OBJECTIVE: In the literature, there is no consistent classification of healthcare facilities. In order to benchmark, assess, and compare the environmental performance of these buildings, it is important to clearly identify the typology within the scope of a particular research. This article identifies the different typologies within the healthcare sector, particularly in Australia, with the aim of the development of energy performance benchmarks for day surgery/procedure centers. BACKGROUND: Healthcare buildings encompass a wide range of facilities. They all share the same purpose of healing and offering a health service for patients. However, they vary significantly in terms of patient type and service provided. These buildings consume a considerable amount of energy, and as a result of the different designs and sizes, their pattern of energy consumption varies. METHODS: The research used a systematic review of the literature to determine how the term "healthcare facility" has been employed in different contexts. In order to better understand the differences in healthcare facilities, definitions and the origin of hospitals and healthcare facilities are introduced and a framework for the classification of healthcare facilities and hospitals is proposed. RESULTS: Healthcare facilities are classified into the following six categories: patient type, care provided, management and ownership, level of care, facility size, and location. Based on these classifications, a categorization for the studies of energy performance in healthcare is introduced. CONCLUSIONS: This study provides a basis for assessment and comparison for a particular healthcare building typology that will assist researchers working in the field of design and energy assessment of healthcare facilities.

BACKGROUND: It has been suggested in the literature that raised heart rate in the early period after trauma is associated with the development of post-traumatic psychopathology, but little account has been taken of the potential confounding effect of injury severity.

MATERIALS AND METHODS: A cohort of 154 patients, studied as part of a wider investigation of trauma outcomes, was included. Initial heart rate in the accident & emergency department, and injury severity score and new injury severity scores were recorded. Patients completed the General Health Questionnaire (GHQ-28) as a measure of psychopathology at presentation and again at two- and six-month follow-up.

RESULTS: There was no relationship between psychopathology at presentation and initial heart rate or injury severity. Raised heart rate was associated with post-traumatic psychopathology at two months but not at six months. When the potential confounding effect of injury severity was controlled for, there was no independent correlation between heart rate and post-traumatic psychopathology. Injury severity score and new injury severity scores were strongly associated with GHQ-28 caseness.

CONCLUSION: Post-traumatic tachycardia is not associated with development of psychopathology, but injury severity is. Previous studies that have suggested a link between tachycardia and development of psychopathology are flawed because they have not considered the confounding effect of severity of injury.

Aim: To explore and compare call bell response times in two wards in a geriatric evaluation and management facility before and after the introduction of a suite of interventions aimed at decreasing patient falls. Method: Data on call bell response times were collected over two periods. The first were before implementation of falls prevention initiatives. Data were retrieved from the call bell system that detailed the time taken to respond to every call bell activation. A second period of data collection was conducted six months after implementation of the initiatives. Results: Prioritising call bell response and raising staff awareness improved response to patient calls. There was a slight decrease in falls although call bell activations did not decrease. Conclusion: Strong leadership is necessary from nurse managers to stress the importance of prompt call bell response. Visual surveillance of high-risk fallers is important as they are generally unable to ring for assistance when required.

 With the rising demands on cloud services, the electricity consumption has been increasing drastically as the main operational expenditure (OPEX) to data center providers. The geographical heterogeneity of electricity prices motivates us to study the task placement problem over geo-distributed data centers. We exploit the dynamic frequency scaling technique and formulate an optimization problem that minimizes OPEX while guaranteeing the quality-of-service, i.e., the expected response time of tasks. Furthermore, an optimal solution is discovered for this formulated problem. The experimental results show that our proposal achieves much higher cost-efficiency than the traditional resizing scheme, i.e., by activating/deactivating certain servers in data centers.

Invasive mammalian predators are major drivers of species extinctions globally. To protect native prey, lethal control is often used with the aim of reducing or exterminating invasive predator populations. The efficacy of this practice, however, is often not considered despite multiple practical and ecological factors that can limit success. Here, we summarize contemporary knowledge regarding the use and challenges of both lethal control and alternative approaches for reducing invasive predator impacts. As the prevailing management approach, we outline four key issues that can compromise the effectiveness of lethal control: release of herbivore and mesopredator populations, disruption of predator social systems, compensatory predator immigration, and ethical concerns. We then discuss the relative merits and limitations of four alternative approaches that may enhance conservation practitioner's ability to effectively manage invasive predators: top-predator conservation or reintroduction, maintaining habitat complexity, exclusion fencing, and behavioral and evolutionary ecology. Considerable uncertainty remains regarding the effectiveness of management approaches in different environmental contexts. We propose that the deficiencies and uncertainties outlined here can be addressed through a combination of adaptive management, expert elicitation, and cost-benefit analyses. Improved management of invasive predators requires greater consideration and assessment of the full range of management approaches available.

BackgroundA dimension of the culture in group homes is staff regard for residents. In underperforming group homes, staff regard residents as being not ‘like us’ (Bigby, Knox, Beadle-Brown, Clement & Mansell, 2012). We hypothesized the opposite pole of this dimension, in higher performing group homes, would be that staff regard residents positively.MethodThree in-depth qualitative case studies were conducted in higher performing group homes using participant observation, interviews and document review.ResultsConsistent pattern of staff practices and talk, as well as artefacts, demonstrated staff had a positive regard for residents, who were seen as being ‘like us’. Explicit and continuing attention was given to sustaining positive regard for residents in everyday staff practices and to turning abstract values into concrete realities.ConclusionsThis positive cultural norm was established, operationalized and embedded through structures, such as a formal policy about language, and processes such as peer monitoring and practice leadership.

QoS plays a key role in evaluating a service or a service composition plan across clouds and data centers. Currently, the energy cost of a service's execution is not covered by the QoS framework, and a service's price is often fixed during its execution. However, energy consumption has a great contribution in determining the price of a cloud service. As a result, it is not reasonable if the price of a cloud service is calculated with a fixed energy consumption value, if part of a service's energy consumption could be saved during its execution. Taking advantage of the dynamic energy-Aware optimal technique, a QoS enhanced method for service computing is proposed, in this paper, through virtual machine (VM) scheduling. Technically, two typical QoS metrics, i.e., the price and the execution time are taken into consideration in our method. Moreover, our method consists of two dynamic optimal phases. The first optimal phase aims at dynamically benefiting a user with discount price by transparently migrating his or her task execution from a VM located at a server with high energy consumption to a low one. The second optimal phase aims at shortening task's execution time, through transparently migrating a task execution from a VM to another one located at a server with higher performance. Experimental evaluation upon large scale service computing across clouds demonstrates the validity of our method.

Live video forwarding for IP cameras has become a popular service in video data centers. In the forwarding service, requests of end users from different regions arrive in real-time to gain live video streams of IP cameras from inter-connected video data centers. A fundamental scheduling problem is how to assign resources with the global optimal resource cost and forwarding delay to forward live video streams. We introduce the resource provisioning cost as the combination of media server cost, connection bandwidth cost, and forwarding delay cost. In this paper, a multi-objective resource provisioning (MORP) approach is proposed to deal with the online inter-datacenter resource provisioning problem. The approach aims at minimizing the resource provisioning cost during live video forwarding. It adaptively allocates media servers in appropriate video data centers and connects the chosen media servers together to provide system scalability and connectivity. Different from previous works, MORP takes both resource capacity and diversity (e.g. location and price) into consideration during live video forwarding. Finally, the experimental results show that MORP approach not only cuts the resource provisioning cost of 3% to 10% comparing to the bench mark approach, but also shortens the resource provisioning delay.

Both James Britton and James Moffett were keynote speakers at the Sydney International Federation for the Teaching of English conference in 1980 - a fact reflective of the wide recognition and acceptance of their work and influence throughout Australia by that time. In Victoria, Moffett's writings became known initially through teacher education, in particular at the University of Melbourne and the State College of Victoria, Rusden, then through the visits and writing of figures from the London Institute and others, and through the State and national English teaching association, the Australian Association for the Teaching of English. In the 1970s, Moffett's influence in Victoria came rather through the mix of his vision and writing, both theoretical and practical, in conjunction with others in Australia and elsewhere. This paper takes two separate but related sites or moments in English education in the 1970s in the Australian city of Melbourne, Victoria, as instances of the permeating influence of Moffett's work - in conjunction with leading figures from the London School associated with the 'New' English' - on education discourses and practice in that State's English curriculum history. It concludes with a consideration of the ways in which Moffett's work might still act as a 'rallying call' today.

The purpose of this thesis was to redesign a commercial center in Miami, Florida in a manner that incorporates the needs of pedestrians as well as the automobile. In my research, I studied projects that had been successful at integrating cars in retail design. I applied the strategies learned from this research to the design of a center that creates a positive interaction of pedestrian and car traffic, addressing the needs of the surrounding community. I designed a master plan that includes a mix of residential, retail, commercial and parking space. The parking is designed so that the retail center is not dominated by surface parking. Rather, the automobile is introduced into the different layers of the proposed buildings. The design focused on connecting pedestrian plazas and parking areas beneath them through the introduction of light and greenery. The findings show how a shopping center might transform the area around it by including spaces for residential, civic, cultural and social functions, as well as for the automotive infrastructure that make those functions possible.

The rapid growth of virtualized data centers and cloud hosting services is making the management of physical resources such as CPU, memory, and I/O bandwidth in data center servers increasingly important. Server management now involves dealing with multiple dissimilar applications with varying Service-Level-Agreements (SLAs) and multiple resource dimensions. The multiplicity and diversity of resources and applications are rendering administrative tasks more complex and challenging. This thesis aimed to develop a framework and techniques that would help substantially reduce data center management complexity. We specifically addressed two crucial data center operations. First, we precisely estimated capacity requirements of client virtual machines (VMs) while renting server space in cloud environment. Second, we proposed a systematic process to efficiently allocate physical resources to hosted VMs in a data center. To realize these dual objectives, accurately capturing the effects of resource allocations on application performance is vital. The benefits of accurate application performance modeling are multifold. Cloud users can size their VMs appropriately and pay only for the resources that they need; service providers can also offer a new charging model based on the VMs performance instead of their configured sizes. As a result, clients will pay exactly for the performance they are actually experiencing; on the other hand, administrators will be able to maximize their total revenue by utilizing application performance models and SLAs. This thesis made the following contributions. First, we identified resource control parameters crucial for distributing physical resources and characterizing contention for virtualized applications in a shared hosting environment. Second, we explored several modeling techniques and confirmed the suitability of two machine learning tools, Artificial Neural Network and Support Vector Machine, to accurately model the performance of virtualized applications. Moreover, we suggested and evaluated modeling optimizations necessary to improve prediction accuracy when using these modeling tools. Third, we presented an approach to optimal VM sizing by employing the performance models we created. Finally, we proposed a revenue-driven resource allocation algorithm which maximizes the SLA-generated revenue for a data center.

The study of acoustic communication in animals often requires not only the recognition of species specific acoustic signals but also the identification of individual subjects, all in a complex acoustic background. Moreover, when very long recordings are to be analyzed, automatic recognition and identification processes are invaluable tools to extract the relevant biological information. A pattern recognition methodology based on hidden Markov models is presented inspired by successful results obtained in the most widely known and complex acoustical communication signal: human speech. This methodology was applied here for the first time to the detection and recognition of fish acoustic signals, specifically in a stream of round-the-clock recordings of Lusitanian toadfish (Halobatrachus didactylus) in their natural estuarine habitat. The results show that this methodology is able not only to detect the mating sounds (boatwhistles) but also to identify individual male toadfish, reaching an identification rate of ca. 95%. Moreover this method also proved to be a powerful tool to assess signal durations in large data sets. However, the system failed in recognizing other sound types.

On-call work is becoming an increasingly common work pattern, yet the human impacts of this type of work are not well established. Given the likelihood of calls to occur outside regular work hours, it is important to consider the potential impact of working on-call on stress physiology and sleep. The aims of this review were to collate and evaluate evidence on the effects of working on-call from home on stress physiology and sleep. A systematic search of Ebsco Host, Embase, Web of Science, Scopus and ScienceDirect was conducted. Search terms included: on-call, on call, standby, sleep, cortisol, heart rate, adrenaline, noradrenaline, nor-adrenaline, epinephrine, norepinephrine, nor-epinephrine, salivary alpha amylase and alpha amylase. Eight studies met the inclusion criteria, with only one study investigating the effect of working on-call from home on stress physiology. All eight studies investigated the effect of working on-call from home on sleep. Working on-call from home appears to adversely affect sleep quantity, and in most cases, sleep quality. However, studies did not differentiate between night's on-call from home with and without calls. Data examining the effect of working on-call from home on stress physiology were not sufficient to draw meaningful conclusions.
