This study aims to clarify the Surgeon Dentist about the possible etiological factors responsible for Oro-antral Communication, to identify its clinical and radiographic signs, to explain the Buccal Fat Pad’s anatomy and functioning and to describe the surgical technique adopted front of these cases. For this, the authors present a case of a patient who had a fistula in the region where there was a dental extraction of the upper posterior element. The treatment of Oro-antral Fistula using the buccal fat pad provides to be a safe and effective surgical method, because this element presents a rich blood supply and easy access. Many authors have found that this method have a broad application, large index of success, lower risk of infection, provides a comfortable post-operative for the patient. However, it needs to be done properly so that you have minimum incidence of failures, and this requires some caution on the part of professional.

A Fístula Buco-Sinusal é uma ocorrência patológica comum, caracterizada pela comunicação do seio maxilar com a cavidade bucal durante extrações dentárias de elementos superiores posteriores cujas raízes possuem íntima relação com o seio maxilar. Este estudo tem por objetivo esclarecer ao Cirurgião Dentista quanto aos possíveis fatores etiológicos responsáveis por tal complicação, identificar seus sinais clínicos e radiográficos, explicar a anatomia e o funcionamento do Corpo Adiposo Bucal bem como descrever a técnica cirúrgica adotada frente a esses casos. Para isso, os autores apresentam o caso clínico de uma paciente com fístula buco-sinusal pós exodontia do primeiro molar superior direito permanente. O tratamento da fístula buco-sinusal utilizando o tecido adiposo bucal é um método cirúrgico seguro e eficaz, pois esse elemento apresenta um rico suprimento sanguíneo e um acesso fácil. Essa técnica cirúrgica apresenta um alto índice de sucesso, baixo risco de infecção, proporciona um pós-operatório confortável para o paciente e não modifica a profundidade do sulco vestibular, evitando a necessidade de um segundo procedimento cirúrgico para a reabilitação protética do paciente. Contudo, ela precisa ser realizada apropriadamente para que tenha mínima incidência de falhas e isso requer certo cuidado por parte do profissional.
