In this paper we examine the use of electronic patient records (EPR) by clinical specialists in their development of multidisciplinary care for diagnosis and treatment of breast cancer. We develop a practice theory lens to investigate EPR use across multidisciplinary team practice. Our findings suggest that there are oppositional tendencies towards diversity in EPR use and unity which emerges across multidisciplinary work, and this influences the outcomes of EPR use. The value of this perspective is illustrated through the analysis of a year-long, longitudinal case study of a multidisciplinary team of surgeons, oncologists, pathologists, radiologists, and nurse specialists adopting a new EPR. Each group adapted their use of the EPR to their diverse specialist practices, but they nonetheless orientated their use of the EPR to each others' practices sufficiently to support unity in multidisciplinary teamwork. Multidisciplinary practice elements were also reconfigured in an episode of explicit negotiations, resulting in significant changes in EPR use within team meetings. Our study contributes to the growing literature that questions the feasibility and necessity of achieving high levels of standardized, uniform health information technology use in healthcare.

The fastest ever 11.25Gb/s real-time FPGA-based optical orthogonal frequency division multiplexing (OOFDM) transceivers utilizing 64-QAM encoding/decoding and significantly improved variable power loading are experimentally demonstrated, for the first time, incorporating advanced functionalities of on-line performance monitoring, live system parameter optimization and channel estimation. Real-time end-to-end transmission of an 11.25Gb/s 64-QAM-encoded OOFDM signal with a high electrical spectral efficiency of 5.625bit/s/Hz over 25km of standard and MetroCor single-mode fibres is successfully achieved with respective power penalties of 0.3dB and -0.2dB at a BER of 1.0 x 10(-3) in a directly modulated DFB laser-based intensity modulation and direct detection system without in-line optical amplification and chromatic dispersion compensation. The impacts of variable power loading as well as electrical and optical components on the transmission performance of the demonstrated transceivers are experimentally explored in detail. In addition, numerical simulations also show that variable power loading is an extremely effective means of escalating system performance to its maximum potential.

Despite use of the best in current design practices, high-speed shaft (HSS) bearings, in a wind-turbine gearbox, continue to exhibit a high rate of premature failure. As HSS bearings operate under low loads and high speeds, these bearings are prone to skidding. However, most of the existing methods for analyzing skidding are quasi-static in nature and cannot be used to study dynamic operating conditions. This paper proposes a dynamic model, which includes gyroscopic and centrifugal effects, to study the skidding characteristics of angular-contact ball-bearings. Traction forces between rolling-elements and raceways are obtained using elastohydrodynamic (EHD) lubrication theory. Underlying gross-sliding mechanisms for pure axial loads, and combined radial and axial loads are also studied. The proposed model will enable engineers to improve bearing reliability at the design stage, by estimating the amount of skidding. © 2011 Published under licence by IOP Publishing Ltd.

The last few years have seen considerable progress in pedestrian detection. Recent work has established a combination of oriented gradients and optic flow as effective features although the detection rates are still unsatisfactory for practical use. This paper introduces a new type of motion feature, the co-occurrence flow (CoF). The advance is to capture relative movements of different parts of the entire body, unlike existing motion features which extract internal motion in a local fashion. Through evaluations on the TUD-Brussels pedestrian dataset, we show that our motion feature based on co-occurrence flow contributes to boost the performance of existing methods. © 2011 IEEE.

To explore the machining characteristics of glassy carbon by focused ion beam (FIB), particles induced by FIB milling on glassy carbon have been studied in the current work. Nano-sized particles in the range of tens of nanometers up to 400 nm can often be found around the area subject to FIB milling. Two ion beam scanning modes - slow single scan and fast repetitive scan - have been tested. Fewer particles are found in single patterns milled in fast repetitive scan mode. For a group of test patterns milled in a sequence, it was found that a greater number of particles were deposited around sites machined early in the sequence. In situ EDX analysis of the particles showed that they were composed of C and Ga. The formation of particles is related to the debris generated at the surrounding areas, the low melting point of gallium used as FIB ion source and the high contact angle of gallium on glassy carbon induces de-wetting of Ga and the subsequent formation of Ga particles. Ultrasonic cleaning can remove over 98% of visible particles. The surface roughness (Ra) of FIB milled areas after cleaning is less than 2 nm. © 2010.
