The proportion of patients over 75 years of age, receiving all different types of healthcare, is constantly increasing. The elderly undergo surgery and anaesthetic procedures more often than middle-aged patients. Poor pain management in the elderly is still an issue. Although the elderly consumes the greatest proportion of prescribed medicines in Western Europe, most clinical pharmacological studies have been performed in healthy volunteers or middle-aged patients. The aim of this study was to investigate pain measurement and management in cognitively impaired patients in long term hospital care and in cognitively normal elderly patients after cardiac surgery. This thesis incorporated 366 patients, including 86 home-dwelling or hospitalized elderly with chronic pain and 280 patients undergoing cardiac surgery with acute pain. The mean age of patients was 77 (SD ± 8) years and approximately 8400 pain measurements were performed with four pain scales: Verbal Rating Scale (VRS), the Visual Analogue Scale (VAS), the Red Wedge Scale (RWS), and the Facial Pain Scale (FPS). Cognitive function, depression, functional ability in daily life, postoperative sedation and postoperative confusion were assessed with MMSE, GDS, Barthel Index, RASS, and CAM-ICU, respectively. The effects and plasma concentrations of fentanyl and oxycodone were measured in elderly (≥ 75 years) and middle-aged patients (≤ 60 years) and the opioid-sparing effect of pregabalin was studied after cardiac surgery. The VRS pain scores after movement correlated with the Barthel Index. The VRS was most successful in the groups of demented patients (MMSE 17-23, 11-16 and ≤ 10) and in elderly patients on the first day after cardiac surgery. The elderly had a higher plasma concentration of fentanyl at the end of surgery than younger patients. The plasma concentrations of oxycodone were comparable between the groups. Pain intensity on the VRS was lower and the sedation scores were higher in the elderly. Total oxycodone consumption during five postoperative days was reduced by 48% and the CAM-ICU scores were higher on the first postoperative day in the pregabalin group. The incidence of postoperative pain during movement was lower in the pregabalin group three months after surgery. This investigation demonstrates that chronic pain did not seem to impair daily activities in home-dwelling Finnish elderly. The VRS appeared to be applicable for elderly patients with clear cognitive dysfunction (MMSE ≤17) and it was the most feasible pain scale for the early postoperative period after cardiac surgery. After cardiac surgery, plasma concentrations of fentanyl in elderly were elevated, although oxycodone concentrations were at similar level compared to middle-aged patients. The elderly had less pain and were more sedated after doses of oxycodone. Therefore, particular attention must be given to individual dosing of the opioids in elderly surgical patients, who often need a smaller amount for adequate analgesia than middle-aged patients. The administration of pregabalin reduced postoperative oxycodone consumption after cardiac surgery. Pregabalin-treated patients had less confusion, and additionally to less postoperative pain on the first postoperative day and during movement at three months post-surgery. Pregabalin might be a new alternative as analgesic for acute postoperative and chronic pain management in the elderly. Its clinical role and safety remains to be verified in large-scale randomized and controlled studies. In the future, many clinical trials in the older category of patients will be needed to facilitate improvements in health care methods.

Objective: The aim of this study was to investigate tendon pain in abnormal patellar tendons with and without neovascularization.

Study design: Comparative design.

Setting: Multidisciplinary tendon study group at a competitive volleyball venue.

Participants: One hundred eleven volleyball players volunteered to participate in the study.

Main Outcome Measures: Subjects' patellar tendons were imaged with ultrasound, with and without Doppler. Tendons that were imaging abnormal were categorized according the presence of tendon neovascularization. Subjects completed 3 pain scales that examined function (Victorian Institute of Sport Assessment score, 100-point maximum), pain with tendon load (decline squat, visual analogue scale, 100-mm maximum), and maximum pain for the previous week (visual analogue scale, 100-mm maximum). A 1-tailed Mann-Whitney U test compared pain scores in abnormal tendons without neovascularization to abnormal tendons with neovascularization.

Results: Functional scores were lower (Victorian Institute of Sport score, median, 78; P = 0.045) and pain scores under tendon load were greater (decline squat pain, median, 19; P = 0.048) in subjects with abnormal tendons with neovascularization than subjects with abnormal tendons without neovascularization (Victorian Institute of Sport Assessment score, median, 87; decline squat pain, median, 0).

Conclusions: This study indicates that the presence of neovascularization in abnormal patellar tendons is associated with greater tendon pain compared with abnormal tendons without neovascularization in active jumping athletes.

BACKGROUND: In Switzerland approximately 8% of infants are born prematurely. Some of them undergo mechanical ventilation including endotracheal suctioning (ETS). ETS is one of the most frequently performed interventions and is linked to stress and pain, but its treatment is controversial. In Switzerland there is a lack of standardisation in pain relief for ETS. AIMS: To test the hypothesis that an intermittent dose of morphine reduces pain during ETS and that subsequent multisensorial stimulation (MSS), as a non pharmacological comforting intervention, helps infants to recover from experienced pain. METHOD: A randomized placebo controlled trial in two tertiary neonatal intensive care units (NICU) with a sample of 30 mechanically ventilated preterm infants was conducted. Pain was measured by three pain assessment tools (Bernese Pain Scale for Neonates, Premature Infant Pain Profile and Visual Analogue Scale) RESULTS: Morphine did not lead to any pain relief from ETS as measured by three pain scales. Nor did the comforting intervention of MSS show any effect. Repeated-measure analysis of variance for the within and between groups comparison showed no statistical significance. CONCLUSIONS: The administration of morphine for pain relief in ventilated preterm neonates during ETS remains questionable and the use of MSS as a comforting intervention after painful stimulus cannot be recommended. The validity testing of the instruments for this patient population should undergo a systematic validation trajectory. Future research should focus on options among non pharmacological interventions for relieving pain during ETS.

L’évaluation de la douleur chez les oiseaux est difficile, puisque la plupart se comportent comme des proies et ont tendance à masquer tout signe extérieur de douleur. Les doses et les drogues utilisées pour traiter la douleur des oiseaux sont la plupart du temps basées sur une extrapolation d’autres espèces, ainsi que sur l’expérience clinique. Peu d’études de pharmacocinétique, d’efficacité et de toxicité sont disponibles dans la littérature. La plupart des études rapportées utilisent des stimuli nociceptifs éloignés des douleurs cliniques, comme les stimuli électriques ou thermiques, qui sont difficilement extrapolables à des situations rencontrées en pratique. L’objectif de notre projet était d’évaluer les effets analgésiques de deux doses de meloxicam chez le pigeon à l’aide d‘un modèle de fracture du fémur. La douleur post-opératoire a été évaluée pendant les quatre premiers jours suivant la chirurgie par trois méthodes : le suivi du poids porté sur la patte opérée comparativement à l’autre patte, quatre différentes échelles descriptives de douleur et la réalisation d’éthogrammes à l’aide d’enregistrements vidéo. L’administration de 0,5 mg/kg PO q12h de meloxicam n’a pas permis de réduire significativement les indicateurs de douleur mesurés comparativement à un groupe témoin recevant de la saline. Les pigeons ayant reçu 2 mg/kg PO q12h de meloxicam ont montré une réduction significative des indicateurs de douleur mesurés par les différentes méthodes. Nos résultats suggèrent que l’administration de 2 mg/kg PO q12h aux pigeons suite à une chirurgie orthopédique procure une analgésie supérieure aux doses actuellement recommandées dans la littérature.

La douleur est une expérience multidimensionnelle comportant des aspects sensoriels, émotionnels et cognitifs. Théoriquement, des méthodes de mesures comportementales, physiologiques, neurophysiologiques et sensorielles peuvent quantifier la douleur. Peu d’études ont étudié la validation des mesures utilisées en médecine vétérinaire. La recherche combine les travaux de Maîtrise et de Doctorat, traite en partie de la validité de méthodes. Dans cet objectif, nos travaux de recherche étudiaient la validité de méthodes comportementales, physiologiques et neurophysiologiques usuelles pour la mesure de la douleur en comparant les expressions de douleur (vache et chien) chez des animaux contrôle par comparaison à des animaux sous analgésie préventive ou sous traitement curatif suivant une douleur induite par chirurgie (modèles de douleur viscérale bovine ou orthopédique canine) ou causée par une maladie naturelle (arthrose canine). Une première étude comparait les mesures de la douleur entre les vaches du groupe placebo et celles sous analgésie postopératoire sur une durée de 21 jours suivant l’induction d’une douleur viscérale chronique. Les vaches du groupe placebo ont présenté une plus forte sensibilité à la douleur et une diminution de la noradrénaline et de la transthyrétine mesurées dans le liquide cérébro-spinal, une diminution de l’activité motrice (AM) (moindre que dans les groupes avec analgésie), de l’agitation enregistrée par vidéo-analyse et une augmentation du stress selon la mesure de l’activité électrodermique (AED). Les méthodes d’intérêt identifiées étaient les marqueurs spinaux, la mesure de la sensibilisation, de comportements par vidéo-analyse et de l’AM par bio-télémétrie. En utilisant des méthodes semblables à celles précédemment décrites, deux études expérimentales de douleur orthopédique ont été réalisées afin de comparer les réponses à la douleur entre des chiens traités avec une analgésie préventive (opioïdes et anti-inflammatoires, étude #2) ou un biphosphonate (tiludronate, étude #3) par comparaison à des chiens contrôles. Seules les échelles de douleur étaient différentes entre les études de recherche. Pour l’étude #2, les ii chiens sous analgésie ont présenté de plus faibles scores de douleur mesurés avec l’échelle de douleur nommée 4A-VET et ceci simultanément à une faible réponse de l’AED une heure après la chirurgie de trochléoplastie. La fréquence du comportement spontané de ‘la marche avec plein appui de la patte opérée’ mesurée à l’aide de la vidéo-analyse augmentait chez les chiens sous analgésie préventive 24 heures après la chirurgie. L’étude #3 démontrait surtout l’apparition de sensibilisation centrale (à la fois par l’évaluation sensorielle quantitative et les marqueurs spinaux) chez les chiens contrôle, 56 jours après l’induction d’arthrose chirurgicale. Ainsi, les chiens traités avec le tiludronate ont présenté une différence sur la substance P et la transthyrétine cérébro-spinale, une diminution de la sensibilisation périphérique, plus d’appui de la patte opérée lors de la marche selon la mesure du pic de force verticale (PFV), une augmentation de la fréquence de ‘la marche avec plein appui de la patte opérée’. La sensibilisation centrale était associée à la diminution de PFV, et une augmentation de l’AED et du comportement spontané de ‘la marche avec plein appui de la patte opérée’. Pour l’étude #4, la validité et la sensibilité des méthodes ont été évaluées dans une condition d’arthrose naturelle chez des chiens traités avec une diète enrichie en moule verte, un produit ayant des effets anti-inflammatoires et chondroprotecteurs attendus. Les chiens traités présentaient une diminution des scores de douleur via l’échelle nommée CSOM, une augmentation de PFV et une augmentation de l’AM. Dans l’ensemble, les résultats confirment que la vidéo-analyse évaluait la douleur de façon objective et pour des modèles différents de douleur et les marqueurs spinaux sont prometteurs. Le PFV était spécifique de la douleur orthopédique. La sensibilisation était présente lors de douleur pathologique. L’AED n’est pas valide pour la mesure de la douleur. La baisse d’AM suggèrerait un comportement de douleur. Les études étaient exploratoires pour les échelles de douleur en raison de leur niveau (débutant) de développement et du manque d’informations sur les qualités métrologiques de ces mesures.

El dolor es un problema importante para los pacientes hospitalizados en las UCI porque genera malestar y distrés. Además, la investigación ha demostrado que en algunos pacientes críticos el dolor agudo puede persistir después de alta y convertirse en crónico. La gestión eficaz del dolor en pacientes críticos requiere un enfoque interdisciplinario, que incorpore la visión y trabajo de expertos que representan una amplia variedad de especialidades clínicas. Así, la utilización de la intervención psicológica en el tratamiento del dolor es una parte integral de un enfoque global. Basado en una revisión de la evidencia científica, se identifican y señalan: (1) los tipos de dolor más comunes; (2) las características del dolor; (3) las patologías más frecuentes asociadas con la presencia de dolor; (4) los procedimientos que generan dolor en la UCI; (5) los métodos de evaluación del dolor; (6) la intervención del mismo y; (7) la contribución del psicólogo en la evaluación y manejo del dolor con el paciente, los familiares y los profesionales de la salud. La revisión realizada indica que los procesos psicológicos influyen tanto en la experiencia del dolor como en los resultados del tratamiento, por lo tanto la integración de los principios psicológicos en el tratamiento del dolor parecen tener potencial mejora de los resultados beneficiando la salud del paciente.

Introducción: La calidad de las compresiones torácicas tiene importancia durante la reanimación pediátrica y se ve afectada por diversos factores como la fatiga del reanimador, esta puede verse condicionada por las características de las compresiones establecidas según la presencia o ausencia de un dispositivo avanzado en la vía aérea determinando la interrupción continuidad de las mismas. En este estudio se realizó una simulación clínica, evaluando la presencia de fatiga del reanimador frente a pacientes con y sin dispositivo avanzado de la vía aérea. Metodología: Se incluyeron 12 participantes, quienes realizaron compresiones torácicas a un simulador clínico, tanto para el caso de la maniobra 1 correspondiente a ciclos interrumpidos con el fin de proporcionar ventilaciones, como para el caso de la maniobra 2 en la que la actividad fue continua. Se midieron calidad de compresiones, VO2 max y fatiga mediante escala de Borg RPE 6-20. Resultados: La calidad de las compresiones disminuyó en ambos grupos después del minuto 2 y más rápidamente cuando fueron ininterrumpidas. La fatiga se incrementó cuando las compresiones fueron continuas. Discusión: Se evidencia una relación directamente proporcional del aumento de la fatiga en relación al tiempo de reanimación e inversamente proporcional entre la calidad de las compresiones y la sensación de cansancio, en especial después del minuto 2. Un tiempo de 2 minutos podría ser el tiempo ideal para lograr compresiones de calidad y para realizar el reemplazo de la persona que realiza las compresiones.

Since all analgesics currently available for use in dogs have been associated with some adverse effects, the search for an effective analgesic that does not cause harm is important. This study investigated the postoperative analgesic effects of ozone administered either intrarectally or into acupoints in bitches undergoing ovariohysterectomy (OH). Twenty-four healthy adult bitches were randomly assigned to one of the three treatments 10min after sedation, as follows: 0.2mg/kg of intramuscular (IM) meloxicam (M); rectal insufflation of 10mL of 30μg/mL ozone (OI), or acupoint injection of 0.5mL ozone (30μg/mL; OA). Following sedation with acetylpromazine, anaesthesia was induced with propofol and fentanyl and maintained with isoflurane/O2. Pain was assessed using the modified Glasgow pain scale (MGPS) and the visual analogue scale (VAS) on the day before surgery, before anaesthesia, and at 1, 2, 4, 6, 8, 12 and 24h after surgery. Rescue analgesia was performed using 0.5mg/kg of morphine IM if MGPS was >3.33 points.No statistically significant differences in pain scales were found among the three analgesic protocols or the time points in each group ( P>. 0.05). Two dogs treated with OA required rescue analgesia. Meloxicam, rectal insufflation of ozone and ozone injected into acupoints provided satisfactory analgesia for 24. h in bitches undergoing elective OH. Ozone had no measurable adverse effects and is an alternative option to promote pain relief. © 2013 Elsevier Ltd.

Background: Tumescent anaesthesia (TA) is a widely used technique in oncologic surgeries necessitating large resection margins. This technique produces transoperative and postoperative analgesia, reduces surgical bleeding, and facilitates tissue divulsion. This prospective, randomised, blind study evaluated the use of TA in bitches submitted to mastectomy and compared the effect of TA with an intravenous fentanyl bolus. A 2.5-mcg/kg intravenous fentanyl bolus (n = 10) was compared with TA using 0.275% lidocaine (n = 10) in bitches submitted to unilateral mastectomy. Sedation was performed by intramuscular (IM) injection of 0.05 mg/kg of acepromazine combined with 2 mg/kg of meperidine. Anaesthesia was induced with 5 mg/kg of intravenous propofol and maintained with isoflurane/O2. Heart and respiratory rates; systolic, mean, and diastolic arterial blood pressures; central venous pressure; SpO2; ETCO2; inspired and expired isoflurane concentrations; and temperature were measured transoperatively. Visual analogue scales for sedation and pain and the Glasgow composite and Melbourne pain scales were used for postoperative assessment. The surgeon investigated the quality of the surgical approach, considering bleeding and resection ability, and the incidence of postoperative wound complications.Results: The heart rate was lower and the end-tidal isoflurane concentration was higher in dogs treated with fentanyl than in dogs treated with TA. A fentanyl bolus was administered to 8 of 10 dogs treated with fentanyl and to none treated with TA. Intraoperative bleeding and the mammary gland excision time were lower in dogs treated with TA. The maximal mean and individual plasma lidocaine concentrations were 1426 ± 502 ng/ml and 2443 ng/ml at 90 minutes after infiltration, respectively. The Glasgow Composite Pain Scale scores were higher in dogs treated with fentanyl than in dogs treated with TA until 2 hours after extubation.Conclusions: Compared with intravenous fentanyl, TA in bitches: may be easily performed in non-inflamed, ulcerated, adhered mammary tumours; has an isoflurane-sparing effect; improves transoperative and immediate postoperative analgesia; is apparently safe for use in clinical conditions as evidenced by the fact that it did not produce any adverse signs or lidocaine plasma concentrations compatible with toxicity; does not modify the recovery time; and facilitates the surgical procedure without interfering with wound healing. © 2013 Credie et al.; licensee BioMed Central Ltd.

The objective of this study was to evaluate the clinical usefulness, in terms of analgesic efficacy and safety, of ultrasound-guided pudendal nerve block performed with bupivacaine in cats undergoing perineal urethrostomy. Eighteen client-owned male cats scheduled for perineal urethrostomy were enrolled in the study and assigned to one of two treatment groups. The pudendal nerve block was performed under general anaesthesia as described elsewhere, with 0.3 ml/kg of either saline (group C) or 0.5% bupivacaine (group B) - the total injection volume being split equally on the two sites of injection (left and right). Intra-operatively, assessment of nociception was based on the rescue analgesics requirement, as well as on the evaluation of changes in physiological parameters in comparison with the baseline values. Post-operative pain assessment was performed using three different pain scales at recovery and then 1, 2 and 3 h after recovery. Cats in group B showed lower heart rates and required fewer analgesics during surgery than group C. Post-operatively, group B had lower pain scores and needed less rescue buprenorphine than group C. Iatrogenic block-related complications were not observed. In conclusion, the ultrasound-guided pudendal nerve block can be considered clinically useful in feline medicine as it provides reliable analgesia in cats undergoing perineal urethrostomy.
