At present, many countries allow citizens or entities to interact with the government outside the telematic environment through a legal representative who is granted powers of representation. However, if the interaction takes place through the Internet, only primitive mechanisms of representation are available, and these are mainly based on non-dynamic offline processes that do not enable quick and easy identity delegation. This paper proposes a system of dynamic delegation of identity between two generic entities that can solve the problem of delegated access to the telematic services provided by public authorities. The solution herein is based on the generation of a delegation token created from a proxy certificate that allows the delegating entity to delegate identity to another on the basis of a subset of its attributes as delegator, while also establishing in the delegation token itself restrictions on the services accessible to the delegated entity and the validity period of delegation. Further, the paper presents the mechanisms needed to either revoke a delegation token or to check whether a delegation token has been revoked. Implications for theory and practice and suggestions for future research are discussed.

World Health Organization actively stresses the importance of health, nutrition and well-being of the mother to foster children development. This issue is critical in the rural areas of developing countries where monitoring of health status of children is hardly performed since population suffers from a lack of access to health care. The aim of this research is to design, implement and deploy an e-health information and communication system to support health care in 26 rural communities of Cusmapa, Nicaragua. The final solution consists of an hybrid WiMAX/WiFi architecture that provides good quality communications through VoIP taking advantage of low cost WiFi mobile devices. Thus, a WiMAX base station was installed in the health center to provide a radio link with the rural health post "El Carrizo" sited 7,4 km. in line of sight. This service makes possible personal broadband voice and data communication facilities with the health center based on WiFi enabled devices such as laptops and cellular phones without communications cost. A free software PBX was installed at "San José de Cusmapa" health care site to enable communications for physicians, nurses and a technician through mobile telephones with IEEE 802.11 b/g protocol and SIP provided by the project. Additionally, the rural health post staff (midwives, brigade) received two mobile phones with these same features. In a complementary way, the deployed health information system is ready to analyze the distribution of maternal-child population at risk and the distribution of diseases on a geographical baseline. The system works with four information layers: fertile women, children, people with disabilities and diseases. Thus, authorized staff can obtain reports about prenatal monitoring tasks, status of the communities, malnutrition, and immunization control. Data need to be updated by health care staff in order to timely detect the source of problem to implement measures addressed to alleviate and improve health status population permanently. Ongoing research is focused on a mobile platform that collects and automatically updates in the information system, the height and weight of the children locally gathered in the remote communities. This research is being granted by the program Millennium Rural Communities of the Technical University of Madrid.

The demand for electronic identity has grown as a result of governments? promotion of e-Government, in which the citizen-public administration relationship often has a strictly personal nature and requires digital identification systems that are univocal, secure, and global. The management of this identity by public administrations is an important challenge, accentuated when interoperability among public administrations of different countries become necessary. In this paper current trends in pan-euroean identity management systems are analysized and a outlook of the future European scenary is shown.

This issue's Works-In-Progress department has four entries related to the issue's theme, Information and Communication Technologies for Development (ICTD). They are “Sustainable ICT in Agricultural Value Chains”, “Measuring Social Inclusion in Primary Schools”, “An Architecture for Green Mobile Computation”, and “Improving Communication in Resource-Poor Settings”. A fifth entry, “mFeel: An Affective Mobile System”, covers the mFeel mobile system, which combines context awareness with affective and cognitive techniques.

Los Objetivos de Desarrollo del Milenio señalan la importancia de promover el conocimiento del estado de salud materno-infantil, en especial en las zonas más desprotegidas. El presente artículo detalla las acciones llevadas a cabo durante los años 2008 y 2011 en algunas comunidades rurales de Nicaragua con el objetivo de brindar un servicio de comunicaciones de bajo coste. Dicho servicio es administrado por el centro de salud de Cusmapa, que lidera el programa y mantiene una estrecha comunicación entre su personal y los brigadistas de salud de 26 comunidades aisladas. Los agentes locales, a través de la red de datos y voz WiMAX/WiFi instalada, pueden utilizar el sistema de comunicaciones para informar acerca de episodios urgentes de salud materno-infantil a fin de que sean evaluados o atendidos por dos médicos y seis enfermeras que se encuentran en el centro de salud. El objetivo de monitorizar el estado de salud y nutrición de la población materno-infantil es prevenir la aparición de enfermedades, evitar la desnutrición e incluso la muerte. Esta investigación asume los principios básicos de la tecnología adecuada buscando soluciones basadas en la comunidad, sostenibles, replicables y escalables a fin de garantizar futuros despliegues de acuerdo con las estrategias de Naciones Unidas.

Millennium Development Goals point out the necessity of actively promoting maternal-child health care status, especially in underserved areas. This article details the development actions carried out between 2008 and 2011 in some rural communities of Nicaragua with the aim to provide a low-cost tele-health communication service. The service is managed by the health care center of Cusmapa, which leads the program and maintains a communication link between its health staff and the health brigades of 26 distant communities. Local agents can use the system to report urgent maternal-child health care episodes to be assessed through WiMAX-WiFi voice and data communications attended by two physicians and six nurses located at the health care center. The health and nutritional status of the maternal-child population can be monitored to prevent diseases, subnutrition, and deaths. The action approach assumes the fundamentals of appropriate technology and looks for community- based, sustainable, replicable, and scalable solutions to ensure future deployments according to the strategies of the United Nations.

The presented work proposes a new approach for anomaly detection. This approach is based on changes in a population of evolving agents under stress. If conditions are appropriate, changes in the population (modeled by the bioindicators) are representative of the alterations to the environment. This approach, based on an ecological view, improves functionally traditional approaches to the detection of anomalies. To verify this assertion, experiments based on Network Intrussion Detection Systems are presented. The results are compared with the behaviour of other bioinspired approaches and machine learning techniques.

In the last years significant efforts have been devoted to the development of advanced data analysis tools to both predict the occurrence of disruptions and to investigate the operational spaces of devices, with the long term goal of advancing the understanding of the physics of these events and to prepare for ITER. On JET the latest generation of the disruption predictor called APODIS has been deployed in the real time network during the last campaigns with the new metallic wall. Even if it was trained only with discharges with the carbon wall, it has reached very good performance, with both missed alarms and false alarms in the order of a few percent (and strategies to improve the performance have already been identified). Since for the optimisation of the mitigation measures, predicting also the type of disruption is considered to be also very important, a new clustering method, based on the geodesic distance on a probabilistic manifold, has been developed. This technique allows automatic classification of an incoming disruption with a success rate of better than 85%. Various other manifold learning tools, particularly Principal Component Analysis and Self Organised Maps, are also producing very interesting results in the comparative analysis of JET and ASDEX Upgrade (AUG) operational spaces, on the route to developing predictors capable of extrapolating from one device to another.

Primary-care pediatricians could play a key role in early detection of development disorders as quick as they might have enough time and knowledge for suitable screenings at clinical routine. This research paper focuses on the development and validation of a knowledge-based web tool whose aim is to support a smart detection of developmental disorders in early childhood. Thus, the use of the system can trigger the necessary preventive and therapeutic actions from birth until the age of six. The platform was designed on the basis of an analysis of significant 21 cases of children with language disorders that supported the creation of a specific knowledge base, its ontology and a set of description logic relations. The resulting system is being validated in a scalable approach with a team of seven experts from the fields of neonathology, pediatrics, neurology and language therapy.

Monitoring of neuro-evolutive development from birth until the age of six is a decisive factor in a child's quality of life. Early detection of development disorders in early childhood can facilitate necessary diagnosis and/or treatment. Primary-care pediatricians play a key role in early detection of development alterations as they can undertake the preventive and therapeutic actions necessary in the interest of a child's optimal development. The focus of this research paper is the construction of a Knowledge Base for smart screening aimed to assist pediatricians in processes of early referral in language disorders. The proposed model provides health professionals with a decision-making tool that supports referral processes. In this way, essential diagnostic and/or therapeutic actions are triggered for a comprehensive individual development. The resulting system was developed on the basis of an analysis and verification of 21 cases of children with language disorders.

El presente Proyecto Fin de Carrera consiste en un estudio de los accesos a red que utilizan los servicios a los que están adscritos los usuarios de servicios de teleasistencia, planteando al final del mismo un modelo de previsión de caídas que permita que ese acceso a red no sea un problema para la prestación del servicio. Para poder llegar a los objetivos anteriormente descritos, iniciaremos este documento presentando qué se entiende actualmente como servicios de telemedicina y teleasistencia. Prestaremos atención a los actores que intervienen, usos y beneficios que tienen tanto para los pacientes como para las administraciones públicas. Una vez sepamos en qué consisten, centraremos la atención en las redes de acceso que se utilizan para prestar los servicios de telemedicina, con sus ventajas y desventajas. Puesto que no todos los servicios tienen los mismos requisitos generales de fiabilidad o velocidad de transmisión, veremos cómo se puede garantizar las necesidades de cada tipo de servicio por parte del proveedor de red. El siguiente paso para llegar a establecer el modelo de previsión de caídas será conocer las necesidades técnicas y de los actores para prestar un servicio de teleasistencia en el hogar de un paciente. Esto incluirá estudiar qué equipos se necesitan, cómo gestionarlos y cómo marcar el tráfico para que el operador de red sepa cómo tratarlo según el servicio de teleasistencia que se está utilizando, llevando a generar un modelo de supervisión de enlaces de teleasistencia. Llegados a este punto estaremos ya preparados para establecer un modelo de previsión de caídas de la conexión, describiendo la lógica que se necesite para ello, y poniéndolo en práctica con dos ejemplo concretos: un servicio de telemonitorización domiciliaria y otro servicio de telemonitorización ambulatoria. Para finalizar, realizaremos una recapitulación sobre lo estudiado en este documento y realizaremos una serie de recomendaciones. ABSTRACT. This Thesis is a study of the access network to be used with services assigned to patients that are users of telecare services. In the last chapter we will describe a fall forecasting model that allows the access network to not be an issue for the service. For achieving the objectives described above, this paper will begin with the presentation of what is now understood as telemedicine and telecare services. We pay attention to the actors involved, uses and benefits that they have both for patients and for public administrations. Once we know what telecare means and what requisites they have, we will focus on access networks which are used to provide telemedicine services, with their advantages and disadvantages. Since not all services have the same general requirements of reliability and transmission speed, we will try to see how you can ensure the needs of each type of service from the network provider's point of view. The next step is to establish that the forecasting model of falls will meet the technical needs and actors to provide telecare service in the home of a patient. This will include a study of what equipment is needed, how to manage and how to mark traffic for the network operator knowing how to treat it according to the telecare service being used, and this will lead us to the creation of a model of telecare link monitoring. At this point we are already prepared to establish a forecasting model of connection drops, describing the logic that is needed for this, and putting it into practice with two concrete examples: telemonitoring service and an ambulatory telemonitoring service. Finally, we will have a recap on what has been studied in this paper and will make a series of recommendations.

Background: Healthy diet and regular physical activity are powerful tools in reducing diabetes and cardiometabolic risk. Various international scientific and health organizations have advocated the use of new technologies to solve these problems. The PREDIRCAM project explores the contribution that a technological system could offer for the continuous monitoring of lifestyle habits and individualized treatment of obesity as well as cardiometabolic risk prevention. Methods: PREDIRCAM is a technological platform for patients and professionals designed to improve the effectiveness of lifestyle behavior modifications through the intensive use of the latest information and communication technologies. The platform consists of a web-based application providing communication interface with monitoring devices of physiological variables, application for monitoring dietary intake, ad hoc electronic medical records, different communication channels, and an intelligent notification system. A 2-week feasibility study was conducted in 15 volunteers to assess the viability of the platform. Results: The website received 244 visits (average time/session: 17 min 45 s). A total of 435 dietary intakes were recorded (average time for each intake registration, 4 min 42 s ± 2 min 30 s), 59 exercises were recorded in 20 heart rate monitor downloads, 43 topics were discussed through a forum, and 11 of the 15 volunteers expressed a favorable opinion toward the platform. Food intake recording was reported as the most laborious task. Ten of the volunteers considered long-term use of the platform to be feasible. Conclusions: The PREDIRCAM platform is technically ready for clinical evaluation. Training is required to use the platform and, in particular, for registration of dietary food intake.

Neuro-evolutive development from birth until the age of six years is a decisive factor in a child?s quality of life. Early detection of development disorders in early childhood can facilitate necessary diagnosis and/or treatment. Primary-care pediatricians play a key role in its detection as they can undertake the preventive and therapeutic actions requested to promote a child?s optimal development. However, the lack of time and little specific knowledge at primary-care avoid to applying continuous early-detection anomalies procedures. This research paper focuses on the deployment and evaluation of a smart system that enhances the screening of language disorders in primary care. Pediatricians get support to proceed with early referral of language disorders. The proposed model provides them with a decision-support tool for referral actions to trigger essential diagnostic and/or therapeutic actions for a comprehensive individual development. The research was conducted by starting from a sample of 60 cases of children with language disorders. Validation was carried out through two complementary steps: first, by including a team of seven experts from the fields of neonatology, pediatrics, neurology and language therapy, and, second, through the evaluation of 21 more previously diagnosed cases. The results obtained show that therapist positively accepted the system proposal in 18 cases (86%) and suggested system redesign for single referral to a speech therapist in three remaining cases.

Las Tecnologías de la Información y las Comunicaciones han propiciado avances en el contexto de la salud tanto en la gestión efectiva de información socio‐sanitaria de forma electrónica, como en la provisión de servicios de e‐salud y telemedicina. Los antecedentes de investigación publicados en esta área corroboran este hecho presentando las mejoras experimentadas en la atención de la población y en la provisión de servicios sanitarios. La atención temprana, cuyos principios científicos se fundamentan en los campos de la pediatría, neurología, psicología, psiquiatría, pedagogía, fisiatría y lingüística, entre otros, tiene como finalidad ofrecer a los niños con déficit o con riesgo de padecerlos un conjunto de acciones optimizadoras y compensadoras, que faciliten su adecuada maduración en todos los ámbitos y que les permita alcanzar el máximo nivel de desarrollo personal y de integración social. La detección de posibles alteraciones en el desarrollo infantil es un aspecto clave de la atención temprana en la medida en que puede posibilitar la puesta en marcha de diversos mecanismos de actuación disponibles en las entidades implicadas, valiosos para la calidad de vida de la persona. Cuanto antes se realice la detección, existen mayores garantías de prevenir patologías añadidas, lograr mejoras funcionales y posibilitar un ajuste más adaptativo entre el niño y su entorno. El objetivo de la investigación presentada en esta tesis doctoral es analizar, diseñar, verificar y validar un sistema de información abierto, basado en conocimiento, que facilite efectivamente a los profesionales que trabajan con la población infantil entre 0 y 6 años la detección precoz de posibles trastornos del lenguaje. Desde el punto de vista metodológico, la Ingeniería del Conocimiento ofrece un marco conceptual sólido que permite desarrollar y validar Sistemas de Ayuda a la Toma de Decisiones distribuidos y escalables, capaces de ayudar al pediatra de Atención Primaria y al educador infantil en la detección precoz de posibles trastornos del lenguaje en niños. La evaluación del sistema se ha realizado de forma incremental mediante el diseño y validación de pruebas de campo experimentales consistentes en la evaluación de niños en dos escenarios distintos: la escuela infantil y el centro de atención temprana. Los experimentos realizados en poblaciones distintas con alrededor de 344 niños durante 2 años, han permitido contrastar la buena adecuación del sistema propuesto a las necesidades de detección de los profesionales que trabajan con niños entre 0 y 6 años. La tesis resultante ha permitido caracterizar el uso del sistema en entornos reales, conocer la aceptación entre los usuarios y su impacto en la provisión de un servicio de atención temprana como el descrito para el correcto seguimiento del desarrollo del lenguaje en los niños, además de proponer un nuevo modelo de atención y evaluación cooperativa que permita incrementar el conocimiento experimental existente al respecto. ABSTRACT The Information and Communication Technology have led to advances in the context of health both in the effective management of socio‐health information electronically, and in the provision of e‐health and telemedicine. The history of research published in this area confirm this fact by presenting the improvements in the care of the population and the provision of health services. Early attention, whose scientific principles are based on the fields of pediatrics, neurology, psychology, psychiatry, pedagogy, physical medicine and linguistics, among others, aims to provide children with deficits or risk of suffering a set of enhancer actions, which facilitate adequate maturation in all areas and allow them to achieve the highest level of personal development and social integration. The detection of possible changes in child development is a key aspect of early intervention to the extent that it can enable the implementation of different mechanisms of action available to the entities involved, valuable to the quality of life of the person. The earlier the detection is made, there are more guarantees added to prevent diseases, achieving functional improvements and enable a more adaptive fit between the child and his environment. The aim of the research presented is to analyze, design, verify and validate an open information system, based on knowledge, which effectively provide professionals working with the child population between 0 and 6 years, in processes of early detection of language disorders. From the methodological point of view, Knowledge Engineering provides a solid conceptual framework to develop and validate a distributed and scalable decision support systems aim to assist pediatricians and language therapists at early identification and referral of language disorder in childhood. The system evaluation was performed incrementally with the design and validation of consistent experimental field tests in the assessment of children in two different scenarios: the nursery and early intervention center. Experiments in different populations with about 344 children over 2 years, allowed to testing the adequacy of the proposed good detection needs of professionals working with children between 0 and 6 years old system. The resulting thesis has allowed to formalizing the system at real environments and to identifying the acceptance by users as well as its impact on the provision of an early intervention service, such as the one described for the proper monitoring of language development in children. In addition, it proposes a new model of care and cooperative evaluation that lets to increase the existing experimental knowledge about it.
