Background Complex medication regimens may adversely affect compliance and treatment outcomes. Complexity can be assessed with the medication regimen complexity index (MRCI), which has proved to be a valid, reliable tool, with potential uses in both practice and research. Objective To use the MRCI to assess medication regimen complexity in institutionalized elderly people. Setting Five nursing homes in mainland Portugal. Methods A descriptive, cross-sectional study of institutionalized elderly people (n = 415) was performed from March to June 2009, including all inpatients aged 65 and over taking at least one medication per day. Main outcome measure Medication regimen complexity index. Results The mean age of the sample was 83.9 years (±6.6 years), and 60.2 % were women. The elderly patients were taking a large number of drugs, with 76.6 % taking more than five medications per day. The average medication regimen complexity was 18.2 (±SD = 9.6), and was higher in the females (p < 0.001). The most decisive factors contributing to the complexity were the number of drugs and dosage frequency. In regimens with the same number of medications, schedule was the most relevant factor in the final score (r = 0.922), followed by pharmaceutical forms (r = 0.768) and additional instructions (r = 0.742). Conclusion Medication regimen complexity proved to be high. There is certainly potential for the pharmacist’s intervention to reduce it as part as the medication review routine in all the patients.

O objetivo desta investigação é testar o contributo das necessidades interpessoais - os sentimentos de não pertença e a perceção de ser um fardo – para o risco de suicídio, avaliado através da ideação suicida. Também se pretende investigar a possível interação entre as necessidades interpessoais e respetivos efeitos quadráticos, controlando o impacto de um conjunto de variáveis que apresentam, muitas vezes, uma correlação significativa com a ideação suicida. Neste estudo participaram 80 utentes idosos em recuperação de uma doença aguda. De acordo com os resultados, os sentimentos de não pertença e a perceção de ser um fardo correlacionam-se com a ideação suicida. No entanto, quando numa análise da regressão múltipla se considera simultaneamente o efeito de ambas as variáveis, apenas os sentimentos de não pertença contribuem significativamente na previsão da ideação suicida. Não se verificaram efeitos de interação nem efeitos quadráticos entre as variáveis necessidades interpessoais; Abstract: “Interpersonal needs and suicide risk in a sample of elderly patients” The aim of this study is to test the contribution of interpersonal needs - thwarted belongingness and perceived burdensomeness - to the risk of suicide, assessed by the presence of suicidal ideation. It also intends to investigate the possible interaction between interpersonal needs and the respective quadratic effects, by controlling the impact of a set of variables which have, many times, a significant correlation with suicidal ideation. In this study participated 80 elderly patients recovering from an acute medical condition. According to the results, thwarted belongingness and perceived burdensomeness are correlated with suicidal ideation. However, when in a multiple regression analysis we considerate simultaneously the effect of both variables, only thwarted belongingness contributes significatively to suicide ideation prediction. No interactions effects or quadratic effects were observed between interpersonal needs variables.

Objectives: To adapt for the Portuguese language the Self-Care of Heart Failure Index V6.2, analyse the self-care in maintenance, management skills and self-confidence of patients with heart failure who attend the nursing consultation of two Hospitals. Method: Explo- ratory study, sample 110 patients who frequented the nursing consultation for patients with heart failure of two Portuguese hospitals, conducted in the period of 6 months. He appea- led to the descriptive statistics and psychometric tests. Results: Internal consistency similar to the original scale. Mostly elderly, low literacy patients in self-care, low values associated with physical activity to the control of salt in meals taken away from home and inadequate control of signs and symptoms. Conclusion: Patients’ present difficulties in maintenance and management of the disease are self-confident face anyway. This instrument allows for individualized assessment leading to decision-making and action adjusted.
