Perfil tensional na fase aguda pós AVC e o seu significado prognóstico

The role of arterial blood pressure control in the secondary prophylaxis of stroke is well documented. However, its contribution to the reduction of the mortality rate associated with these events has not been clearly established yet, particularly, in the context of acute stroke. Despite the vast research literature on pathophysiological mechanisms, clinical trials have not yet been conclusive, in regards to the role of controlling arterial blood pressure in the acute phase of the stroke for the prognosis of these patients. The aim of this study is to summarize the latest findings of the role of arterial blood pressure monitoring, with emphasis on the 24-hour ABPM, in the acute context of stroke and to hypothesize about the way in which the blood pressure profile of this phase may influence this disease's prognosis (up to 3 months).

O papel do controlo da pressão arterial na profilaxia secundária do acidente vascular cerebral está bem estabelecido, no entanto não é claro o seu contributo para a redução da mortalidade associada a estes eventos vasculares. Continua por esclarecer o papel do controlo da PA no contexto agudo do AVC. Apesar das várias considerações sobre os mecanismos fisiopatológicos, os estudos clínicos ainda não foram conclusivos sobre a influência do controlo da PA na fase aguda do AVC no prognóstico destes doentes. O presente trabalho pretende não só sumarizar o conhecimento atual sobre o papel da monitorização da pressão arterial, com ênfase na MAPA de 24 horas, no contexto agudo de AVC, mas também equacionar de que forma o perfil tensional durante esta fase poderá influenciar o prognóstico (até 3 meses) desta doença cérebro-vascular.
