Diagnóstico de vertigem no serviço de urgência

The diagnosis of vertigo may be challenging for the doctor in the Emergency Department. This is a common symptom in this context and can be caused by a wide variety of pathologies. These range from benign diseases to those that can threaten the patient’s life. This differentiation should be done in an accurate way and, for this reason, the main features of each pathology should be recognized. In an initial stage the distinction between central and peripheral causes must be done in order to, in a second phase, being able to determine the specific cause of vertigo.The clinical judgment, based on the interview and in the examination, is the cornerstone for the diagnosis of the different etiologies in the Emergency Department. The skill to ommunicate with the patient, understanding if he has truly vertigo and also the ability to obtain information about the duration and characteristics of the vertigo, make possible the correct diagnosis and treatment.

O diagnóstico de vertigem pode ser desafiante para os médicos no Serviço de Urgência. Trata-se de um sintoma comum neste contexto, podendo ter uma grande variedade de etiologias. Estas vão desde as mais benignas, até às que podem por em risco a vida do doente. É essencial que esta diferenciação seja feita de forma precisa e para isso as principais particularidades de cada patologia devem ser reconhecidas. Numa primeira fase urge fazer a distinção entre causas centrais ou periféricas para, num segundo tempo, se determinar a causa específica de vertigem. O raciocínio clinico, baseado na anamnese e no exame objetivo, é basilar para o diagnóstico das diferentes etiologias em contexto de urgência. A capacidade de comunicar com o doente, compreendo se este apresenta verdadeiramente um episódio de vertigem, bem como conseguir obter informação quanto à duração e características da mesma possibilitam o diagnóstico e o correto tratamento.
