This study examined the impact of hospital restructuring moves on a sample of Australian nurses' health. The role of organizational support, assessed via levels of consultation with staff, social support, and nurses coping were examined as further contributors or mediators of the relationship between the impact of restructuring and nurses' health. Data from 201 hospital nurses indicated that the factors in the model explained 41% of the variance in nurses' health. “Top-down” communication style by management contributed negatively to nurses' health and increased their perceptions of the impact of restructuring. Support from peers, supervisors, and family together with seeing the demands of impact of restructuring as a challenge, contributed positively to nurses' health and reduced the level of avoidance strategies used. The implications of these findings are discussed.

Background Increasing prevalence of overweight and obesity represents a global pandemic. As the largest occupational group in international healthcare systems nurses are at the forefront of health promotion to address this pandemic. However, nurses own health behaviours are known to influence the extent to which they engage in health promotion and the public's confidence in advice offered. Estimating the prevalence of overweight and obesity among nurses is therefore important. However, to date, prevalence estimates have been based on non-representative samples and internationally no studies have compared prevalence of overweight and obesity among nurses to other healthcare professionals using representative data. Objectives To estimate overweight and obesity prevalence among nurses in Scotland, and compare to other healthcare professionals and those working in non-heath related occupations. Design Cross-sectional study using a nationally representative sample of five aggregated annual rounds (2008-2012) of the Scottish Health Survey. Setting Scotland. Participants: 13,483 adults aged 17 to 65 indicating they had worked in the past 4 weeks, classified in four occupational groups: nurses (n = 411), other healthcare professionals (n = 320), unqualified care staff (n = 685), and individuals employed in non-health related occupations (n = 12,067). Main outcome measures: Prevalence of overweight and obesity defined as Body Mass Index ≥ 25.0. Methods Estimates of overweight and obesity prevalence in each occupational group were calculated with 95% confidence intervals (CI). A logistic regression model was then built to compare the odds of being overweight or obese with not being overweight or obese for nurses in comparison to the other occupational categories. Data were analysed using SAS 9.1.3. Results 69.1% (95% CI 64.6,73.6) of Scottish nurses were overweight or obese. Prevalence of overweight and obesity was higher in nurses than other healthcare professionals (51.3%, CI 45.8,56.7), unqualified care staff (68.5%, CI 65.0,72.0) and those in non-health related occupations (68.9%, CI 68.1,69.7). A logistic regression model adjusted for socio-demographic composition indicated that, compared to nurses, the odds of being overweight or obese was statistically significantly lower for other healthcare professionals (Odds Ratio [OR] 0.45, CI 0.33,0.61) and those in non-health related occupations (OR 0.78, CI 0.62,0.97). Conclusions Prevalence of overweight and obesity among Scottish nurses is worryingly high, and significantly higher than those in other healthcare professionals and non-health related occupations. High prevalence of overweight and obesity potentially harms nurses’ own health and hampers the effectiveness of nurseshealth promotion role. Interventions are therefore urgently required to address overweight and obesity among the Scottish nursing workforce.

Abstract Aims. To investigate the relationship between three types of organizational resources (job control, social support and organizational justice) and the impact of job demands on nurse’s well-being and attitudes towards their work. Background. The negative impact of work-related stress on nurse’s health and attitudes towards their work has been established. Increasingly, research is focusing on the role of organizational resources in reducing the impact of work related stress. Design. Cross-sectional survey. Method. Data collected in November 2008 from 226 Australian nurses and midwives were analysed using the full Job Strain Model with the addition of organizational justice variables. Multiple regression analyses explored the relationships among job control, job demands, three sources of social support and four types of organizational justice on well-being and work attitudes. Results. The overall regression models explained a significant amount of variance in well-being, job satisfaction and organizational commitment. Significant main effects were evident for support variables and organizational justice variables on well-being and job satisfaction. Interactions between job control and supervisor support and between job demands and supervisor support were evident for job satisfaction. Conclusions. Supervisor support and organizational justice have significant relationships with nurses’ well-being and job satisfaction. More broadly, the findings suggest that, in the triple-matching approach from a work-stressor to a resource to a work outcome, personal, supervisory and organizational resources may be substitutable. These findings provide nurse management with empirical endorsement for the development and delivery of the organization’s resources for nursing staff.

Hand hygiene is the primary measure in hospitals to reduce the spread of infections, with nurses experiencing the greatest frequency of patient contact. The ‘5 critical moments’ of hand hygiene initiative has been implemented in hospitals across Australia, accompanied by awareness-raising, staff training and auditing. The aim of this study was to understand the determinants of nurses’ hand hygiene decisions, using an extension of a common health decision-making model, the theory of planned behaviour (TPB), to inform future health education strategies to increase compliance. Nurses from 50 Australian hospitals (n = 2378) completed standard TPB measures (attitude, subjective norm, perceived behavioural control [PBC], intention) and the extended variables of group norm, risk perceptions (susceptibility, severity) and knowledge (subjective, objective) at Time 1, while a sub-sample (n = 797) reported their hand hygiene behaviour 2 weeks later. Regression analyses identified subjective norm, PBC, group norm, subjective knowledge and risk susceptibility as the significant predictors of nurses’ hand hygiene intentions, with intention and PBC predicting their compliance behaviour. Rather than targeting attitudes which are already very favourable among nurses, health education strategies should focus on normative influences and perceptions of control and risk in efforts to encourage hand hygiene adherence.

This qualitative study has as its focus an exploration of health service providers' perceptions and experiences of the processes and implications of delivering workplace cultural diversity education for staff. Data were obtained from conducting in-depth individual and focus group interviews with a purposeful sample of 137 healthcare professionals, recruited from over 17 different organizational sites. Participants included cultural diversity educators, ethnic liaison officers, health service managers, nurses, health interpreters, allied health professionals, and community-based ethnic welfare organization personnel working in or with select metropolitan health services in Victoria, Australia. Analysis of the data revealed that workplace cultural diversity education in healthcare is a significant site of resistance and struggle. 'Resistance' was expressed in several forms including: the problematization of resources and staff availability to attend cultural diversity education forums; indifferent failure to recognize cultural imperatives in healthcare; deliberate refusal to recognize cultural imperatives in healthcare; selective recognition of cultural imperatives in healthcare ('facts sheets' only); and the angry rejection of cultural imperatives in healthcare. 'Struggle', in turn, largely involved cultural diversity educators having to constantly 'cajole and convince' (and even manipulate) staff to attend cultural diversity education forums and using a 'velvet glove and iron fist' approach to teaching staff who remained resolute in their resistance when participating in educational forums. An important implication of this study is that the politics of workplace cultural diversity education - and the 'politics of resistance' to such programs - need to be better recognized and understood if the status quo is to be successfully challenged and changed. The need for critical debate and further comparative research on the subject are also highlighted.

Australia is a large country with 60% of land used for agricultural production. Its interior is sparsely populated, with higher morbidity and mortality recorded in rural areas, particularly farmers, farm families, and agricultural workers. Rural health professionals in addressing health education gaps of farming groups have reported using behavioralist approaches. These approaches in isolation have been criticized as disempowering for participants who are identified as passive learners or 'empty vessels.' A major challenge in rural health practice is to develop more inclusive and innovative models in building improved health outcomes. The Sustainable Farm Families Train the Trainer (SFFTTT) model is a 5-day program developed by Western District Health Service designed to enhance practice among health professionals working with farm families in Australia. This innovative model of addressing farmer health asks health professionals to understand the context of the farm family and encourages them to value the experience and existing knowledge of the farmer, the family and the farm business. The SFFTTT program has engaged with health agencies, community, government, and industry groups across Australia and over 120 rural nurses have been trained since 2005. These trainers have successfully delivered programs to 1000 farm families, with high participant completion, positive evaluation, and improved health indicators. Rural professionals report changes in how they approach health education, clinical practice, and promotion with farm families and agricultural industries. This paper highlights the success of SFFTTT as an effective tool in enhancing primary health practice in rural and remote settings. The program is benefiting not only drought ravaged farmers but assisting rural nurses, health agencies, and health boards to engage with farm families at a level not identified previously. Furthermore, nurses and health professionals are now embracing a more 'farmer-centered model of care.'

This research investigated expertise in hazardous substance risk assessment (HSRA). Competent pro-active risk assessment is needed to prevent occupational ill-health caused by hazardous substance exposure occurring in the future. In recent years there has been a strong demand for HSRA expertise and a shortage of expert practitioners. The discipline of Occupational Hygiene was identified as the key repository of knowledge and skills for HSRA and one objective of this research was to develop a method to elicit this expertise from experienced occupational hygienists. In the study of generic expertise, many methods of knowledge elicitation (KE) have been investigated, since this has been relevant to the development of 'expert systems' (thinking computers). Here, knowledge needed to be elicited from human experts, and this stage was often a bottleneck in system development, since experts could not explain the basis of their expertise. At an intermediate stage, information collected was used to structure a basic model of hazardous substance risk assessment activity (HSRA Model B) and this formed the basis of tape transcript analysis in the main study with derivation of a 'classification' and a 'performance matrix'. The study aimed to elicit the expertise of occupational hygienists and compare their performance with other health and safety professionals (occupational health physicians, occupational health nurses, health and safety practitioners and trainee health and safety inspectors), as evaluated using the matrix. As a group, the hygienists performed best in the exercise, and this group were particularly good at process elicitation and at recommending specific control measures, although the other groups also performed well in selected aspects of the matrix and the work provided useful findings and insights. From the research, two models of HSRA have been derived, an HSRA aid, together with a novel videotape KE technique and interesting research findings. The implications of this are discussed with respect to future training of HS professionals and wider application of the videotape KE method.

Estudo que possui como objeto violência laboral em hospital psiquiátrico como risco psicossocial à saúde dos trabalhadores de enfermagem, cujo interesse investigativo iniciou-se na graduação e posteriormente ao trabalhar como enfermeiro em um hospital psiquiátrico. Tais experiências marcaram de forma peculiar a minha trajetória profissional e o interesse acerca do objeto de estudo, por entender que a violência em âmbito psiquiátrico precisa ser desvelada e discutida para que medidas preventivas sejam adotadas no coletivo dos trabalhadores com vistas ao bem estar, a satisfação no trabalho e qualidade do serviço ofertado a população. Objetivou-se neste estudo, identificar os tipos de violência presentes no trabalho da enfermagem em hospital psiquiátrico; descrever as repercussões da violência laboral para a saúde dos trabalhadores de enfermagem em hospital psiquiátrico e analisar os mecanismos de enfrentamento adotados pelos trabalhadores da enfermagem diante da violência laboral em hospital psiquiátrico. Estudo qualitativo, descritivo, cujos dados foram obtidos em um hospital psiquiátrico situado no município do Rio de Janeiro, no período janeiro a fevereiro de 2013 com 16 trabalhadores (7 enfermeiros e 9 técnicos de enfermagem), a partir dos critérios de inclusão adotados. Trabalhou-se com a técnica de entrevista semiestruturada, mediante um roteiro contendo questões sobre o objeto de estudo. O projeto atendeu as exigências presentes na Resolução 196/96, do Ministério da Saúde (MS), tendo sido aprovado pela Comissão de Ética em Pesquisa (CEP) com o número 070.3.2012. Aplicada a técnica de conteúdo ao material emergiram os seguintes resultados: na vivencia dos trabalhadores do hospital psiquiátrico há três tipos de violência. A primeira refere-se à violência sofrida, principalmente durante as emergências psiquiátricas, momento em que o trabalhador sofre com as agressões verbais e, em alguns casos, físicas cometidas pelo paciente. Outro tipo de violência no trabalho foi a perpetrada pelo familiar em momentos de tensão e a terceira envolveu a violência simbólica por parte dos médicos, principalmente os residentes. A violência do trabalho foi identificada em decorrência da precarização das condições de trabalho em termos de recursos humanos e materiais. A violência laboral revelou-se como um risco psicossocial a saúde do trabalhador por acarretar sofrimento psíquico e físico evidenciado através de queixas de desgaste, estresse e medo, levando a insatisfação e desmotivação no trabalho. Para se manterem no trabalho, os trabalhadores elaboram estratégias de enfrentamento centradas na resolução dos problemas decorrentes da violência e na regulação da emoção. Diante dos resultados, concluiu-se que a violência em hospital psiquiátrico é um risco psicossocial que afeta a saúde dos trabalhadores de enfermagem, cabendo a organização juntamente com os trabalhadores propor medidas que deem visibilidade a violência sofrida, através do diagnostico, da prevenção e enfrentamento coletivo, o que pode ser realizado mediante denuncia dos próprios trabalhadores junto a instituição, sindicatos e órgãos de classe. Salienta-se a importância de suporte psicoterápico dos trabalhadores de enfermagem vitimas de violência com vistas à identificação dos fatores de risco e fortalecimento dos fatores protetores. Recomenda-se a continuidade de estudos na área, considerando a incipiência dos mesmos.

Atualmente, no Brasil e no mundo, há um aumento do número de mulheres que postergam a gravidez para após os 35 anos em função da estabilidade profissional, espera de um relacionamento, infertilidade, incerteza sobre o desejo de ser mãe. Independente do motivo, na proporção em que acontece, o aumento do número de mulheres que vivenciam a gravidez tardia configura-se um fenômeno irreversível, que só tende a aumentar. Esta pesquisa teve como objeto a vivência da gravidez tardia e seus objetivos foram: descrever a vivência da mulher que engravida após os 35 anos e analisar a vivência sob a perspectiva de gênero e vulnerabilidade. Estudo descritivo, com abordagem qualitativa, tendo como técnica de coleta de dados a entrevista aberta, realizada com 16 mulheres no ciclo gravídico puerperal, com idade entre 36 e 48 anos, internadas em duas maternidades públicas, cenários da pesquisa, situadas no município do Rio de Janeiro. Foram observados os princípios da Resolução n 466 de 12 de dezembro de 2012 do Conselho Nacional de Saúde. Realizou-se análise temática dos dados categorizados. O estudo mostrou que fazem parte da vivência da mulher que engravida após os 35 anos, o desejo de ser mãe; aspectos positivos relativos à idade, maturidade, discernimento, profissão mais definida, aspectos que interferem favoravelmente na relação com a criança; e como aspectos negativos, cansaço, discriminação geracional e problemas de saúde. A espiritualidade, religiosidade e rede de apoio fortalecem a mulher para o enfrentamento de possíveis dificuldades. O planejamento familiar mostrou-se inexistente para as participantes e conclui-se que ao postergar a gravidez a mulher reduz a sua prole. Os resultados evidenciaram que para o planejamento da assistência o enfermeiro ou profissional de saúde deve trabalhar em equipe e conhecer o risco e a vulnerabilidade (individual, social e programática) da sua cliente. Deve considerar a situação socioeconômica e aspectos da vida familiar e afetiva, profissional, jornada de trabalho, trabalho doméstico, rede de apoio e acessibilidade aos serviços. A fim de detectar aspectos individuais, coletivos e contextuais que aumentam a suscetibilidade da cliente. A escuta qualificada é a primeira providência para identificar as necessidades e singularidades das clientes e definir a estratégia de cuidados.

Background: The Centers for Disease Control and Prevention and the US Department of Health and Human Services promote breastfeeding as a strategy for reducing childhood overweight. We evaluated the relation between infant feeding and the development of overweight and obesity throughout life course. Methods: We investigated the association between infant feeding and obesity among 35 526 participants in the Nurses' Health Study II who were followed prospectively from 1989 to 2001. Mothers of participants provided information by mailed questionnaires on the duration of breast- and bottle-feeding, as well as the type of milk or milk substitute in the bottle. Information on body shape at ages 5 and 10, weight at age 18, current weight between 1989 and 2001, and height was reported by the participants. Results: The duration of breastfeeding, including exclusive breastfeeding, was not related to being overweight (25 body mass index (BMI)

To identify common variants influencing body mass index (BMI), we analyzed genome-wide association data from 16,876 individuals of European descent. After previously reported variants in FTO, the strongest association signal (rs17782313, P = 2.9 x 10(-6)) mapped 188 kb downstream of MC4R (melanocortin-4 receptor), mutations of which are the leading cause of monogenic severe childhood-onset obesity. We confirmed the BMI association in 60,352 adults (per-allele effect = 0.05 Z-score units; P = 2.8 x 10(-15)) and 5,988 children aged 7-11 (0.13 Z-score units; P = 1.5 x 10(-8)). In case-control analyses (n = 10,583), the odds for severe childhood obesity reached 1.30 (P = 8.0 x 10(-11)). Furthermore, we observed overtransmission of the risk allele to obese offspring in 660 families (P (pedigree disequilibrium test average; PDT-avg) = 2.4 x 10(-4)). The SNP location and patterns of phenotypic associations are consistent with effects mediated through altered MC4R function. Our findings establish that common variants near MC4R influence fat mass, weight and obesity risk at the population level and reinforce the need for large-scale data integration to identify variants influencing continuous biomedical traits.

Dans le cadre de cette étude, nous nous sommes intéressés aux infirmières soignantes qui possèdent un diplôme d’études collégiales ou un baccalauréat. L’infirmière est celle ayant reçu une formation collégiale et la clinicienne, une formation universitaire de premier cycle. Au niveau législatif, selon que l’infirmière soit titulaire ou non d’un baccalauréat, le champ de pratique autorisé demeure le même. Cependant, l’étendue de la pratique pourrait varier selon la formation reçue et le poste occupé. Cette étude descriptive et interprétative de type qualitatif avait pour but de décrire l’étendue de la pratique des infirmières et des infirmières cliniciennes dans une unité de médecine et de chirurgie et d’identifier les facteurs organisationnels influençant l’étendue de la pratique. Le cadre de référence ayant été utilisé est le cadre d’analyse de l’étendue de la pratique conçu pour le Collège des infirmières autorisées de la Nouvelle-Écosse (CRNNS, 2005). Selon ce cadre, l’étendue optimale de la pratique des infirmières se divise en 4 catégories distinctes, soit le processus de soins infirmiers, les pratiques relationnelles, le leadership ainsi que l’enseignement et le transfert des connaissances. La collecte de données a été conduite au moyen d’entrevues individuelles semi-structurées auprès de 8 informateurs clés soit trois infirmières et trois infirmières cliniciennes d’une unité de médecine et de chirurgie et de deux gestionnaires. Les résultats semblent montrer que les différentes composantes de l’étendue de la pratique sont mobilisées tant chez les infirmières que chez les infirmières cliniciennes interviewées. Toutefois, les résultats semblent montrer que certaines composantes, soit le leadership et le transfert des connaissances, semblent davantage mobilisées chez les infirmières cliniciennes. De plus, l’étude a permis d’identifier les différents facteurs organisationnels qui semblent influencer l’étendue de la pratique des infirmières et des infirmières cliniciennes. La charge de travail semble être le principal facteur d’influence. Nous avons aussi pu constater l’influence certaines composantes reliées à l’environnement de travail et à la composition et aux caractéristiques des équipes de soins.

Contexte : Pour favoriser l’allaitement, la Condition 3 de l’Initiative des amis des bébés (IAB) (OMS / UNICEF) vise à offrir une information complète aux femmes enceintes. Or, cette condition est implantée de façon variable dans les CLSC de Montréal car les intervenants de la santé ne semblent pas confortables à faire la promotion de l’allaitement en prénatal, surtout dans les milieux « québécois » et défavorisés. Objectif : Explorer les expériences personnelles et professionnelles des infirmières en santé communautaire qui sont reliées à la promotion de l’allaitement en prénatal en milieu défavorisé. Dans la présente étude, les informations moins souvent transmises, soit les risques du non-allaitement ainsi que la recommandation de poursuite de l’allaitement jusqu’à deux ans ou au-delà, ont été examinées. Méthodologie : La collecte des données de cette recherche qualitative s’est effectuée auprès d’infirmières de huit CLSC montréalais offrant des services à une population importante de femmes défavorisées, francophones, nées au Canada et ce, sous forme d’entrevues individuelles (n=12 infirmières) et d’entrevues de groupe (n=36 infirmières). Résultats : Les principaux facteurs favorables au niveau de confort des infirmières à faire la promotion de l’allaitement sont d’avoir suivi 20 heures ou plus de formation en allaitement dans les cinq dernières années, et d’avoir des croyances profondes positives quant à la valeur de l’allaitement comparativement aux préparations commerciales pour nourrissons (PCN). Craindre de susciter la culpabilité nuit à la promotion de l’allaitement. De plus, les infirmières exposées à la culture d’allaitement pendant l’enfance, ayant eu une expérience personnelle d’allaitement positive, qui perçoivent que leur rôle est d’encourager les mères à allaiter, ou qui recommandent rarement de donner des PCN en postnatal en cas de problèmes d’allaitement, sont plus confortables à informer les femmes enceintes des risques du non-allaitement. Conclusion : Plusieurs infirmières semblent manquer de connaissances sur la qualité supérieure de l’allaitement par rapport à l’alimentation artificielle et sur les risques du non-allaitement. De plus, il semble que plusieurs infirmières n’aient pas les habiletés cliniques optimales pour soutenir les mères dans leur allaitement. Des formations appropriées aideraient les infirmières à avoir davantage confiance dans leur capacité à soutenir les mères en postnatal, ainsi qu’à promouvoir l’allaitement en prénatal. Finalement, les infirmières devraient prendre conscience de leurs biais personnels, afin d’en réduire les impacts négatifs sur leur pratique professionnelle.

Cette thèse s’intéresse à l’amélioration des soins et des services de santé et touche aux relations entre 3 grands thèmes de l’analyse des organisations de santé : la gouvernance, le changement et les pratiques professionnelles. En nous appuyant sur l’analyse organisationnelle contemporaine, nous visons à mieux comprendre l’interface entre l’organisation et les pratiques cliniques. D’une part, nous souhaitons mieux comprendre comment l’organisation structure et potentialise les pratiques des acteurs. D’autre part, dans une perspective d’acteurs stratégiques, nous souhaitons mieux comprendre le rôle des pratiques des professionnels dans l’actualisation de leur profession et dans la transformation et l’évolution des organisations. Notre étude se fonde sur l’hypothèse qu’une synergie accrue entre l’organisation et les pratiques des professionnels favorisent l’amélioration de la qualité des soins et des services de santé. En 2004, le gouvernement ontarien entreprend une importante réforme des soins et services dans le domaine du cancer et revoit les rôles et mandats du Cancer Care Ontario, l’organisation responsable du développement des orientations stratégiques et du financement des services en cancer dans la province. Cette réforme appelle de nombreux changements organisationnels et cliniques et vise à améliorer la qualité des soins et des services dans le domaine de l’oncologie. C’est dans le cadre de cette réforme que nous avons analysé l’implantation d’un système de soins et de services pour améliorer la performance et la qualité et analysé le rôle des pratiques professionnelles, spécifiquement les pratiques infirmières, dans la transformation de ce système. La stratégie de recherche utilisée correspond à l’étude approfondie d’un cas correspondant à l’agence de soins et de services en oncologie en Ontario, le Cancer Care Ontario, et des pratiques professionnelles infirmières évoluant dans ce modèle. Le choix délibéré de ce cas repose sur les modalités organisationnelles spécifiques à l’Ontario en termes de soins en oncologie. La collecte de données repose sur 3 sources principales : les entrevues semi-structurées (n=25), l’analyse d’une abondante documentation et les observations non participatives. La thèse s’articule autour de trois articles. Le premier article vise à définir le concept de gouvernance clinique. Nous présentons l’origine du concept et définissons ses principales composantes. Concept aux frontières floues, la gouvernance clinique est axée sur le développement d’initiatives cliniques et organisationnelles visant à améliorer la qualité des soins de santé et la sécurité des patients. L’analyse de la littérature scientifique démontre la prédominance d’une vision statique de la gouvernance clinique et d’un contrôle accentué des pratiques professionnelles dans l’atteinte de l’efficience et de l’excellence dans les soins et les services. Notre article offre une conception plus dynamique de la gouvernance clinique qui tient compte de la synergie entre le contexte organisationnel et les pratiques des professionnels et soulève les enjeux reliés à son implantation. Le second article s’intéresse à l’ensemble des leviers mobilisés pour institutionnaliser les principes d’amélioration continue de la qualité dans les systèmes de santé. Nous avons analysé le rôle et la portée des leviers dans l’évolution du système de soins en oncologie en Ontario et dans la transformation des pratiques cliniques. Nos données empiriques révèlent 3 phases et de nombreuses étapes dans la transformation du système. Les acteurs en position d’autorité ont mobilisé un ensemble de leviers pour introduire des changements. Notre étude révèle que la transformation du Cancer Care Ontario est le reflet d’un changement radical de type évolutif où chacune des phases est une période charnière dans la transformation du système et l’implantation d’initiatives de qualité. Le troisième article pose un regard sur un levier spécifique de transformation, celui de la communauté de pratique, afin de mieux comprendre le rôle joué par les pratiques professionnelles dans la transformation de l’organisation des soins et ultimement dans le positionnement stratégique de la profession infirmière. Nous avons analysé les pratiques infirmières au sein de la communauté de pratique (CDP) des infirmières en pratique avancée en oncologie. En nous appuyant sur la théorie de la stratégie en tant que pratique sociale, nos résultats indiquent que l’investissement de la profession dans des domaines stratégiques augmente les capacités des infirmières à transformer leurs pratiques et à transformer l’organisation. Nos résultats soulignent le rôle déterminant du contexte dans le développement de capacités stratégiques chez les professionnels. Enfin, nos résultats révèlent 3 stratégies émergentes des pratiques des infirmières : une stratégie de développement de la pratique infirmière en oncologie, une stratégie d’institutionnalisation des politiques de la CDP dans le système en oncologie et une stratégie de positionnement de la profession infirmière. Les résultats de notre étude démontrent que l’amélioration de la qualité des soins et des services de santé est située. L’implantation de transformations dans l’ensemble d’un système, tel que celui du cancer en Ontario, est tributaire d’une part, des capacités d’action des acteurs en position d’autorité qui mobilisent un ensemble de leviers pour introduire des changements et d’autre part, de la capacité des acteurs à la base de l’organisation à s’approprier les leviers pour développer un projet professionnel, améliorer leurs pratiques professionnelles et transformer le système de soins.
