos pacientes relataram alto grau de satisfação, considerando seus resultados bons (notas 7 ou 8) ou ótimos (notas 9 ou 10). CONCLUSÕES: A técnica de abdominoplastia vertical aparece como uma nova opção para o tratamento do excesso de pele abdominal em pacientes ex-obesos.
