Descentralización de los sistemas de salud: revisión sistemática de la literatura y estudio de caso en Guinea Ecuatorial

Las intervenciones dirigidas a fortalecer los sistemas de salud, en sus distintas tipologías y formas, han demostrado ser una herramienta esencial para mejorar y mantener la salud de la población. A pesar de esto, no está claro qué modelo de sistema de salud es más efectivo o qué organización estructural mejora la gobernanza global del sistema. Las reformas del sector salud basadas en atención primaria de salud (APS) han sido posiblemente las más extendidas de los últimos 30 años. La estrategia de APS se reconoció como el pilar fundamental para la consecución del objetivo de salud para todos en el año 2000, y se centró en dos elementos principales que se añadían al modelo de cuidados: la promoción de la salud basada en la lucha contra los determinantes sociales de la salud, y la descentralización de los servicios de salud asociado a participación comunitaria. Mientras la eficacia de las estrategias basadas en atención primaria ha sido ampliamente demostrada tanto en países de renta alta como en países de bajos recursos, no existe consenso entre los investigadores sobre si la descentralización, de forma aislada, es una reforma que mejora el desempeño de los sistemas de salud o si es una estrategia adecuada para países con bajos recursos. A pesar de ello, la mayor parte de los países africanos se embarcaron en los años 80 en procesos de descentralización a diferentes escalas con el aumentar la sostenibilidad del sistema y mejorar su eficiencia. Rafael Azcona y José Luis Cuerda, literatura y cine

Los tres trabajos realizados por Rafael Azcona con José Luis Cuerda son adaptaciones al cine de textos literarios. Recordarlos nos ofrece una buena ocasión de revisar algunas ideas sobre la conexión de ambos mundos, a partir del análisis de tres textos literarios (Fernández Flórez, Manuel Rivas, Alberto Méndez); y de tres guiones cinematográficos, adaptados por Rafael Azcona, de tres películas realizadas por José Luis Cuerda.

Los jardines "pintados" de Émile Zola, lugares de encuentro entre literatura y arte

The thesis here presented, entitled “PAINTED” GARDENS BY ÉMILE ZOLA, MEETING PLACES OF LITERATURE AND ART, aims to analyze the treatment of the theme of garden in La Curée, the second of the twenty novels that make up the cycle of Les Rougon-Macquart, series published between 1871 and 1893. In this story, the author, an ardent republican, bluntly attacks the imperial regime, emphasizing the confusion of high society, the immorality of speculation occurred under the Haussmann's renovation of Paris, the profusion of luxury and pleasure, and, in short, a dynamism geared exclusively to the sense of destruction or perversion. Hence we have devoted an entire chapter to argue the presentation of this novel as a chronicle of Parisian society of the Second Empire. Also, because reading Zola is to recognize the profusion of metaphorical language, the analysis of the poetic discourse has played an important role in the body of this thesis. This analysis focuses on the two major descriptions of the greenhouse of hôtel Saccard, in chapters I and IV, both aiming to present this garden as a disturbing space, favorable to the discovery of unsuspected amorous capabilities, according to the mentality of the nineteenth century, so full of puritan principles, for which the tropical world of greenhouses meant a sexual freedom which did not understand bans or limitations. The sexual corruption that occurs in this greenhouse, is not only indicative of moral laxity in personal relationships, but also works as a symbolic image of decay and breakdown of society as a whole...

Literatura y pensamiento en Francia. Un ejemplo : el panfleto

First of all, with this dissertation we have set as principal goal to define and to characterize the pamphlet in a French-speaking context. To do this, we will try to delimit this type of text and to demonstrate that in many occasions this term is used for classifying documents whose principal characteristic is the polemic. In the approach to this dissertation, we have mainly found two problems: on the one hand to define what a pamphlet is and to demarcate it from other similar texts both from the point of view of the content as well as the form; and on the other hand, to try to discern if the pamphlet can be included under the title of Literature and if it constitutes a literary genre for itself. To do so, we will structure our investigation in several chapters gathered in two clearly delimited parts. At the end of each of them, we will draw some brief conclusions. The first part will be focused on the study of theoretical notions. We will approach this reflection from different perspectives: etymological, comparative, literary and generic. To do this, we will approach the theories of those schools or of those authors who are more convenient to our purpose, so both our matter of study and the areas where we want to study it, raise many problems of interpretation and classification.
