OBJECTIVE: To investigate relationships between institutional mistrust (systematic discrimination, organizational suspicion, and conspiracy beliefs), HIV risk behaviors, and HIV testing in a multiethnic sample of men who have sex with men (MSM), and to test whether perceived susceptibility to HIV mediates these relationships for White and ethnic minority MSM. METHOD: Participants were 394 MSM residing in Central Arizona (M age = 37 years). Three dimensions of mistrust were examined, including organizational suspicion, conspiracy beliefs, and systematic discrimination. Assessments of sexual risk behavior, HIV testing, and perceived susceptibility to HIV were made at study entry (T1) and again 6 months later (T2). RESULTS: There were no main effects of institutional mistrust dimensions or ethnic minority status on T2 risk behavior, but the interaction of systematic discrimination and conspiracy beliefs with minority status was significant such that higher levels of systematic discrimination and more conspiracy beliefs were associated with increased risk only among ethnic minority MSM. Higher levels of systematic discrimination were significantly related to lower likelihood for HIV testing, and the interaction of organizational suspicion with minority status was significant such that greater levels of organizational suspicion were related to less likelihood of having been tested for HIV among ethnic minority MSM. Perceived susceptibility did not mediate these relationships. CONCLUSION: Findings suggest that it is important to look further into the differential effects of institutional mistrust across marginalized groups, including sexual and ethnic minorities. Aspects of mistrust should be addressed in HIV prevention and counseling efforts.

Understanding confinement and its complex workings between individuals and society has been the stated aim of carceral geography and wider studies on detention. This project contributes ethnographic insights from multiple sites of incarceration, working with an under-researched group within confined populations. Focussing on young female detainees in Scotland, this project seeks to understand their experiences of different types of ‘closed’ space. Secure care, prison and closed psychiatric facilities all impact on the complex geographies of these young women’s lives. The fluid but always situated relations of control and care provide the backdrop for their journeys in/out and beyond institutional spaces. Understanding institutional journeys with reference to age and gender allows an insight into the highly mobile, often precarious, and unfamiliar lives of these young women who live on the margins. This thesis employs a mixed-method qualitative approach and explores what Goffman calls the ‘tissue and fabric’ of detention as a complex multi-institutional practice. In order to be able to understand the young women’s gendered, emotional and often repetitive experiences of confinement, analysis of the constitution of ‘closed space’ represents a first step for inquiry. The underlying nature of inner regimes, rules and discipline in closed spaces, provide the background on which confinement is lived, perceived and processed. The second part of the analysis is the exploration of individual experiences ‘on the inside’, ranging from young women’s views on entering a closed institution, the ways in which they adapt or resist the regime, and how they cope with embodied aspects of detention. The third and final step considers the wider context of incarceration by recovering the young women’s journeys through different types of institutional spaces and beyond. The exploration of these journeys challenges and re-develops understandings of mobility and inertia by engaging the relative power of carceral archipelagos and the figure of femina sacra. This project sits comfortably within the field of carceral geography while also pushing at its boundaries. On a conceptual level, a re-engagement with Goffman’s micro-analysis challenges current carceral-geographic theory development. Perhaps more importantly, this project pushes for an engagement with different institutions under the umbrella of carceral geography, thus creating new dialogues on issues like ‘care’ and ‘control’. Finally, an engagement with young women addresses an under-represented population within carceral geography in ways that raise distinctly problematic concerns for academic research and penal policy. Overall, this project aims to show the value of fine grained micro-level research in institutional geographies for extending thinking and understanding about society’s responses to a group of people who live on the margins of social and legal norms.

While there is growing use of online counselling, little is known about its interactional organisation and how it compares to telephone counselling. This is despite past research suggesting that both counsellors and clients report the impact of the different modalities on the presentation and management of the counselling interaction. This paper compares the interactional affordances of telephone and online web counselling in opening sequences on Kids Help Line, a 24-hour Australian counselling service for children and young people up to the age of 25. We examine two ways that counsellors show active listening through response tokens and formulations. The analysis describes how counsellors’ use of minimal response tokens facilitate the clients’ problem presentation and are used in the management of turn taking and sequence organisation. For example, counsellors use the response token Mm hm to show that they understand that the client’s unit of talk to is not yet complete, and to affirm or invite the client to continue speaking. Formulations in phone and web counselling are another way that counsellors display active listening to re-present stretches of the clients’ preceding talk. In phone and web counselling, however, the respective modalities can complicate matters of turn transition and sequence organisation. By examining actual phone and online counselling sessions, this paper offers empirical demonstrations of the interactional affordances of phone and online counselling, and shows how the institutional practice of active listening is accomplished across different counselling modalities

This conversation analytical study analyses the interactional practices adopted by speech therapists and their clients during their training in voice therapy. This study also describes how learning takes place during the therapy process. In contrast to traditional voice therapy studies, change is examined here by using qualitative research methodology, namely conversation analysis. This study describes the structures of interaction in voice therapy, shows how the shortcomings in the client s performance are evaluated and corrected and finally, how the voice training sequence and the participation changes during therapy. The database consists of 51 videotaped voice therapy sessions from six clients with voice disorders. The analytic focus is on the practices in one voice training exercise of the trilled /r/. All the sequences of this exercise (in total 36) and all adjacency pairs within (N = 627) were transcribed and analysed in detail. This study shows that voice training consists of successive model imitation adjacency pairs. This adjacency pair works as a resource in voice training. Furthermore, the use of this particular adjacency pair is an institutional practice in all therapies in this study. The structure of interaction in voice training sequences resembles the practices found in aphasia therapy and in speech therapy of children, as well as the practices of educational and counselling interaction and physiotherapy. More than half of the adjacency pairs were expanded to three (or more) part structures as client s responses were typically followed by therapist s feedback. With their feedback turns, therapists: 1) maintain training practice, 2) evaluate the problem of client s performance, 3) deliver information, 4) activate the client to observe the performance and 5) assist her in correcting the performance. This study describes the four different ways that therapists help their clients to improve the performance after encountering a problem. The longitudinal data shows that learning in therapy is manifested in the changing participation. As clients learn to identify their voice features, they can participate in evaluating or correcting their performances by themselves. This study describes the recurrent professional practices of voice therapists and shows how the institutional commitments of voice therapy are managed in and through talk and interaction. The study also provides detailed description of the management of help in voice training. By describing the interaction in training sequences, this study expands the conception of voice rehabilitation and how it can be researched. The results demonstrate that the learning process and therapy outcomes can be assessed by analysing interaction in therapy. Moreover, this analysis lays the foundation for a novel understanding of the practices in speech therapy and for the development of speech therapy theory. By revealing the activities of interaction, it also makes it possible to discuss them explicitly with speech therapy students. This edition scales the merlons and embrasures that mark the epistemological barriers that contemporary colonising power continually puts in place. Each article harnesses a critical Indigenous perspective in order to challenge conservative approaches or positions, be they concerned with reconciliation, Indigenous-led research, research tools or the nature of Aboriginal being. The first article, by Barry Judd and Emma Barrow, examines reconciliation discourse within the higher education sector and highlights the ways a normative Anglo-Australian identity militates against genuine ‘whitefella’ attempts to ‘reconcile’. The authors stress the importance of inclusive, institutional practice that serves to decentre Anglo-centrism and which, in turn, brings Indigenous peoples more fully into the fold of Australian university life.

Analisa a Questão de Ordem (QO) 411/09, respondida por Michel Temer, então Presidente da Câmara dos Deputados, que alterou o entendimento a respeito do sobrestamento da pauta na Câmara. A opção pela análise de uma questão de ordem justifica-se pelo entendimento de que se trata de um dos instrumentos regimentais mais legítimos e definidores do processo legislativo, pois tem o potencial de orientar os caminhos de uma proposição e criar jurisprudência legislativa, podendo até mesmo definir os rumos da atividade parlamentar, como é o caso da QO 411/09. O estudo pretende, a partir da abordagem teórica e metodológica da vertente francesa da análise do discurso, desvelar as relações de poder no plano das formações discursivas e suas respectivas formações ideológicas no interior do Legislativo vis-à-vis o Executivo, com ênfase no poder de agenda e no equilíbrio entre os Poderes da República. Para tanto, propõe-se analisar pronunciamentos proferidos em Plenário por ocasião da supracitada Questão de Ordem, tendo como referência o discurso político como cena teatral indicado por Charaudeau, de forma a tentar fornecer novos elementos interpretativos aos dilemas institucionais e às escolhas políticas dos atores envolvidos nesse debate. Conclui-se que as estratégias discursivas empregadas pelos atores políticos evidenciaram o processo de significação, circulação e entrecruzamento dos saberes a respeito da relação institucional entre os Poderes. A análise do discurso contribuiu para a compreensão do contexto social da produção discursiva da QO e da alteração do sobrestamento da pauta, além de reiterar a ideia de que a formação discursiva está intimamente relacionada à prática institucional das relações de poder dentro e fora da Câmara dos Deputados. Os embates dos pronunciamentos evidenciam esta premissa. Além disso, a análise da QO foi capaz de revelar a força política e retórica dos pronunciamentos em Plenário no sentido de modificar e, sobretudo, legitimar o entendimento sobre o trancamento da pauta. Ao utilizar a análise do discurso, tendo como pano de fundo o processo legislativo, direito constitucional e a ciência política, pretendeu-se dar novo enfoque à temática do sobrestamento da pauta e, dessa forma, tentar fornecer novos elementos interpretativos aos dilemas institucionais e às escolhas políticas dos atores envolvidos nesse contexto.

Dado que el concepto de salud-enfermedad puede ser abordado desde diversas perspectivas teóricas, en este apartado se presentan de manera sucinta los principales enfoques que se han desarrollado en torno a éste, con la intención de identificar sus alcances. Se busca evidenciar las perspectivas conceptuales de la salud y la promoción de la salud, como un hecho histórico circunscrito y afectado por una realidad social, y en esa medida constructoras no sólo de un saber, sino también de una práctica institucional, que determina los diferentes movimientos desde los cuales las comunidades se dirigen hacia modos de entender y manejar su salud, lo cual no quiere decir que los individuos y colectivos, como seres históricos y activos, no ejerzan de manera autónoma sus propias convicciones y decisiones frente al curso de su vida.

Objective: To examine the test–retest reliability and construct validity of cervical active range of motion and isometric neck muscle strength as measured by the Multi Cervical Rehabilitation Unit (Hanoun Medical Inc., Ontario).
Design: A cross-sectional study.
Setting: Institutional practice.
Subjects: Twenty-one patients with neck pain and 25 healthy volunteers.
Methods: After a trial-run session, active range of motion (AROM) was measured in the subsequent two sessions, with 2–3 days in between. During each session, three measurements were taken for each direction (flexion, extension, lateral flexions and rotations). The measurement of isometric strength was after a 15-minute break following completion of the measurement of AROM. Three measurements were made for each of the six directions (flexion, extension, lateral flexions, protraction and retraction). The software of the Multi Cervical Rehabilitation Unit automatically recorded and calculated the maximum AROM and isometric strength.
Results: There was a good to high level of reliability in the measurement of AROM for both groups of subjects, with intraclass correlation coefficients (ICCs) ranging from 0.81 to 0.96. Results also demonstrated very good to excellent reliability in isometric strength measurement (ICCs ranged from 0.92 to 0.99). Moreover, there was a significant difference in isometric neck muscle strength (p = 0.001) and in AROM (p = 0.034) between the two groups.
Conclusions: The Multi Cervical Rehabilitation Unit was found to be reliable and valid for testing the cervical active range of motion and isometric neck muscle strength for both normal and patient subjects.

Theoretical paternalism and the convenience of working within ‘accepted’ frameworks have appropriated the Indigenous subject within the boundaries of colonial relations. The establishment of post-colonial theory as one of the only ‘acceptable’ frameworks for exploring the Indigenous subject has limited the subject’s theoretical development within the binary of coloniser/colonised. Breaking from this tradition, the Foucauldian concepts of governmentality, ethics and care-ofthe-self will be used as a template for expansion. This paper will explore the passages of the Indigenous subject in theoretical development. It will examine the significance of post-colonial and settler colonial theories in the conceptualisation of the subject, and consider the transformations that occur when the borders established by these theories are crossed. The paper will therefore be comprised of four sections. The first will address the value and limitations of post-colonial and settler colonial theory. The second will posit reasons and implications for why theoretical neglect has occurred. The third will present and critique the Foucauldian concepts of governmentality, ethics and care-of-the-self. Applying Foucault’s concepts to examples of Indigenous offenders in the settler societies of Australia and New Zealand, the final section will examine the impact of the Indigenous subject in Western thought and institutional practice.

This work aims at investigating the strategies of management used by State School Joaquim Jose de Medeiros, located in the city of Cruzeta - RN, as award practice of "School of Reference in Management", with a state title in 1999 at the beginning of the decade from 2000, being "Outstanding Brazil" from best management strategies nationally. In 2005, it is ranked among the best experiences of the state. It has been become complex by experiencing in that institution, but at the same time an enjoyable practice, mainly by using a social research, in a qualitative and quantitative approach, and a analysis of the institutional performance of two management practices: from 1995 to 2002 and from 2003 to 2007 . To understand the object, a investigative questionnaire was used with representatives of the school community and carried out an analysis of assessment of institutional and documents of institutional organization of the periods above in order to identify management strategies and examine the influences of the share capital, public policies , the institutional evaluation, in the process of democratic construction that has allowed achievements, highlighting Cruzeta, in the Seridó in the state of Rio Grande do Norte, semi-arid northeast, in the national and international juncture of well-successful experiences in management in institutional categories and leadership. This study becomes relevant to allow understand that the cultural practices of seridoenses that symbolize a capital generated by principles of trust, reciprocity, civic culture, as a driving force in overcoming the problems community and the conquest of democratic principles, worked for the school become a national reference in educational administration. Another aspect that made it natural that school was the collective creation of educational project with the participation and civic culture of self-evaluation as a way to understand the institutional practice and establish guidelines to improve the teaching and practice management in its many dimensions educational, social, administrative - financial and legal

This dissertation aims to answer the question: What are the specifics of psychoanalytical clinic with children in neurosis and psychosis and its consequences for the treatment direction? It constitutes a theoretical study based on Freud, Lacan and the current productions of Lacanian psychoanalysts about the clinic with children. It presents some clinical vignettes. To answer this question, were constructed four chapters. The chapter The subject constitution treats the psychoanalysis subjectivity, based on a structure from the relationship with the Other. Key concepts of Lacanian psychoanalysis are shown, necessary to understand what becomes present in clinic with children. The second chapter, The clinic of neurosis, reveals the structure of the subject in its oedipal mooring held by the Name-of the-Father, that separates the mother-child dual relationship. The child neurosis is the effect of psyche constitution and the symptoms are an interpretation of what child picks up from parents and helps him/her on the passage through the Oedipus. The analyst is there to help him/her through this path. The next chapter is entitled The clinic of psychosis. In psychosis the non-occurrence of the Name-of-the-Father is concerned. The subject is stuck in duality with the mother, and becomes what fills the Other s gap. To protect themselves, they have to be in incessant work. The analyst will be a child s partner in daily work already carried out by him/her. The last chapter, The consequences for the treatment direction, shows that the standard analytic treatment works well to the clinic of neurosis. To psychosis it s not true. Psychoanalysts thought about a different way of psychotic children treatment: the practice held in a multiprofessional team work. The practice shared by many has been a team strategy applied to the institutional practice that aims to attenuate the invasive character of the Other, facilitating the partnership between the analyst and the child in treatment and the Other contention

Este trabalho consiste em estudar a prática clínica e institucional na formação do psicólogo sob a perspectiva da integralidade em saúde no Hospital Universitário João de Barros Barreto - HUJBB. A escolha do campo de estudo deveu-se ao conhecimento do hospital enquanto área de trabalho, assim como pelo fato deste ser um dos três únicos locais no SUS que servem de prática para o Curso de Psicologia da UFPA, onde se propicia aos alunos terem contato com a prática clínica e institucional, aplicando referenciais teórico-metodológicos de Psicanálise, análise institucional, processos grupais e de comunicação e conhecimentos de Saúde Pública. Utilizando-se da abordagem qualitativa, os procedimentos de coleta de dados foram a entrevista semi-estruturada, os questionários e a observação participante. Entre os aspectos relevantes o estudo observou: a fragmentação constante do trabalho dos diversos profissionais envolvidos e a busca do psicólogo por um espaço mais consolidado e articulado de atuação na equipe de saúde; a falta de conhecimentos prévios de saúde coletiva por parte dos discentes motivando um esforço concentrado dos supervisores e orientadores de campo na relação ensino-serviço; o sucateamento das instalações físicas e dos equipamentos do hospital gerando dificuldades, mas, simultaneamente, estimulando uma subjetividade expressa na colaboração entre funcionários e alunos; o esforço do Serviço de Psicologia em reconduzir o trabalho centrado no ato médico e no corpo enfermo para uma atenção marcada pela relação com o paciente como sujeito e para os distintos modos de subjetivação psicopatológicos que constituem o problema teórico-metodológico colocado para sua eficácia psicoterápica; e, por fim, sua inserção nas equipes multiprofissionais. A trama formada pelo conjunto desses protagonismos configura o desafio do psicólogo, junto com os demais profissionais, para compreender e levar a cabo a integralidade da atenção, em exercício permanentede construção no hospital. Dois teóricos destacam-se na argumentação que problematiza a atenção integral no hospital - Canguilhem, para a discussão do conceito de saúde enquanto possibilidade de viver em conformidade com o meio e Winnicott, na perspectiva clínica possível nesse ambiente.

Il lavoro intende dimostrare che lo sviluppo giurisprudenziale del principio di equilibrio istituzionale, il rapporto tra detto principio e il principio di leale cooperazione, il riconoscimento istituzionale e giurisprudenziale dell’importanza della scelta della base giuridica nella tutela dell’equilibrio istituzionale hanno concorso a determinare la dinamicità dell’evoluzione dell’assetto interistituzionale della Comunità e dell’Unione. Focalizzata l’attenzione sulle nuove basi giuridiche introdotte dal Trattato di Lisbona, sono stati definiti gli assetti del nuovo equilibrio istituzionale analizzando, da un lato, il nuovo quadro istituzionale definito dal titolo III del Trattato sull’Unione europea e, in particolare la “costituzionalizzazione” del principio orizzontale di leale cooperazione. In conclusione, si rileva che con l´entrata in vigore del Trattato di Lisbona le dimensioni politica e giuridica dell’equilibrio istituzionale sono state interessate da due mutamenti di ampia portata. In primo luogo, il completamento del processo di revisione dei trattati apertosi con la dichiarazione di Laeken ha definito un nuovo quadro istituzionale, che si è riflesso in rinnovati meccanismi di funzionamento dell’architettura istituzionale. In secondo luogo, la risposta dell’Unione alla crisi economica e finanziaria ha messo al centro dell’agenda il suo bilancio, la programmazione pluriennale e l’Unione economica. Nel primo caso un’analisi dell’articolo 295 TFUE ha costituito la base di una riflessione sulle modalità di codificazione delle relazioni istituzionali attraverso accordi e sul rapporto tra questi ultimi e il titolo III TUE. Si è rilevata, in particolare, un’incongruenza tra gli obblighi di leale cooperazione orizzontale sanciti dall’articolo 13(2) TUE e gli strumenti finalizzati alla loro istituzionalizzazione. Nel secondo caso, invece, è stato evidenziato come il preminente ruolo del Consiglio europeo, al quale il Trattato di Lisbona ha riconosciuto lo status d'istituzione, abbia modificato gli equilibri, determinando un ritorno del ricorso all’integrazione differenziata rispetto a politiche disciplinate dai Trattati.

This brief paper draws upon part of the findings of a HEA Sponsored evaluation of work conducted in 8 Universities across UK aimed at addressing the attainment gap between BME and White students. Following a grounded theory approach, semi-structured interviews with staff at each of the institutions were analysed and three main themes identified: Organisational Sensitivities: Language: and, Ownership. This paper provides a brief discussion of the issues identifying two areas where positive change is needed in institutional practice. The conclusion highlights the complexities of the underlying issues impacting and shaping the Attainment Gap before reaffirming the need to identify and evaluate which interventions are most likely to be transferable across the Sector so as to address the issues and thus enhance the experiences of all students.
