PURPOSE: Heavy episodic (i.e., "binge") drinking (i.e., ≥five drinks/occasion) is highly prevalent among young adults; those who binge do so four times per month on average, consuming nine drinks on average on each occasion. Although it is well established that chronic heavy drinking (≥two alcoholic beverages per day) increases the risk of hypertension, the relationship between binge drinking and blood pressure is not well described. Our aim was to describe the relationship between frequency of binge drinking, both current (at age 24 years) and past (at age 20 years), and systolic blood pressure (SBP) at age 24 years. METHODS: Participants (n = 756) from the longitudinal Nicotine Dependence in Teens study reported alcohol consumption at ages 20 and 24 years and had SBP measured at age 24 years. We examined the association between binge drinking and SBP using multiple linear regression, controlling for sex, race/ethnicity, education, monthly drinking in high school, cigarette smoking, and body mass index. RESULTS: Compared to nonbinge drinkers, SBP at age 24 years was 2.61 [.41, 4.82] mm Hg higher among current monthly bingers and 4.03 [1.35, 6.70] mm Hg higher among current weekly bingers. SBP at age 24 years was 2.90 [.54, 5.25] mm Hg higher among monthly bingers at age 20 years and 3.64 [.93, 6.35] mm Hg higher among weekly bingers at age 20 years, compared to nonbinge drinkers. CONCLUSIONS: Frequent binge drinking at ages 20 and 24 years is associated with higher SBP at age 24 years and may be implicated in the development of hypertension.

Smoking is one of the leading causes of preventable death. In recent years, numerous countries have initiated the prohibition of smoking in restaurants, workplaces and public spaces. The Vietnamese government intends to follow the precautions against public smoking as well. Over and above the number of some hazardous chemical components found in tobacco, 210Po isotope content could enhance the probability of the development of lung cancer. In this study 14 Vietnamese tobacco products (commercial cigarettes and pipe tobacco) 210Po activity concentration were determined using PIPS semiconductor alpha spectrometry. The results showed that the 210Po activity concentration of the investigated samples varied between 7.40 ± 1.09 - 128.64 ± 11.22 mBq g-1. The average 210Po content of commercial cigarettes was 15.5 mBq g-1, whilst the average of pipe tobacco was 20.4 mBq g-1. To estimate the risk of inhalation of 210Po isotopes originating as a result of smoking, dose estimations were carried out. © Versita Sp. z o.o.

Objective: To identify modifiable factors associated with sessile serrated polyps (SSPs), and compare the association of these factors to conventional adenomas (ADs) and hyperplastic polyps (HPs). Design: We utilized data from the Tennessee Colorectal Polyp Study, a colonoscopy-based case-control study. Included were 214 SSP cases, 1779 AD cases, 560 HP cases and 3851 polyp-free controls. Results: Cigarette smoking was associated with increased risk for all polyps and was stronger for SSPs than for ADs (OR 1.74. 95% CI: 1.16-2.62, for current vs. never, ptrend=0.008). Current regular use of nonsteroidal anti-inflammatories (NSAID) was associated with a 40% reduction in SSPs risk in comparison to never-users (OR 0.68, 95% CI 0.48-0.96, ptrend=0.03), similar to the association with AD. Red meat intake was strongly associated with SSPs risk (OR 2.59, 95% CI 1.41-4.74 for highest vs. lowest intake, ptrend<0.001) and the association with SSP was stronger than with AD (ptrend=0.003). Obesity, folate intake, fiber intake, and fat intake were not associated with SSP risk after adjustment for other factors. Exercise, alcohol use, and calcium intake were not associated with risk for SSPs. Conclusion: SSPs share some modifiable risk factors for ADs, some of which are more strongly associated with SSPs than ADs. Thus, preventive efforts to reduce risk for ADs may also be applicable to SSPs. Additionally, SSPs have some distinctive risk factors. Future studies should evaluate the preventive strategies for these factors. The findings from this study also contribute to an understanding of the etiology and biology of SSPs.

Contemporary public health approaches increasingly draw attention to the unequal social distribution of cigarette smoking. In contrast, critical accounts emphasize the importance of smokers’ situated agency, the relevance of embodiment and how public health measures against smoking potentially play upon and exacerbate social divisions and inequality. Nevertheless, if the social context of cigarettes is worthy of such attention, and sociology lays a distinct claim to understanding the social, we need to articulate a distinct, positive and systematic claim for smoking as an object of sociological enquiry. This article attempts to address this by situating smoking across three main dimensions of sociological thinking: history and social change; individual agency and experience; and social structures and power. It locates the emergence and development of cigarettes in everyday life within the project of modernity of the nineteenth and twentieth centuries. It goes on to assess the habituated, temporal and experiential aspects of individual smoking practices in everyday lifeworlds. Finally, it argues that smoking, while distributed in important ways by social class, also works relationally to render and inscribe it.

Berichtet wird über die Evaluation eines schulischen Gesundheitsförderungsprogramms, wobei es spezifisch um Wechselwirkungen zwischen Programmeffekten und schulklimatischen Bedingungskonstellationen (Klassenklima/Lehrerrückhalt) auf Wissen über, Erwartungen an und Konsum von Zigaretten geht. An der Untersuchung waren 388 Schüler und Schülerinnen achter und neunter Klassen beteiligt. Im Rahmen eines quasi-experimentellen Untersuchungsdesigns wurde in der Hälfte der Schulklassen ein 28stündiges Unterrichtsprogramm zur Gesundheitsförderung durchgeführt, während die andere Hälfte als Kontrollgruppe ohne Treatment diente. In der Interventionsgruppe zeigten sich Wissenszuwächse sowie positive Veränderungen in den Ergebniserwartungen, zum Teil in Abhängigkeit von den Klassenklimavariablen. Positive Veränderungen im Konsumverhalten als Folge der Intervention ergaben sich bei jüngeren Raucher/innen, und es zeigten sich Zusammenhänge zwischen Veränderungen in den Erwartungen an den Zigarettenkonsum und solchen im Konsumverhalten. OBJETIVO: El presente estudio tiene como objetivo determinar las conductas de riesgo de los adolescentes escolarizados que frecuentan los bares y discotecas de la “Calle Larga” de la ciudad de Cuenca (Calle del Centro Histórico, donde se sitúa la mayor cantidad de centros de diversión nocturna). MATERIALES Y MÉTODOS: Se trata de un estudio descriptivo, de tipo cuantitativo, transversal. Incluye un universo de 307 adolescentes, del Cantón Cuenca en el año 2014. (1) Los datos fueron recolectados mediante un formulario validado, que fue aplicado previo asentimiento y/o consentimiento informado de los adolescentes entre 10 y 19 años que acuden a “La Calle Larga” los días viernes y sábados durante el mes de Mayo del año 2014. RESULTADOS: En el estudio encontramos que el 67,75% de adolescentes ingiere alcohol, el 28,66% fuma cigarrillo y el 8.79% fuma marihuana, siendo ésta la sustancia ilícita de elección. CONCLUSIONES: La prevalencia de consumo para alcohol, cigarrillo y otras drogas en los adolescentes que asisten a sitios de diversión nocturna en la “Calle Larga”, Cuenca mayo 2014, fue mayor en la adolescencia tardía y constituyen conductas de riesgo la compañía de los amigos.  Introdução: Durante a gravidez, devido ao aumento da exigência metabólica placentária há um aumento na produção das espécies reativas de oxigénio (ROS) que podem causar, por exemplo, oxidação de ácidos graxos poli-insaturados na placenta, além disso, nesta fase ocorre um aumento na expressão da aromatase e do receptor relacionado ao estrógeno gama (ERRgama) na placenta humana. Objetivo: O objetivo do estudo foi avaliar os parâmetros de estresse oxidativo e imunomarcação da aromatase e do ERRgama na placenta humana. Métodologia: A capacidade antioxidante total (ACAP), atividade da glutamato cisteína ligase (GCL), concentração glutationa (GSH), peroxidação lipídica e imunomarcação da aromatase e do ERRgama foram analisados em tecido placentário de 58 parturientes. Estas análises foram relacionadas com os dados socio-demográficos das participantes. Resultados: Os recém-nascidos de mães fumantes nasceram com menor peso (p=0,001). A concentração de GSH diminuiu a produção da peroxidação lipídica (p<0,05), por outro lado, a atividade de GCL teve efeito oposto (p<0,001). Encontramos uma diminuição na capacidade antioxidante total e aumento da peroxidação lipídica (p<0,05) na placenta. A placenta de mães fumantes tinham menos marcação da aromatase (p=0,037) já, as mães mais velhas tiveram menos marcação do ERRgama (p=0,009) na placenta. A GSH teve efeito positivo na imunomarcação de ERRgama (p=0,001). Conclusões: A expressão da aromatase e do ERRgamma na placenta são alterados tanto por fatores exógenos, tais como o fumo do cigarro, como por fatores endógenos, tais como a concentração de GSH e a idade da mãe. Os marcadores de estresse oxidativo na placenta são mais elevados em mães mais velhas e em placenta com menor capacidade antioxidante total.

Background: Students’ alcohol and khat use have been associated with various health related problems. However, its magnitude and associated factors among Ethiopian students are not yet well documented. Objective: The study aimed to assess the prevalence of alcohol use, khat chewing and its associated factors among Hawassa University students. Methods: A cross-sectional study was conducted from June to July 2011. Multistage stratified sampling technique was employed to select 590 students. Self administered questionnaires were used to collect data. Data was entered and analysed by SPSS version 20.0. Logistic regression analyses were used to identify the association of different variables. Results: The current prevalence of student’s alcohol and khat use were 29.5% (95% CI: 25.8-33.3) and 16.3% (95% CI: 13.7- 20.0) respectively. Being male (AOR 1.8; 95% CI 1.1-3.0) and living alone (AOR 20.1; 95% CI 2.5-166.7) had a higher odds of alcohol use. Similarly, family substance use history (AOR 4.8; 95% CI 2.5-9.3) and peer influence (AOR 4.6; 95% CI 2.3-9.0) had also higher odds of khat use. Conclusion and recommendation: The proportion of student’s khat chewing and alcohol use was significant. Hence, higher education in collaboration with other stakeholders should work on convincing students about the ill effects of these substances.

Aim There is a high burden of oesophageal cancer in Malawi with dismal outcomes. It is not known whether environmental factors are associated with oesophageal cancer. Without knowing this critical information, prevention interventions are not possible. The purpose of this analysis was to explore environmental factors associated with oesophageal cancer in the Malawian context. Methods A hospital-based case-control study of the association between environmental risk factors and oesophageal cancer was conducted at Kamuzu Central Hospital in Lilongwe, Malawi and Queen Elizabeth Central Hospital in Blantyre, Malawi. Ninety-six persons with squamous cell carcinoma and 180 controls were enrolled and analyzed. These two groups were compared for a range of environmental risk factors, using logistic regression models. Unadjusted and adjusted odds ratios and 95% confidence intervals (CI) were calculated. Results Firewood cooking, cigarette smoking, and use of white maize flour all had strong associations with squamous cell carcinoma of the oesophagus, with adjusted odds ratios of 12.6 (95% CI: 4.2-37.7), 5.4 (95% CI: 2.0-15.2) and 6.6 (95% CI: 2.3-19.3), respectively. Conclusions Several modifiable risk factors were found to be strongly associated with squamous cell carcinoma. Research is needed to confirm these associations and then determine how to intervene on these modifiable risk factors in the Malawian context.

To establish the prevalence of olfactory dysfunction in smoking and non-smoking students of our Faculty who attend the Department of Otolaryngology (ENT) of our Hospital. Materials and method: Students (smokers and non-smokers) that do and do not suffer from olfactory dysfunction. We applied a questionnaire and a pocket smell test for screening all of the students. Results: We evaluated 207 students, between 18 and 30 years old; 50.7% (n=105) were women and 49.3% (n=102) were men. The smokers among them smoked up to 6 packs per year. One hundred twenty three students were non-smokers and 84 students were smokers. Of the 84 students who were smokers, 67 (79.7%) answered the Pocket Smell Test correctly (3/3) and 17 (20.2%) students had one or more errors. We had 123 non-smoker students and 103 (83.7%) students answered the Pocket Smell Test correctly and 20 (16.2%) answered with one or more errors. The prevalence of olfactory dysfunction in young smokers with a 95% conidence interval would be 32.8%. Conclusions: This study informed us about olfactory dysfunctions in our student population and their smoking habits. We corroborate that the Pocket Smell Test is reliable with the questionnaire; nevertheless it is a screening test. We have a population of young people who smoke one cigarette per day and who didn’t have a signiicant alteration in their ability of smell at the time of the study. This is consistent with medical literature. More studies should be conducted in order to expand this information.

Background: Passive smokers are involuntarily exposed to cigarette or tobacco smoke and as known, inhalation of environmental tobacco smoke is a serious threat. There is little information about the effect of passive smoking on salivary markers and periodontal indices. Objectives: This study investigated the effect of passive smoking on lactoferrin and AST in 12 - 15 years old children and adolescents. Patients and Methods: This case-control analytic correlation type study with no-convenience random sampling method was performed on 160 children aged 12 - 15 who had smokers in their families. The eligible children were divided into two equal groups; 80 cot+ children as case group and 80 cot– children as control group, matched according to age, sex and plaque index. Plaque index was obtained from all subjects. 2 cc unstimulated salivary samples were collected by spitting method. The collected specimens were tested by lactoferrin and AST kits in biochemistry were measured on the day of sampling laboratory. Gingival index Loe and Silness (GI) and Probing Pocket Depth (PPD). Results: Mean and Standard Deviation of PPD and GI was 2.01 ± 0.077 and 1.53 ± 0.055 in experimental group and 1.93 ± 0.073 and 1.49 ± 0.046 in control group respectively (P < 0.001). The Mean and Standard Deviation parameters of lactoferrin and AST, in the experimental group was 38.66 ± 25.15 and 13.45 ± 6.33 and in the control group 10.18 ± 6.82 and 6.53 ± 2.65 group, respectively (P < 0.001). Conclusions: Passive smoking can be effective on inflammatory process of periodontal and salivary biomarkers related to inflammation. Lactoferrin was 11 - 104 in case group and 0.5 - 38 in control group. Aspartat aminotransferase in case group was 2.64 - 30.43 and in control group it was 2.16 - 12.02.

Two as-synthesized meso- and macro-porous siliceous materials (MPSMs), i.e., Al-MCM-41 and SBA-15, were mixed with tobacco to study their effect on tobacco smoke chemistry. A reference cigarette, 3R4F, and a commercial cigarette, Fortuna, containing different percentages of MPSM were smoked in a smoking machine, and the mainstream smoke was analyzed. SBA-15 showed the highest reductions of nicotine; close to 90% when it was added at 8 mass %. The superb behavior of these materials may be related to their high particulate matter filtering efficiency in combination with their catalytic activity. The selectivity of these materials with respect to nicotine was also analyzed. Al-MCM-41 presents higher selectivity for condensed compounds than for gases, whereas SBA-15 presents similar ratios for both fractions. The highest selectivity was obtained for the liquid fraction when smoking 3R4F cigarettes mixed with Al-MCM-41.

Diammonium hydrogen phosphate (DAP) is commonly used as a flavor ingredient of commercial cigarettes. In addition, among its other uses, it is employed to expand the tobacco volume, to manufacture reconstituted tobacco sheet, and to denicotinize tobacco. However, the use of DAP as a cigarette ingredient is a controversial issue. Some authors have stated that ammonium compounds added to tobacco increase smoke ammonia and “smoke pH”, resulting in more free nicotine available in the smoke. On the other hand, other researchers have reported that the larger ammonium content of a cigarette blend due to the presence of DAP was not reflected in increased smoke ammonia. In this work, the thermal behavior of DAP, tobacco and DAP-tobacco mixtures has been studied by TGA/FTIR. The chemical processes involved in the different pyrolysis steps of DAP have been suggested. Marked changes in the pyrolytic behavior of both, tobacco and DAP have been detected when analyzing the behavior of the mixtures. A displacement of the decomposition steps mainly related to the glycerol and lignin from tobacco toward lower temperatures has been observed, whereas that associated with cellulose is displaced toward higher temperature. Additionally, no peak corresponding to the phosphorous oxides decomposition has been detected in the curves relating to the DAP-tobacco mixtures. All these features are indicative of the strong interactions between DAP and tobacco. The FTIR spectra show no significant qualitative differences between the qualitative overall composition of the gases evolved from the pyrolysis of tobacco in the absence and in the presence of DAP. Nevertheless, depending on the temperature considered, the addition of DAP contributes to a decrease in the generation of hydrocarbons and an increase in the formation of CO, CO2 and oxygenated compounds in terms of amount generated per mass of pyrolysed tobacco.
