Alguns autores entendem o declínio cognitivo como fator de risco para fragilidade. No entanto, outros estudos apresentam a fragilidade como fator de risco para declínio cognitivo. O presente estudo também entende a relação entre fragilidade e desempenho cognitivo neste sentido. Ainda são poucos os estudos que avaliaram esta associação na literatura internacional e não identificamos nenhum estudo conduzido no Brasil, ou mesmo na América Latina, que a tenha investigado. Este estudo tem como objetivo, investigar a associação entre a síndrome da fragilidade e desempenho cognitivo em idosos, e o efeito da escolaridade e da idade nesta associação. Para isto, analisaram-se dados seccionais da Fase 1 do Estudo da fragilidade em Idosos Brasileiros (Rede FIBRA - Fragilidade em Idosos Brasileiros), relativos à clientes de uma operadora particular de saúde, com 65 anos ou mais, residentes na zona norte da cidade do Rio de Janeiro. A população final de estudo foi de 737 idosos. O desempenho cognitivo foi avaliado através do Mini Exame do Estado Mental (MEEM). Foram considerados frágeis os indivíduos que apresentaram três ou mais das seguintes características: perda de peso não intencional (mais de 4,5Kg no último ano); sensação de exaustão auto-relatada; baixo nível de força de preensão palmar (sujeitos no primeiro quintil); baixo nível de atividade física (sujeitos no primeiro quintil do Minnesota) e lentificação da marcha (sujeitos no primeiro quintil). Também foram coletadas informações sobre características socioeconômicas e demográficas, apoio social, condições médicas e capacidade funcional. O desempenho cognitivo (MEEM) e a fragilidade foram analisados como variáveis dicotômicas. Avaliou-se o papel da idade e escolaridade como possíveis modificadoras de efeito na associação entre fragilidade e baixo desempenho cognitivo. A avaliação da associação entre fragilidade e desempenho cognitivo foi feita através de regressão logística multivariada. A variável idade se comportou como modificadora de efeito na associação entre fragilidade e desempenho cognitivo, x(5) = 806,97, p<0,0001. O mesmo não ocorreu com a variável escolaridade. Os idosos frágeis apresentaram uma maior prevalência de baixo desempenho cognitivo, quando comparados aos idosos não-frágeis ou pré-frágeis, nos três estratos estudados (65 a 74 anos;75 a 84 anos; >_85 anos), p<0,001. A associação entre fragilidade e baixo desempenho cognitivo foi encontrada somente em idosos com 75 anos ou mais, sendo OR bruto=2,68 (IC 95% 1,29 5,53) para idosos de 75 a 84 anos e OR bruto= 6,39 (IC 95% 1,82 - 22,42) para idosos de 85 anos ou mais. Após ajuste pelas condições de saúde, capacidade funcional e pelas variáveis socioeconômicas e demográficas, a associação entre fragilidade e baixo desempenho cognitivo se manteve nesses estratos, OR aj=2,78 (IC 95% 1,23 - 6,27) para 75 a 84 anos e OR aj=15,62 (IC 95% 2,20 110,99) para 85 anos ou mais. A síndrome da fragilidade está, portanto, associada ao baixo desempenho cognitivo em idosos. A idade revelou-se como modificadora de efeito nesta associação. Os idosos com idade mais avançada revelaram uma associação mais expressiva entre os dois fenômenos.

O turismo na natureza é uma das modalidades de turismo que mais tem crescido no Brasil, sendo visto como alternativa econômica para municípios de médio e pequeno porte que possuem Unidades de Conservação em seus territórios ou áreas com remanescentes florestais ou ainda áreas de interesse geológico, geomorfológico, concentração hídrica que possam atrair o turista. A preocupação quanto ao sustento da atividade turística desta categoria ocorre, quando o ambiente natural na qual se pretende instalar os empreendimentos turísticos não possui estudos pretéritos sobre o meio físico e nem instrumento legal de planejamento ambiental que apoie a tomada de decisão. Neste contexto, esta dissertação versa sobre o estudo da fragilidade turística do município de Duas Barras Rio de Janeiro com objetivos de balizar os resultados obtidos, com estudos pretéritos sobre potencialidades turísticas da região, assim como os empreendimentos turísticos já instalados e verificando se há conformidade entre o ambiente explorado e a atividade turística em curso. Como metodologia, a presente pesquisa utilizou técnicas de fragilidade ambiental desenvolvida de Ross (2010), a análise da fragilidade turística teve adaptações com base em Costa (2009), o modelo de indicadores turísticos de Barbosa (2009) e os dados sobre a potencialidade turística local foram extraídos de Goes (2007). Utilização de técnicas de geoprocessamento foram fundamentais para desenvolvimento de análises espaciais, geográficas e a visão integrada dos dados. Como resultados, foram gerados mapas temáticos do meio físico, socioeconomia e a avaliação das atuais instalações turísticas na região.

A síndrome de fragilidade pode ser definida como um estado de vulnerabilidade a agentes estressores, um resultado de declínios orgânicos observados em múltiplos sistemas, comprometendo a habilidade do indivíduo em manter a homeostase. O objetivo deste estudo transversal foi avaliar a prevalência desta condição através da Escala de Fragilidade proposta pelo Cardiovascular Health Study em uma população de idosos, clientes de uma operadora de saúde, que vivem na zona norte da cidade do Rio de Janeiro, além de observar o comportamento deste instrumento em uma amostra brasileira. 754 indivíduos foram avaliados quanto aos cinco critérios da escala além de variáveis sociodemográficas, capacidade funcional, quedas, perfil cognitivo e comorbidades relatadas. Foram considerados Frágeis aqueles que apresentaram três ou mais dos seguintes critérios: a) lentificação da velocidade da marcha; b) reduzida força de preensão palmar; c) sensação de exaustão; d) baixa atividade física; e) perda de peso. Os resultados apontam que, dentre os avaliados, 9,2% eram Frágeis. Estes eram mais idosos, com pior status socioeconômico, com pior desempenho cognitivo e maior comprometimento funcional (p< 0,05). Entre os 9,2% de Frágeis, 87% apresentaram alteração da velocidade da marcha, 79,7% da força de preensão palmar, 66,8% baixa atividade física, 52,2% relato de sensação de exaustão e 36,2% relato de perda de peso. A distribuição de suas frequências quando comparado ao estudo original foi bastante semelhante. Vários estudos partem do postulado que a fragilidade pode ser identificada através de medidas clínicas. Através de sua identificação precoce, é possível reconhecer uma entidade potencialmente reversível, reduzindo morbidade e mortalidade na população idosa, no entanto é fundamental um estudo acurado sobre como esta entidade será mensurada e quais serão os critérios adotados para defini-la. Maiores estudos são necessários para realizar, no Brasil, uma análise mais aprofundada desta pertinente questão.

O presente trabalho é uma aplicação da estrutura analítica de Minsky às relações financeiras entre países, no atual contexto de abertura financeira das economias emergentes. Constrói-se assim uma tipologia das posturas financeiras para países e do modelo dos dois sistemas de preços com financiamento externo. A separação da dinâmica dos diferentes ativos enseja a ocorrência de ciclos de ativos e/ou de investimentos, dados pelo influxo de capitais nas economias em desenvolvimento e se analisa a possibilidade de crises endógenas, a partir da estratégia adotada pelos países, de endividamento externo com uma oferta de crédito que tende a se tornar crescentemente menos elástica. O instrumental teórico é aplicado à experiência do Plano Real e procura-se averiguar quais foram as condições intrínsecas da política econômica e da conjuntura externa para a crise cambial ocorrida em janeiro de 1999.

Esta dissertação tem como objetivo geral analisar as conseqüências da crise de 2008 na economia russa e como objetivos específicos: apontar as fragilidades da economia russa e a sua consequente vulnerabilidade a choques econômicos externos, além de analisar os efeitos da dependência do petróleo na geração de receitas para o governo e para o PIB do país no contexto da crise. Dessa forma, o trabalho será dividido em cinco partes: Introdução, Capítulo 2 - onde se discutirá a Crise de 2008 e os seus efeitos na economia global, Capítulo 3 - onde se abordará a crise e a Rússia, Capítulo 4 - onde será analisada a dependência russa, através de um teste estatístico de regressão linear simples, em relação às exportações de gás natural e mais profundamente às de petróleo e Conclusão.

Technetium-99m (99mTc) has been used to obtain several radiobiocomplexes utilized to aid in the diagnosis of diseases. Blood constituents, as red blood cells (RBC) and plasma proteins, have been labeled with 99mTc. Natural and synthetic drugs can alter the labeling of these constituents. The aim of this work was to investigate the possibility of a Buzhong YiQi Wan extract to alter (i) the labeling of blood constituents with 99mTc, (ii) the RBC morphology, and (iii) osmotic fragility of RBC withdrawn from Wistar rats. The data showed that the BYQW extract (i) could affect labeling of blood constituintes with 99mTc, (ii) could affect the membrane integrity decreasing the osmotic resistance and (iii) could not alter the shape of RBC. Probably, these findings would be associated with properties of the substances present in the aqueous extract of BYQW. This study aims to analyze social representations of elders to their fragile situation at home, with the presence of one or more characteristics, as defined by the Brazilian Ministry of Health. It is a descriptive and qualitative study, based on methodological -principles of the Theory of Social Representations. Setting was the homes of elderly residents in the area ascribed to a Family Health Unit (FHU) in the city of Natal. A total of 10 elderly subjects, whose choice was intentional and according to the need for USF home visit in a period of time, considering the saturation process of the information. As collection procedures were used the semi-structured interview and participant observation in accordance with the ethical rules of Resolution No. 196/96, with the assent of the Ethics and Research UFRN. To analyze the results, it was used the thematic content analysis in the aspect of preparation of representations, focusing on the totality of the discourse of the subjects. The results indicate that most study participants felt difficult to give meaning to the terms weakness and to be weak, although many present one or more aspects of the syndrome of frailty. From the content analysis of participants speeches in this study, we achieved the following categories: fragility as illness and disease as aging, aging and frailty as causes of changes and difficulties in daily life, the presence of family life in the fragile elderly, fragility as weakness and the risk for falls, the perception of being weak like a different person in addition to the absence of fragility in elderly life. Thus, through the processes of anchoring and objectification, the "fragile being" became familiar and concrete, showing that the meaning of weakness, besides the scientific definition found in the reified universes, can be reinterpreted and built within the consensus universes. About the care received by the staff of Family Health, from the viewpoint of older people there seems to be an understanding about the role of professional nurses; on the other hand, older people often mention the role of the Community Health Agent

The objective of this paper is to analyze the work of the American writer Nathaniel Hawthorne (1804 - 1864), The Scarlet Letter (1850), in the light of the contributions of Literature and History of the American people in the context of New England. Accordingly, we highlight aspects that justify the inclusion of the work as a historical novel, especially based on the reading of The Historical Novel (1936-37) written by Georg Lukács. The diversity of voices and social interrelationships that come out of the main characters of the plot of the novel, as well as their contextual buildings, constituted as important traces to understanding the novel as being of historical value. During our study, we found out that it is in the plots of the novels that the characters reflect, at the same time, the specific conditions of their singularities, the general trends of the historical process and the social conditions from which they arise. We also could see that it is in their singularities that lie special tendencies of human beings. The frailty in the elderly is characterized by decreased physiological reserves, and is associated with increased risk of disability and high vulnerability to morbidity and mortality. This study is part of a multicentric project on Frailty in Elderly Brazilians (REDE FIBRA). Aims: to investigate characteristics, prevalence and associated factors related to frailty. Metodology: We interviewed 391 elderly patients aged 65 years, selected randomly. Data collection was performed using a multidimensional questionnaire containing information about socio-demographic and clinical variables. To characterize the frail elderly, was considered the phenotype proposed by Fried. Data were analyzed using descriptive statistics, bivariate analysis (χ ²) and binary logistic regression. Results: The prevalence of frailty was 17.1%. In the final model of multivariate analysis, was obtained as factors associated with frailty: advanced chronological age (p <0.001), have comorbidity (p <0.035), show dependence on performing basic activities of daily living (p <0.010) and instrumental (p <0.003) and have poor perception of health status (p <0030). Conclusions: The factors associated with frailty suggest a predictive model helping to understand the syndrome, guiding actions that minimize adverse effects on the aging process

Gait speed has been described as a predictive indicator of important adverse outcomes in older populations. Among the criteria to evaluate frailty, gait speed has been identified as the most reliable predictor of fragility, practical and low cost. Objective: This study assesses the discriminating capability of gait speed in determining the presence of fragility in the elderly community in northeast of Brazil. Method: We performed an observational analytic study with a transversal character with a sample of 391 community-living elders, aged 65 years or older, of both sexes, in the city of Santa Cruz-RN. Participants were interviewed using a multidimensional questionnaire to obtain sociodemographic information, physical-related and mental health-related information. The unintentional weight loss, muscle weakness, self-reported exhaustion, slow gait and low-physical activity were considered to evaluate the frailty syndrome. Gait velocity was measured as the time taken to walk the middle 4,6 meters of 8,6 meters (excluding 2 meters to warm-up phase and 2 meters to deceleration phase).We calculate the sensitivity and specificity of gait speed test in different cutoff points for the test run time, from which ROC curve was constructed as a measure of test predictive value to identify frail elders. The prevalence of frailty in Santa Cruz-RN was 17.1%. The gait speed test accuracy was 71%when speed is below 0,91m/s. Among women, the gait speed test accuracy was 80%(gait speed below 0.77m/s) and among men, the test accuracy was 86% (gait spend below 0,82%) (p<0,0001).Conclusion: our findings have clinical relevance when we consider that the detection of frailty presence by the gait speed test can be observed in elderly men and women by a simple, cheap and efficient exam

Introduction: The SPPB provides information about physical function and is a predictor of adverse events in the elderly. Frailty is a multidimensional syndrome that increases susceptibility to diseases and disability. However it may be possible to prevent or postpone frailty if is identified early. Our objective is to analyze SPPB s ability in screening for frailty a community-dwelling young elderly from cities with distinct socioeconomic conditions. Methods: Data were originated from community dwelling adults (65-74 years old) in Canada (Saint Bruno; n = 60) and Brazil (Santa Cruz; n = 64). SPPB was used to assess physical performance. Frailty was defined as the presence of ≥ 3 of these criteria: weight loss, exhaustion, weakness, mobility limitation and low physical activity. One point was given for each criterion met, totalizing a frailty score ranged from 0 to 5. The Linear Regression and Receiver Operating Characteristics analyses were performed to evaluate the SPPB s screening ability. Results: Mean age was 69.48, 10.0% of the Saint Bruno s sample and 28.1% of Santa Cruz s were frail (p = 0.001), the SPPB score means were 9.6 and 8.5 respectively (p = 0.01). SPPB correlated with the frailty score (R2 = 0.33), with better results for Saint Bruno. A cutoff of 9 in SPPB had good sensitivity and specificity in discriminating frail from non frail in Saint Bruno (AUC = 0.81) but showed fair results in Santa Cruz (AUC = 0.61). Conclusion: The SPPB has moderate ability in predicting frailty among older adult s population, and is an useful test to identify people with good functionality and low frailty when SPPB scores are ≥9

Introduction: The aging process causes quantitative and qualitative changes in sleeping. Such changes affects more than half of the adults above 65 years old, that live in the community and 70% of the institutionalized, a great negative impact in their quality of life. One of the pathological displays of aging, that share some characteristics with sleeping disorders and predict similar results, is the Frailty Syndrome, that characterize the most weakened and vulnerable elderly. The way sleeping disorders play a role in the frailty pathogeneses remains uncertain. Objective: Evaluate the relation between the sleeping and the frailty syndrome on institutionalized elderly. Methodology: A transversal study was performed with 69 elderly in institutions in the city of João Pessoa PB. Were used the Pittsburgh Sleeping Quality Index and actigraphy to subjective and objective variables, respectively, and questionnaires and specific tests to frailty phenotype variant (Fried Frailty Criteria). In the statistic analysis were used the Pearson correlation test, Chi Square and One-way ANOVA test, with Tukey-Krammer posttest. Subsequently, a Simple Linear Regression model was built. On every statistical analysis were considered a confidence interval of 95% and a p < 0,05. Results: The sample was characterized by the prevalence of the frail (49,3%), women (62,3%), single (50,7%) and 77,52 (±7,82).The frail elderly obtained the worst sleeping quality 10,37 (±4,31) (f = 4,15, p = 0,02), when compared with the non-frail. The sleep latency influenced more the frailty (R2 = 0,13, β standard = 1,76, β = 0,41, p = 0,001). Weren t found differences between the standard resting-activity variable and the frailty phenotype categories. Conclusion: Sleeping alterations, including bad sleeping quality, prolonged sleep latency, low sleep efficiency and day drowsiness, influenced the frailty in institutionalized elderly

Introduction: The Frailty Syndrome is characterized by the decrease of energy reserve and the reduced resistance to stressors. Studies indicate that the neuroendocrine markers can be related to the appearance of this syndrome. The main endocrine answer to stress is the increase of cortisol levels. Objective: To analyze the correlation between the frailty syndrome the salivary cortisol in elderly residing in nursing homes. Method: A traversal study was accomplished, in João Pessoa city, PB, with a sample composed by 69 institutionalized elderly. The collected data refer to the frailty phenotype (weight loss, exhaustion, slowness, weakness, and lower level of physical activity) and to salivary cortisol parameters (first measure - 6-7h; second measure - 11-12h; third measure - 16-17h). In the statistical analysis the Pearson s correlation test was used, Chi square Test and Anova and Simple Linear Regression analyses. Results: The sample was composed by 37.7% of men and 62.3% of women, with age average of 77.52 (±7.82). There was a percentile of 45.8% frail elderly. The frail elderly obtained higher cortisol values in the third measure (p=0.04) and the frailty load was significantly associated to the first measure (r=0.25, p=0.04). The simple linear regression analysis presented a determination rate (R2=0.05) between frailty load and first cortisol measure. Conclusion: The largest cortisol values in the morning and before sleeping among the frail elderly supply indications that can have a relationship of cortisol increase levels and the frailty presence in elderly from nursing homes.

Introduction: The frailty in the elderly is the result of a complex interplay between many social and clinical factors involved in its genesis. Although there is an understanding of its association with increased risk of adverse clinical outcomes, still it is unclear whether this syndrome can be aggravated due to lack of social support. So, the objective of this study was to analyze the association between social support and frailty syndrome in elderly community residents. Materials and methods: Observational analytical cross-sectional study, with a sample of 300 elderly living in the city of Natal-RN. Were collected informations about socialdemographic, economics and physical health data. The Social support was assessed by the status of cohabitation; marital status; contact frequency and diversity rate; received and given attendance frequency rate; and Map Minimum Relations of the Elderly. The frailty was assessed using the following criteria: unintentional weight loss, weakness, low physical activity, exhaustion and Slowness. To observe any possible associations, we performed the Pearson chi-square test, the Student t test and multivariate analysis using binary logistic regression, adopting a significance level of 5%. Results: It was observed that there were no associations of frailty with the social support variables, except for housekeeping mode (p = 0.046) of the MMRI. For the sociodemographic, economic, physical health and social support data, only age (p <0.001), sedentary lifestyle (p = 0.002) and poor perceived health (p = 0.001) were the ones that remained in the logistic regression model, with strong association for the presence of frailty. Conclusion: Among the variables related to social support, only to help with housekeeping was significantly associated with frailty. However, more studies need to be developed to characterize the social vulnerability, as well as health services need to recognize the importance of social support as an integral part of care for the elderly
