Avaliação da força muscular do assoalho pélvico em mulheres nulíparas continentes: influência das diferentes posições corporais

Avaliar a força da musculatura do assoalho pélvico (AP) nas diferentes posições corporais em mulheres nulíparas continentes. Foram estudadas 50 mulheres nulíparas voluntárias continentes com média de idade de 23 anos (variando de 20 a 30 anos). Após assinatura de Termo de Consentimento Livre e Esclarecido, as voluntárias foram avaliadas em diferentes parâmetros como questionário clínico, avaliação funcional do AP (AFA) na porção anterior e posterior, avaliação objetiva do AP com perineômetro em quatro diferentes posições corporais e satisfação sexual. Resultados: Observamos que 58% das voluntárias tinham atividades físicas regulares (em média 3 vezes por semana), o índice de massa corpórea (IMC) foi de 21,76Kg/m2 (normal). Na AFA, nas posições anterior e posterior, houve concordância significativa em 76% dos casos (p<0,05). Na avaliação objetiva com perineômetro nas diferentes posições, na posição em pé, a força muscular do AP foi significativamente maior em relação às demais posições (p<0,05). Em relação à média do tempo de contração observou-se que foi significantemente maior nas pacientes em pé em relação as demais posições (p<0,05). Não houve uma associação significativa entre orgasmo e força muscular do AP. Conclusão: Houve concordância significativa em 76% dos casos entre a AFA anterior e posterior. A avaliação da força muscular do AP foi significativamente maior nas posições sentadas e em pé. Observamos que não houve correlação entre orgasmo e força muscular do assoalho pélvico.

The aim of this study was to assess pelvic floor muscle (PFM) strength in nulliparous using objective evaluation in different test positions. 50 voluntary nulliparous women, mean age 23 years, were studied. Demographic data, physical activity was obtained using clinical questionnaire. Body Mass Index (BMI) was classified according to Garrow. Criteria of inclusion were: interest in participating as healthy volunteers; no presence of urinary complaints and age between 20 and 30 years. Objective evaluations of PFM strength were made using a portable perineometer (DM 01 Dynamed) connected to a balloon catheter, size 11X2.6 cm, inserted into the vagina. The balloon should be 1 cm from the outside of the vaginal conduit. In this way the middle of the balloon was placed 3,5 cm inside the vaginal introitus. Measurements of maximum and mean squeeze pressure, and holding period in seconds, were assessed in four different positions: in lying position with straight limbs (P1) and with bent limbs (P2); sitting (P3) and standing (P4). Only contractions with simultaneously visible inward movement of the perineum were accepted as correct. RESULTS: Patients reported regular physical activity, about 3 times per week, in 58% of cases. In average, BMI was 21.76 kg/m2 and considered normal. By investigating the function of the PFM, it was observed that 68% of nulliparous volunteers were conscious of the contraction of these muscles. The PFM strength measured by the perineometer was 16 cm H2O, 15 cm H2O, 19 cm H2O and 28 cm H2O in lying position with straight limbs and with bent limbs, sitting, and standing respectively. Muscular strength was significantly higher when sitting or standing when compared with the other positions. However, it was even higher in standing position in relation to sitting Abstract (p< 0.05).
