INTRODUCTION AND AIMS: Hypertension is a common side effect of recombinant human erythropoietin (rHuEPO) therapy; however, the exact pathways remain to be elucidated. The discovery of non-hematopoietic actions of rHuEPO increased the number of patients that could putatively benefit from this therapy; however, to achieve those effects higher doses are usually needed, which increase the risk and incidence of adverse events. Our aim was to study the effect of a broad range of rHuEPO doses on hematological and biochemical parameters, blood pressure and renal function and damage in the rat, focusing on endothelial nitric oxide synthase (eNOS) and hypoxia-inducible factors (HIFs). METHODS: Male Wistar rats were divided in 5 groups receiving different doses of rHuEPO (100, 200, 400 and 600 IU/kg body weight (BW)/week) and saline solution (control), during 3 weeks. Blood and 24h urine were collected to perform hematological and biochemical analysis. Blood pressure (BP) was measured by the tail-cuff method. The kidney tissue was collected to mRNA and protein expression assays and to characterize renal lesions. RESULTS: A dose-dependent increase in red blood cells count, hematocrit and hemoglobin levels was found with rHuEPO therapy, in rHuEPO200, rHuEPO400 and rHuEPO600 groups. Increased reticulocyte count was found in the rHuEPO400 and rHuEPO600 groups. BP raised in all groups receiving rHuEPO. The rHuEPO200 and rHuEPO600 groups presented increased kidney protein levels of HIF2α and a reduction in kidney protein levels of eNOS, along with the highest grade of vascular and tubular renal lesions. CONCLUSIONS: Our study showed that rHuEPO-induced hypertension might involve indirect (hematological) and direct (renal) effects which varies according to the dose used. Thus, rHuEPO therapy should be performed rationally and under adequate surveillance, as hypertension develops even with lower doses. Especial caution with higher doses should be taken, as rHuEPO-induced hypertension leads to early renal damage without alterations in traditional markers of renal function, thus masking the serious adverse effects and risks.

In order to assess the rationale and possible indications for the use of recombinant erythropoietin in paroxysmal nocturnal haemoglobinuria (PNH), we have measured endogenous erythropoietin (Epo) levels in 18 patients with PNH and in 44 patients with iron deficiency anaemia (IDA), In both groups of patients we found a significant inverse correlation between Epo and haemoglobin (Hb). However, the mean Epo level was significantly higher in the PNH group (385 mU/ml) than in the IDA group (136 mU/ml), The range of Epo levels at any given Hb was greater in the PNH group than in the IDA group. There was a significant positive correlation between Epo and absolute reticulocyte count, Since Epo administration is unlikely to benefit patients with high levels of endogenous Epo, we conclude that in the majority of patients with PNH there is no indication for treatment with Epo.

Haplotypes linked to the βS gene represent patterns of DNA polymorphisms along chromosome 11 of individuals bearing the βS gene. Analysis of haplotypes, in addition to serving as an important source for anthropological studies about the ethnic origin of a population, contributes to a better understanding of the variations in clinical severity of sickle cell anemia. The aim of the present study was to determine βS gene haplotypes in a group of patients with sickle cell anemia treated at the Dalton Barbosa Cunha Hematology Center (Hemonorte) in Natal, Brazil and the Oncology and Hematology Center in Mossoró, Brazil. Blood samples were obtained from 53 non-related patients (27 males and 26 females), aged between 3 months and 61 years (mean age: 16.9 ± 12.1 years). Laboratory analyses consisted of the following: erythrogram, reticulocyte count, hemoglobin electrophoresis at alkaline pH, measurement of hemoglobin A2 and Fetal hemoglobin, solubility test and molecular analysis to determine βS gene haplotypes. DNA samples were extracted by illustra blood genomicPrep Mini Spin kit and βS gene haplotypes were determined by PCR-RFLP, using Xmn I, Hind III, Hinc II and Hinf I restriction enzymes for analysis of six polymorphic restriction sites in the beta cluster. Of 106 βS chromosomes studied, 75.5% were Central African Republic (CAR) haplotype, 11.3% Benin (BEN) and 6.6% Cameroon (CAM). The atypical haplotypes had a frequency of 6.6%. More than half the patients (58.5%) were identified as CAR/CAR genotype carriers, 16.9% heterozygous CAR/BEN, 13.2% CAR/CAM and 1.9% BEN/BEN. Patients with atypical haplotype in one or two chromosomes accounted for 9.5% (CAR/Atp, BEN/Atp and Atp/Atp). The genotype groups showed no statistically significant difference (p < 0.05) in their laboratory parameters. This is the first study related to βS haplotypes conducted in state of Rio Grande do Norte and the higher frequency of Cameroon halotype found, compared to other Brazilian states, suggests the existence of a peculiarity of African origin

A comparative study was made regarding the clinical and hematological alterations caused by isolates of Babesia bigemina from southeastern, northeastern and northern Brazil in experimentally infected Nelore calves. Eighteen calves between 7 and 9 months of age, without antibodies against Babesia sp and raised free from ticks, were used. Three animals were previously inoculated with 2.0×109 parasitic erythrocytes (PE) for each stabilate. The other 15 calves were subdivided into three groups, with five animals each, that were subinoculated with 1.0×1010 PE of the respective isolates. The clinical and hematological alterations were evaluated by the determination of parasitaemia, haemogram, plasmatic fibrinogen, reticulocyte count, descriptive analysis of the bone marrow and erythrocytic osmotic fragility, for 30 days, totalizing seven moments of observation. The follow-up of the immunological response by the indirect fluorescent antibody test was carried out daily until the 10th day after inoculation (DAI) and after that, on the 15th, 20th, 25th and 30th DAI. A mild clinical manifestation of the disease was observed. The laboratory findings revealed low levels of parasitaemia; decrease of the erythrogram values; absence of reticulocytes, initial decrease in the total count of leukocytes, neutrophils and lymphocytes with a posterior elevation of the number of these cells; hypercellularity of the erythrocytic series and decrease of the myeloid: erythroid relation which was more accentuated between the 8th and 12th DAI, and an increase of the erythrocytic osmotic fragility in the groups inoculated with the Southeast and Northeast isolates. None of the three isolates of B. bigemina gave rise to the clinical characteristic form of the disease, although they induced an humoral immune response.

The hydroxyurea, a cytotoxic drug, is the mainly available therapeutical strategy for the treatment of sickle cell disease. This study aimed to evaluate the mutagenic and genotoxic potential of the hydroxyurea through the Salmonella/Microsome assay and micronucleus test in peripheral blood of mice. The doses were evaluated at 29.25-468 μmol/plate in Salmonella/Microsome assay in presence and absence of metabolic activation the drug. In the micronucleus test the doses were evaluated at 12.5; 25; 50; 75 and 100 mg/kg. The results show that hydroxyurea present mutagenic activity in TA98 and TA100 in doses above 117 μmol/plate and 234 μmol/plate respectively. The drug induced a significant increase in the frequency of micronuclei in reticulocytes of mice at concentrations of 50, 75 and 100 mg/kg, compared to negative control (water). These results demonstrated the mutagenic and genotoxic potential of hydroxyurea. © 2011 Santos et al.

As crianças, assim como os adultos, são susceptíveis em adquirir malária, apresentando manifestações clínicas de intensidade variável na dependência do seu grau de imunidade e da espécie de plasmódio causadora da infecção. Com o objetivo de traçar o perfil epidemiológico, clínico e laboratorial da malária por P. vivax foram avaliadas 100 crianças entre 0 - 14 anos de idade, de ambos os sexos, com diagnóstico positivo para P. vivax (gota espessa), no Ambulatório do Programa de Malária do Instituto Evandro Chagas, Belém - Pará, no período de janeiro de 1995 a novembro de 1996. Em relação à faixa etária, os adolescentes foram os mais acometidos pela doença (37,0%). Os casos autóctones representaram 34,0% da casuística, evidenciando a presença do paludismo nos núcleos urbanos da Região Amazônica. A febre, em 88,0% das crianças se constituiu na principal manifestação clínica inicial da doença. No 1º dia de atendimento (D0), a febre, o calafrio e a cefaléia (tríade malárica) ocorreram respectivamente em 97,0%, 91,0% e 85,0%, enquanto que a hepatomegalia em 29,0% e a esplenomegalia em 46,0% das crianças. Entre palidez e anemia, avaliada pela taxa de hemoglobina, houve uma correlação significativa (p < 0,05), verificando-se que entre as crianças pálidas, 89,2% eram anêmicas. A hemólise parece ter sido a causa básica da anemia, tendo também contribuído para sua instalação o retardo no diagnóstico (média de 12,5 dias) e o parasitismo intestinal por ancilostomídeos. Neste estudo, a desnutrição parece não ter exercido qualquer influencia sobre a anemia. Com a terapêutica, observou-se um declínio tanto no percentual de crianças com tríade malárica como no percentual de crianças com parasitemia assexuada, sendo este declínio de maior intensidade na tríade malárica. Outros sinais e sintomas (palidez, astenia, artralgia, cefaléia, colúria) ocorreram por um período de tempo maior do que o da tríade malárica, em geral, persistindo até 14 dias. As complicações presentes durante ou imediatamente após o tratamento, em 5,0% das crianças, foram pneumonia, broncopneumonia, impetigo generalizado, gastroenterite e exantema de etiologia não definida. Em relação à metodologia empregada para avaliação da hepatoesplenomegalia, a ultrassonografia abdominal mostrou-se mais sensível do que a palpação abdominal. Com o tratamento instituído, as taxas de : hemoglobina, os reticulócitos e o volume corpuscular médio (VCM) tiveram um aumento significativo de D0 (primeiro dia de terapêutica) para D7 (oitavo dia de terapêutica). Entretanto, em relação à concentração da hemoglobina corpuscular média (CHCM) houve uma diminuição significativa nos valores encontrados em D7 quando comparados aos valores de D0, possilvemente às custas de uma menor oferta de ferro para a medula óssea.

The anisocytosis is a hematologic finding that is routinely evaluated from the mean corpuscular volume (MCV) and erythrocyte morphology in assessment of blood smears. The present study tested the hypothesis that the coefficient of variation of the red cell distribution width (RDW-CV - Red Cell Distribution Width) is a more sensitive parameter in the evaluation of hematologic anisocytosis, considering different levels of anemia in dogs and reticulocytosis. Blood counts of 102 anemic dogs and 353 control dogs made by automated hematology counter. The anemic animals were grouped according to the degree of anemia (mild, moderate and severe) and also as medullary response by manual reticulocyte count (none, weak, moderate and strong). The RDW-CV was different to the animal groups with moderate and severe anemia, which were higher, compared to the control group and mild anemia. The highest average value of RDW-CV (14.45%) occurred in patients with severe anemia. In the control group had higher MCV value that anemic animals , this may be due to a mixed population of erythrocytes in that group of animals. However, there wasn’t difference in MCV between different degrees of anemia and reticulocytosis. Therefore, the RDW is a more sensitive indicator anisocytosis than VCM and its value is directly proportional to the degree of anemia and reticulocytosis; when combined, the sensitivity for detection of anisocytosis in anemic dogs is greater. The use of reference values established by the clinical laboratory itself is mighty important.

Pernicious anemia and Vitamin B12 deficiency have a wide range of symptoms and are a common finding in the elderly. A 73 year old female is admitted to the hospital because of dyspnea, fatigue and loss of appetite and weight. While previous medical history and physical examination are inconspicuous, laboratory findings show severe pancytopenia with macrocytosis, low reticulocyte count and marked signs of hemolysis. A very low serum level of vitamin B12 and chronic atrophic type A gastritis upon endoscopy with presence of parietal cell antibodies in the serum lead to the diagnosis of pernicious anemia. Complete restitution is achieved by parenteral vitamin B12 substitution. Nowadays, severe pernicious anemia is only rarely seen. The differential diagnosis of pancytopenia (with macrocytic anemia) combined with hemolysis and the essential hints to the diagnosis of pernicious anemia are discussed, and thereby practical aspects including therapy actualized.

AIM: Suppression of erythropoiesis due to low plasma erythropoietin levels is an important factor in the development of anaemia of prematurity. Premature infants may therefore be treated with recombinant human erythropoietin (rhEPO). This prospective, randomised and controlled study was designed to find out whether rhEPO treatment improves erythrocyte deformability in preterm infants. METHODS: Sixteen infants were treated with rhEPO (250 IU/kg three times weekly) a total of 15 times beginning on day of life 5 whereas fifteen infants served as controls. Haemoglobin concentration, haematocrit, reticulocyte count, ferritin level and erythrocyte deformability were measured on days 5, 14, 28, 42 and 63. Erythrocyte elongation was determined as an indicator of erythrocyte deformability using a shear stress diffractometer (Rheodyn SSD) at shear forces of 0.3 to 60 Pa. RESULTS: Haemoglobin concentration was significantly higher on days 28 and 42 and reticulocyte percentage on day 28 in the rhEPO group compared to the controls. Serum ferritin was lower in the rhEPO group on day 28. Erythrocyte deformability was significantly increased on days 28 and 42 in the infants receiving rhEPO. We found a strong relationship between erythrocyte elongation and reticulocyte count. CONCLUSION: RhEPO markedly increases the erythropoiesis in preterm infants in the critical first weeks of life and the anaemia of prematurity is obviously reduced. The erythrocyte deformability improved under rhEPO treatment. Erythrocyte deformability was significantly related to the reticulocyte count indicating that the improvement of erythrocyte deformability was due to the formation of well-deformable young erythrocytes.
