 The research conducted was based on regenerating the dying heart cells (cardiomyocytes) by employing novel therapeutic proteins and their respective co-encapsulated nanoformulation with an antihypertensive drug. This promising therapeutic strategy to revive the heart can help in the treatment of several cardiac pathologies such as myocardial infarction and drug induced cardiotoxicity.

This chapter reviews the support of cardiovascular function in the face of
many compromises to the system. It focuses on two of the most prevalent and fatal diseases affecting the heart: coronary heart disease and heart failure. These diseases are also a common comorbidity in elderly patients admitted to critical care units. The first section on coronary heart disease reviews the pathophysiological concepts of myocardial ischaemia and associated complications, with detailed consideration of the clinical implications, assessment and associated management. Heart failure is discussed in terms of the body’s compensatory mechanisms and the clinical sequelae and associated clinical features of heart failure. Nursing and medical management is outlined including the management of acute exacerbations of heart failure. Finally, other cardiovascular disorders commonly managed in critical care units are reviewed, ranging from other forms of heart failure to hypertensive emergencies and aortic aneurysms. The case study presented at the end of the chapter highlights the key aspects of the management of coronary heart disease and heart failure in patients admitted to critical care units.

Objective: Type-D personality is a construct that describes a tendency to simultaneously experience negative emotions and inhibit self-expression for fear of negative social judgement. The link between type-D and poor health outcomes may be partly mediated by two prominent psychosocial mechanisms, poor-quality health-related behaviour and poor perceived-social support. Method: The present study replicated and extended a 2008 UK and Irish prevalence study, utilising a sample from the Australian general population. The study aimed to investigate the relationship between type-D personality and subjective levels of social support, health-related behaviours and neuroticism, as well as examining the estimated prevalence rate of type-D in the general Australian population. Nine hundred and fifty five Australian participants over the age of 18 (194 male and 761 female) completed four measures assessing levels of type-D personality, quality of health-related behaviours, perceived-social support and neuroticism. Results: As hypothesised, the estimated prevalence rate was not found to be significantly different from the rate obtained by Williams etal. (2008). In addition, type-D individuals reported significantly lower perceived-social support and poorer-quality health behaviours than non-type-D individuals. Conclusions: The results of this study provide further support for the association of type-D personality with poor health-related behaviours and poor perceived-social support, as well as demonstrating the applicability of the type-D construct to the Australian general population for the first time. General healthcare applications are discussed, as well as the potential for type-D personality research to influence public illness prevention in general.

BACKGROUND: Much attention has been given to identifying and supporting the minority of patients who develop severe clinical depression after a cardiac event. However, relatively little has been given to supporting the many patients who experience transient but significant emotional disturbance that we term the 'cardiac blues'. OBJECTIVE: The aim of this study was to investigate patients' preferences regarding information provision about cardiac blues. METHODS: One hundred and sixty consecutive cardiac patients admitted to two Victorian hospitals in Australia were interviewed three times over six months. They were asked about emotional issues, including information provision preferences. RESULTS: Four out of five (81%) patients would like to have received information about the cardiac blues, but only a minority received this information. CONCLUSION: Most patients want to know about cardiac blues. The development and evaluation of resources for health professionals and patients to support recovery through cardiac blues appears warranted.

BACKGROUND: Cardiac rehabilitation (CR) is an underutilized evidence-based treatment. We described trends in referral to outpatient CR (OCR) and the factors associated with referral. DESIGN: Cross-sectional survey data provided by Hunter residents aged 20 years or older discharged from public hospitals in the region between 2002 and 2007 with an OCR eligible diagnosis were extracted from the Hunter New England Heart and Stroke Register database. METHODS: Trends in referral were determined using the chi test for trend. Factors associated with referral were examined using multiple logistic regression. RESULTS: Sixty-five percent (4971 of 7678) of patients provided sufficient data for inclusion in the analysis. Approximately half of the patients reported being referred to OCR. No increase over time was observed. Factors associated with referral were age less than 70 years, male sex, being married, urban residence, at least one admission to the tertiary referral hospital for cardiology, at least one admission for acute myocardial infarction, revascularization, no admissions for congestive heart failure, a self-reported history of high cholesterol, and no history of stroke or atrial fibrillation. CONCLUSION: Access to this treatment of proven benefit remained suboptimal despite the provision of new programs and expansion of existing programs. Automatic referral, which is recommended in Australia, should be standard practice.

Prediction of patient outcomes is critical to plan resources in an hospital emergency department. We present a method to exploit longitudinal data from Electronic Medical Records (EMR), whilst exploiting multiple patient outcomes. We divide the EMR data into segments where each segment is a task, and all tasks are associated with multiple patient outcomes over a 3, 6 and 12 month period. We propose a model that learns a prediction function for each task-label pair, interacting through two subspaces: the first subspace is used to impose sharing across all tasks for a given label. The second subspace captures the task-specific variations and is shared across all the labels for a given task. The proposed model is formulated as an iterative optimization problems and solved using a scalable and efficient Block co-ordinate descent (BCD) method. We apply the proposed model on two hospital cohorts - Cancer and Acute Myocardial Infarction (AMI) patients collected over a two year period from a large hospital emergency department. We show that the predictive performance of our proposed models is significantly better than those of several state-of-the-art multi-task and multi-label learning methods.

Electronic Medical Record (EMR) has established itself as a valuable resource for large scale analysis of health data. A hospital EMR dataset typically consists of medical records of hospitalized patients. A medical record contains diagnostic information (diagnosis codes), procedures performed (procedure codes) and admission details. Traditional topic models, such as latent Dirichlet allocation (LDA) and hierarchical Dirichlet process (HDP), can be employed to discover disease topics from EMR data by treating patients as documents and diagnosis codes as words. This topic modeling helps to understand the constitution of patient diseases and offers a tool for better planning of treatment. In this paper, we propose a novel and flexible hierarchical Bayesian nonparametric model, the word distance dependent Chinese restaurant franchise (wddCRF), which incorporates word-to-word distances to discover semantically-coherent disease topics. We are motivated by the fact that diagnosis codes are connected in the form of ICD-10 tree structure which presents semantic relationships between codes. We exploit a decay function to incorporate distances between words at the bottom level of wddCRF. Efficient inference is derived for the wddCRF by using MCMC technique. Furthermore, since procedure codes are often correlated with diagnosis codes, we develop the correspondence wddCRF (Corr-wddCRF) to explore conditional relationships of procedure codes for a given disease pattern. Efficient collapsed Gibbs sampling is derived for the Corr-wddCRF. We evaluate the proposed models on two real-world medical datasets - PolyVascular disease and Acute Myocardial Infarction disease. We demonstrate that the Corr-wddCRF model discovers more coherent topics than the Corr-HDP. We also use disease topic proportions as new features and show that using features from the Corr-wddCRF outperforms the baselines on 14-days readmission prediction. Beside these, the prediction for procedure codes based on the Corr-wddCRF also shows considerable accuracy.

A cintilografia com radioisótopos em repouso tem despertado muito interesse nos últimos anos para pesquisa da viabilidade miocárdica na disfunção isquêmica avançada do VE, porque esta pode ser reversível e porque há melhora prognóstica neste contexto. O objetivo desse trabalho é buscar identificar a acurácia da cintilografia tomográfica Tc-99m-Sestamibi, sob a infusão de dobutamina em baixa dose. Foram protocolados trinta pacientes que sofreram um infarto do miocárdio com padrão “Q”, com dissinergia ao ecocardiograma, e submetidos a estudos perfusionais em dias diferentes, com Tc-99m-Sestamibi − dobutamina e Tl-201 em repouso-redistribuição 4 horas, usado como padrão ouro. Não ocorreram complicações com nenhum paciente submetido ao protocolo do estudo. Os resultados revelaram: sensibilidade = 85%, especificidade = 87%, valor preditivo positivo = 96%, valor preditivo negativo = 65 % e acurácia de = 85,2%. A razão de probabilidade positiva foi de 6,68 e a razão de probabilidade negativa de 0,17. Confrontando-se nossos dados com uma metanálise de resultados para o Tc-99m-Sestamibi em repouso, a sensibilidade no presente estudo foi 85% vs. 81% e a especificidade de 87% vs. 60%. Em relação aos dados da literatura para Tl-201: 85% vs. 90% e 87% vs. 54%, sensibilidade e especificidade, respectivamente. Em conclusão, o Tc-99m-Sestamibi sob a infusão de dobutamina parece ser um método promissor para detectar viabilidade superior a esta modalidade de cintilografia em repouso, com valores preditivos comparáveis à literatura disponível para esse mesmo método: 85,2% vs. 87%, 96% vs. 90% e 65% vs. 83%, para acurácia, valor preditivo positivo e valor preditivo negativo, respectivamente.

OBJETIVO: Analisar, comparativamente, a obtenção minimamente invasiva com o uso do MINI-HARVEST® e com instrumental tradicional adaptado. MÉTODO: de junho de 1996 a janeiro de 1999, 63 pacientes submetidos à cirurgia de revascularização do miocárdio tiveram suas veias safenas retiradas segundo técnica minimamente invasiva. Nos 30 primeiros pacientes da série utilizou-se método de visão direta com auxílio de dois afastadores de Langenbeck, e nos 33 restantes utilizou-se o MINI-HARVEST®. RESULTADOS: A idade média dos pacientes era de 61 ± 8,75 anos, sendo 52 homens e 11 mulheres. Quarenta e cinco pacientes eram diabéticos, 45 apresentavam sobrepeso/obesidade, 25 eram tabagistas ativos, 32 apresentavam história pregressa de infarto do miocárdio. O tempo médio de retirada da veia safena com afastadores Langenbeck foi de 34,2 ± 8,14 minutos e com o MINI-HARVEST® de 39,20 ± 9,12 minutos. A extensão de veia retirada foi similar nos dois grupos, variando de 10 a 30 cm. Houve uma deiscência superficial no grupo com afastadores de Langenbeck. Houve necessidade de reversão para método tradicional de retirada em dois casos do grupo MINI-HARVEST® e um do grupo Langenbeck. A incidência de infarto transoperatório foi 4,5% (três) no grupo Langenbeck e 3,1%(dois) no grupo MINI-HARVEST®. CONCLUSÕES: Podemos concluir que o método de obtenção de veia safena minimamente invasivo sob visão direta é efetivo e seguro, tanto com o uso de instrumentos tradicionais adaptados para este fim, como com afastadores especialmente constituídos, ressaltando-se que o MINI-HARVEST® dispensa a presença de um auxiliar.
