Do hospitals experience safety tipping points as utilization increases, and if so, what are the implications for hospital operations management? We argue that safety tipping points occur when managerial escalation policies are exhausted and workload variability buffers are depleted. Front-line clinical staff is forced to ration resources and, at the same time, becomes more error prone as a result of elevated stress hormone levels. We confirm the existence of safety tipping points for in-hospital mortality using the discharge records of 82,280 patients across six high-mortality-risk conditions from 256 clinical departments of 83 German hospitals. Focusing on survival during the first seven days following admission, we estimate a mortality tipping point at an occupancy level of 92.5%. Among the 17% of patients in our sample who experienced occupancy above the tipping point during the first seven days of their hospital stay, high occupancy accounted for one in seven deaths. The existence of a safety tipping point has important implications for hospital management. First, flexible capacity expansion is more cost-effective for safety improvement than rigid capacity, because it will only be used when occupancy reaches the tipping point. In the context of our sample, flexible staffing saves more than 40% of the cost of a fully staffed capacity expansion, while achieving the same reduction in mortality. Second, reducing the variability of demand by pooling capacity in hospital clusters can greatly increase safety in a hospital system, because it reduces the likelihood that a patient will experience occupancy levels beyond the tipping point. Pooling the capacity of nearby hospitals in our sample reduces the number of deaths due to high occupancy by 34%.

The response of submerged slopes on the continental shelf to seismic or storm loading has become an important element in the risk assessment for offshore structures and "local" tsunami hazards worldwide. The geological profile of these slopes typically includes normally consolidated to lightly overconsolidated soft cohesive soils with layer thickness ranging from a few meters to hundreds of meters. The factor of safety obtained from pseudo-static analyses is not always a useful measure for evaluating the slope response, since values less than one do not necessarily imply slope failure with large movements of the soil mass. This paper addresses the relative importance of different factors affecting the response of submerged slopes during seismic loading. The analyses use a dynamic finite element code which includes a constitutive law describing the anisotropic stress-strain-strength behavior of normally consolidated to lightly overconsolidated clays. The model also incorporates anisotropic hardening to describe the effect of different shear strain and stress histories as well as bounding surface principles to provide realistic descriptions of the accumulation of the plastic strains and excess pore pressure during successive loading cycles. The paper presents results from parametric site response analyses on slope geometry and layering, soil material parameters, and input ground motion characteristics. The predicted maximum shear strains, permanent deformations, displacement time histories and maximum excess pore pressure development provide insight of slope performance during a seismic event. © 2006 Author(s). Four fast reactor concepts using lead (LFR), liquid salt, NaCl-KCl-MgCl2 (LSFR), sodium (SFR), and supercritical CO2 (GFR) coolants are compared. Since economy of scale and power conversion system compactness are the same by virtue of the consistent 2400 MWt rating and use of the S-CO2 power conversion system, the achievable plant thermal efficiency, core power density and core specific powers become the dominant factors. The potential to achieve the highest efficiency among the four reactor concepts can be ranked from highest to lowest as follows: (1) GFR, (2) LFR and LSFR, and (3) SFR. Both the lead- and salt-cooled designs achieve about 30% higher power density than the gas-cooled reactor, but attain power density 3 times smaller than that of the sodium-cooled reactor. Fuel cycle costs are favored for the sodium reactor by virtue of its high specific power of 65 kW/kgHM compared to the lead, salt and gas reactor values of 45, 35, and 21 kW/kgHM, respectively. In terms of safety, all concepts can be designed to accommodate the unprotected limiting accidents through passive means in a self-controllable manner. However, it does not seem to be a preferable option for the GFR where the active or semi-passive approach will likely result in a more economic and reliable plant. Lead coolant with its superior neutronic characteristics and the smallest coolant temperature reactivity coefficient is easiest to design for self-controllability, while the LSFR requires special reactivity devices to overcome its large positive coolant temperature coefficient. The GFR required a special core design using BeO diluent and a supercritical CO2 reflector to achieve negative coolant void worth-one of the conditions necessary for inherent shutdown following large LOCA. Protected accidents need to be given special attention in the LSFR and LFR due to the small margin to freezing of their coolants, and to a lesser extent in the SFR. © 2009 Elsevier B.V. Customer feedback is normally fed into product design and engineering via quality surveys and therefore mainly comprises negative comments: complaints about things gone wrong. Whilst eradication of such problems will result in a feeling of satisfaction in existing customers, it will not instil the sense of delight required to attract conquest buyers. CUPID's aim is to conceive and evaluate ideas to stimulate product desirability through the provision of delightful features and execution. By definition, surprise and delight features cannot be foreseen, so we have to understand sensory appeal and, therefore, the "hidden" voice of the customer.
