Docosahexaenoic (DHA) and arachidonic acids (AA) are polyunsaturated fatty acids (PUFAs), major components of brain tissue and neural systems, and the precursors of a number of biologically active metabolites with functions in inflammation resolution, neuroprotection and other actions. As PUFAs are highly susceptible to peroxidation, we hypothesised whether cigarette smokers would present altered PUFAs levels in plasma and erythrocyte phospholipids. Adult males from Indian, Sri-Lankan or Bangladeshi genetic backgrounds who reported smoking between 20 and 60 cigarettes per week were recruited. The control group consisted of matched non-smokers. A blood sample was taken, plasma and erythrocyte total lipids were extracted, phospholipids were separated by thin layer chromatography, and the fatty acid content analysed by gas chromatography. In smokers, dihomo-gamma-linolenic acid, the AA precursor, was significantly reduced in plasma and erythrocyte phosphatidylcholine. AA and DHA were significantly reduced in erythrocyte sphingomyelin. Relatively short term smoking has affected the fatty acid composition of plasma and erythrocyte phospholipids with functions in neural tissue composition, cell signalling, cell growth, intracellular trafficking, neuroprotection and inflammation, in a relatively young population. As lipid peroxidation is pivotal in the pathogenesis of atherosclerosis and neurodegenerative diseases such as Alzheimer disease, early effects of smoking may be relevant for the development of such conditions.

L’encéphalopathie néonatale (EN) affecte plus de 0.8% des nouveau-nés à terme et est la deuxième cause d’handicap neurologique durant l’enfance. Le traitement des enfants atteints d’EN consiste en une prise en charge symptomatique et en l’hypothermie (HT). Néanmoins, plus de 50% des patients traités par HT gardent des séquelles neurologiques majeures. Les données provenant du Canadian Cerebral Palsy Registry montrent que l’HT prévient seulement 4% des cas de paralysie cérébrale (PC). Le fait que l’HT ait des effets neuroprotecteurs chez certains nouveau-nés, mais pas chez tous les nouveau-nés atteints d’EN n’est pas encore totalement élucidé. Il a été rapporté que les nouveau-nés exposés à l’inflammation (sepsis néonatal, infections placentaires) et à l’hypoxie-ischémie (HI) semblent avoir moins d’effets bénéfiques de l’HT que ceux qui ont été seulement exposés à l’HI. Des études précliniques ont établi que l’HT avait des effets neuroprotecteurs dans les cas d’encéphalopathies hypoxique-ischémiques chez les nouveau-nés à terme ou peu prématurés (plus de 35 semaines d’âge gestationnel). Cependant, peu d’études ont investigué les effets de l’HT lorsque l’EN résulte de la combinaison d’HI et d’infection/inflammation, alors qu’il s’agit du scénario pathophysiologique le plus fréquemment rencontré chez les nouveau-nés humains atteints d’EN. Ceci nous a amené à tester les effets neuroprotecteurs de l’HT dans un modèle animal (rat) d’EN induite par l’inflammation et l’HI. Des ratons Lewis au jour postnatal 12 sont injectés avec du lipopolysaccharide (LPS) d’E.coli afin de recréer l’inflammation, puis sont soumis à l’ischémie (ligature de la carotide commune droite) et à l’hypoxie (8% O[indice inférieur 2], 1h30). Les ratons sont ensuite traités ou non par l’HT (32 ± 0.5°C, 4 h). L‘étendue des lésions cérébrales ainsi que les cascades inflammatoires et oxydatives ont été étudiées. Nos résultats montrent que l’HT prévient l’étendue des lésions dans la composante de pénombre ischémique (néocortex et hippocampe). Cet effet neuroprotecteur est indépendant d’un effet anti-inflammatoire cérébral de l’HT portant sur le système de l’interleukine-1. Nos données indiquent que l’HT module l’expression des enzymes responsables de l’élimination des réactifs de l’oxygène (ROS). En conclusion, nos résultats montrent que l'HT a des effets neuroprotecteurs dans un modèle d’EN induite par l’exposition combinée au LPS et à l’HI. Ce projet devrait ouvrir des perspectives translationnelles pour prévenir la PC au-delà de l'hypothermie seule, comme par exemple celle visant à tester le bénéfice de la combinaison thérapeutique d’HT jointe au blocage de cytokines pro-inflammatoires.

Retinitis pigmentosa (RP) is a degenerative retinal disease leading to photoreceptor cell loss. In 2011, our group identified the synthetic progesterone ‘Norgestrel’ as a potential treatment for RP. Subsequent research showed Norgestrel to work through progesterone receptor membrane component 1 (PGRMC1) activation and upregulation of neuroprotective basic fibroblast growth factor (bFGF). Using trophic factor deprivation of 661W photoreceptor-like cells, we aimed to further elucidate the mechanism leading to Norgestrel-induced neuroprotection. In the present manuscript, we show by flow cytometry and live-cell immunofluorescence that Norgestrel induces an increase in cytosolic calcium in both healthy and stressed 661Ws over 24h. Specific PGRMC1 inhibition by AG205 (1 μM) showed this rise to be PGRMC1-dependent, primarily utilising calcium from extracellular sources, for blockade of L-type calcium channels by verapamil (50 μM) prevented a Norgestrel-induced calcium influx in stressed cells. Calcium influx was also shown to be bFGF-dependent, for siRNA knock down of bFGF prevented Norgestrel-PGRMC1 induced changes in cytosolic calcium. Notably, we demonstrate PGRMC1-activation is necessary for Norgestrel-induced bFGF upregulation. We propose that Norgestrel protects through the following pathway: binding to and activating PGRMC1 expressed on the surface of photoreceptor cells, PGRMC1 activation drives bFGF upregulation and subsequent calcium influx. Importantly, raised intracellular calcium is critical to Norgestrel's protective efficacy, for extracellular calcium chelation by EGTA abrogates the protective effects of Norgestrel on stressed 661W cells in vitro.

Retinitis pigmentosa (RP) is a degenerative disease leading to photoreceptor cell loss. Mouse models of RP, such as the rd10 mouse (B6.CXBl-Pde6brd10/J), have enhanced our understanding of the disease, allowing for development of potential therapeutics. In 2011, our group first demonstrated that the synthetic progesterone analogue ‘Norgestrel’ is neuroprotective in two mouse models of retinal degeneration, including the rd10 mouse. We have since elucidated several mechanisms by which Norgestrel protects stressed photoreceptors, such as upregulating growth factors. This study consequently aimed to further characterize Norgestrel’s neuroprotective effects. Specifically, we sought to investigate the role that microglia might play; for microglial-derived inflammation has been shown to potentiate neurodegeneration. Dams of post-natal day (P) 10 rd10 pups were given a Norgestrel-supplemented diet (80mg/kg). Upon weaning, pups remained on Norgestrel. Tissue was harvested from P15-P50 rd10 mice on control or Norgestrel-supplemented diet. Norgestrel-diet administration provided significant retinal protection out to P40 in rd10 mice. Alterations in microglial activity coincided with significant protection, implicating microglial changes in Norgestrel-induced neuroprotection. Utilizing primary cultures of retinal microglia and 661W photoreceptor-like cells, we show that rd10 microglia drive neuronal cell death. We reveal a novel role of Norgestrel, acting directly on microglia to reduce pro-inflammatory activation and prevent neuronal cell death. Norgestrel effectively suppresses cytokine, chemokine and danger-associated molecular pattern molecule (DAMP) expression in the rd10 retina. Remarkably, Norgestrel upregulates fractalkine-CX3CR1 signaling 1 000-fold at the RNA level, in the rd10 mouse. Fractalkine-CX3CR1 signaling has been shown to protect neurons by regulating retinal microglial activation and migration. Ultimately, these results present Norgestrel as a promising treatment for RP, with dual actions as a neuroprotective and anti-inflammatory agent in the retina.

La asfixia perinatal es la principal causa de muerte en la primera semana de vida la nivel mundial, los niños que sufren esta complicación y sobreviven pueden presentar trastornos neurológicos de diferente nivel de compromiso que inciden en su desarrollo personal y social. Las cifras de muerte por este problema de salud han disminuido de manera importante, sin embargo en el reporte de la Organización Mundial de Salud (OPS) del 2010, la asfixia perinatal es causa del 29% de muertes infantiles en los países de América Latina y el Caribe 2. Es necesario conocer además la extensión del daño neurológico que sufren estos niños, con este fin se desarrolló un estudio piloto en el Hospital Universitario Mayor Mederi de Bogotá, en el cual se determinó la concentración de un marcador metabólico de daño cerebral, la proteína S100B en suero de 60 recién nacidos sanos, con el objetivo de analizar la asociación del mismo con el peso al nacer, la edad gestacional y el diagnóstico. Los resultados no mostraron diferencias significativas entre este marcador y las variables analizadas que puede asociarse al pequeño número de pacientes, sin embargo han sentado las bases para el desarrollo de un estudio que incluya varios hospitales de Bogotá y sobre todo la determinación del mismo en recién nacidos con diagnóstico de hipoxia en el período perinatal, lo cual aportará información del grado de la alteración que puedan tener a nivel cerebral y contribuya al mejor manejo evolutivo con la aplicación de medidas de intervención en estadios tempranos de la vida.
