Tratamento do vírus da hepatite C : uma evolução para a cura?

HCV infection is nowadays the most common cause of chronic liver disease, with world prevalence around 160 million. During the last years the treatment has been revolutionized. With the introduction in 2011 of the first direct anti-viral agents and more recently, in 2013, with the introduction of Sofosbuvir, the cure of disease is an increasingly likely scenario, being the virological responses by far superior of those obtained with the association of Peg-IFN and Ribavirin (Standard of Care). So, we are going towards therapeutic regimens with more favorable charateristics, not only with regard to virological response, but also with a higher safety profile and more tolerable. The role of Peg-IFN and Ribavirin in the future is uncertain; however it could be helpful for a small group of patients.

A infecção por VHC constitui actualmente a causa mais comum de doença hepática crónica, apresentando uma prevalência mundial de cerca de 160 milhões. Nos últimos anos o tratamento da infecção crónica por VHC tem registado progressos significativos. Com a introdução em 2011 dos primeiros fármacos com acção anti-viral directa e em 2013 com a introdução do Sofosbuvir, a cura da doença antevê-se como um cenário cada vez mais provável, sendo as respostas virológicas obtidas em muito superiores às que se conseguiam com a associação de Peg-IFN e Ribavirina (SOC-Standard of Care). Caminhamos assim no sentido de esquemas terapêuticos com características cada vez mais favoráveis, não só ao nível da resposta virológica, mas também com maior perfil de segurança e maior tolerância. O papel do Peg-IFN e da Ribavirina a longo prazo não é claro, podendo o seu uso ficar restrito a um pequeno número de doentes.
