Pre-eclampsia (PE) is a complication of human pregnancy characterized by hypertension and proteinuria after 20 weeks of gestation. Its incidence varies from 5% to 7% of pregnancies and is a major cause of morbidity and maternal and fetal mortality. This is a multisystemic disease, with focus on vascular dysfunction and is closely related to the exacerbated activation of the immune system. In addition to increased activation of monocytes and granulocytes, there is an elevated production of proinflammatory cytokines in pregnant women with PE. The nuclear transcription factor-kB (NF-kB) is present in the cells of the immune system and is responsible for transcription of genes related to inflammation. Whereas the PE is associated with intense inflammatory response, the use of substances modulating the activity of NF-kB factor could be useful in alleviating the inflammation present in these patients. Silibinin is the main component of silymarin, a polyphenolic extract obtained from fruits and seeds of Sylibum marianum with potent hepatoprotective, anti-inflammatory and anti-fibrotic activities. The silibinin mechanism of action includes the ability to inhibit NF-kB activation and, consequently, its migration to the nucleus. The objective of this study was to assess whether silibinin modulates the activity of NF-kB and the production of inflammatory cytokines in mononuclear cells of patients with PE. We evaluated 34 pregnant women with PE, 20 normotensive pregnant women (GN) and 15 non-pregnant women (NG). Peripheral blood mononuclear cells (PBMC) were obtained from those groups of women and cultured in the presence or absence of silibinin (5 uM or 50 uM) and stimulated or not with lipopolysaccharide (LPS) for 18 h to obtain supernatant for determination of tumor necrosis factor-alpha (TNF-α) and interleukin-1 (IL-1β) by enzyme immunoassay (ELISA). The pineal gland, a circumventricular organ, plays an integrative role in defense responses. The injury-induced suppression of the pineal gland hormone, melatonin, which is triggered by darkness, allows the mounting of innate immune responses. We have previously shown that cultured pineal glands, which express toll-like receptor 4 (TLR4) and tumor necrosis factor receptor 1 (TNFR1), produce TNF when challenged with lipopolysaccharide (LPS). Here our aim was to evaluate which cells present in the pineal gland, astrocytes, microglia or pinealocytes produced TNF, in order to understand the interaction between pineal activity, melatonin production and immune function. Cultured pineal glands or pinealocytes were stimulated with LPS. TNF content was measured using an enzyme-linked immunosorbent assay. TLR4 and TNFR1 expression were analyzed by confocal microscopy. Microglial morphology was analyzed by immunohistochemistry. In the present study, we show that although the main cell types of the pineal gland (pinealocytes, astrocytes and microglia) express TLR4, the production of TNF induced by LPS is mediated by microglia. This effect is due to activation of the nuclear factor kappa B (NF-kB) pathway. In addition, we observed that LPS activates microglia and modulates the expression of TNFR1 in pinealocytes. As TNF has been shown to amplify and prolong inflammatory responses, its production by pineal microglia suggests a glia-pinealocyte network that regulates melatonin output. The current study demonstrates the molecular and cellular basis for understanding how melatonin synthesis is regulated during an innate immune response, thus our results reinforce the role of the pineal gland as sensor of immune status.

O Chá Verde, derivado das folhas da planta Camellia sinensis, rico em flavonóides, cuja maior concentração é de Epigalocatequina gallato (EGCG), possui efeito termogênico, além de promover a oxidação da gordura corporal, tendo potencial interesse para o tratamento da obesidade, que atinge prevalência alarmante em diversos países no mundo. O objetivo deste estudo foi a avaliação de parâmetros bioquímicos e investigação da função endotelial em mulheres com Índice de Massa Corporal (IMC) entre 30kg/m2 e 40kg/m2, na faixa de 30 e 50 anos, antes e após 03 meses de consumo de chá verde (600mL/dia, equivalente a 114,42mg de EGCG). Todas as 60 pacientes voluntárias foram submetidas à análise das medidas antropométricas (Peso, Altura, Índice de Massa Corporal, Circunferência de Cintura, Circunferência de Quadril, Relação Cintura-Quadril, Pressão Arterial, à análise da bioquímica de rotina (Glicemia e Insulina de jejum, Triglicerídeos, Colesterol Total, HDL-Colesterol, LDL-Colesterol, Teste Oral de Tolerância à Glicose, Hemograma Completo, Proteína C-Reativa), à análise da bioquímica específica para estresse oxidativo e inflamação (Interleucinas 1 e 6, Fator de Necrose Tumoral Alfa, LDL-Oxidado, VCAM Vascular Cell Adhesion Molecule, ICAM Intercellular Adhesion Molecule, e E-Selectina) e à Pletismografia de Oclusão Venosa (variação de fluxo médio máximo durante a Hiperemia Reativa/Fluxo Basal 1 (VQ Hiper) e fluxo após administração de 0,4mg de Nitroglicerina Sublingual/Fluxo Basal 2 (VQ Nitro)). Após os 3 meses (3M) de tratamento houve redução no peso corporal (86,35[83,00-94,25] vs 3M = 86,00[81,50-92,00] Kg, P < 0,05); no IMC (34,02[32,05-35,62] vs 3M = 33,13[32,28-35,05] kg/m2, P < 0,05); na circunferência de cintura (99[93-107] vs 3M = 98[91-105]cm, P < 0,001); na circunferência de quadril (115[110-119] vs 3M = 114[110-117] cm, P < 0,001); na relação cintura-quadril (0,89[0,84-0,93] vs 3M = 0,88[0,83-0,93], P < 0,001); e, na pressão arterial diastólica (75[73-82] vs 3M = 69[67-72] mmHg, P < 0,001); e, melhora significativa no fluxo sanguíneo da VQ Hiper (4,57[3,54-5,01] vs 3M = 5,83[4,46-6,56], P < 0,001); e da VQ Nitro (1,26[1,13-1,38] vs 3M = 1,41[1,25-1,50], P < 0,001). Com o uso do chá verde, 600mL/dia, contendo 114,42mg de EGCG, durante 3 meses observamos a redução de 3% no IMC e a redução da circunferência de cintura e de circunferência de quadril em 1cm; a não modificação do padrão bioquímico, incluindo os marcadores de inflamação e de estresse oxidativo; e, o aumento das vasodilatações endotélio-dependente e endotélio-independente, visualizadas por Pletismografia de Oclusão Venosa Não-Invasiva.

Intestinal ischemia-reperfusion (I/R) injury may cause acute systemic and lung inflammation. Here, we revisited the role of TNF-alpha in an intestinal I/R model in mice, showing that this cytokine is not required for the local and remote inflammatory response upon intestinal I/R injury using neutralizing TNF-alpha antibodies and TNF ligand-deficient mice. We demonstrate increased neutrophil recruitment in the lung as assessed by myeloperoxidase activity and augmented IL-6, granulocyte colony-stimulating factor, and KC levels, whereas TNF-alpha levels in serum were not increased and only minimally elevated in intestine and lung upon intestinal I/R injury. Importantly, TNF-alpha antibody neutralization neither diminished neutrophil recruitment nor any of the cytokines and chemokines evaluated. In addition, the inflammatory response was not abrogated in TNF and TNF receptors 1 and 2-deficient mice. However, in view of the damage on the intestinal barrier upon intestinal I/R with systemic bacterial translocation, we asked whether Toll-like receptor (TLR) activation is driving the inflammatory response. In fact, the inflammatory lung response is dramatically reduced in TLR2/4-deficient mice, confirming an important role of TLR receptor signaling causing the inflammatory lung response. In conclusion, endogenous TNF-alpha is not or minimally elevated and plays no role as a mediator for the inflammatory response upon ischemic tissue injury. By contrast, TLR2/4 signaling induces an orchestrated cytokine/chemokine response leading to local and remote pulmonary inflammation, and therefore disruption of TLR signaling may represent an alternative therapeutic target.

Tumor necrosis factor (TNF) plays a role in the pathogenesis of chronic hepatitis B (CHB) and chronic hepatitis C (CHC). The difference in the cytokine responses between hepatitis B virus (HBV) and hepatitis C virus (HCV) infections may have implications in the pathogenesis of these diseases. We performed a comparative study to examine the possible differences in the TNF-TNF receptor (TNFR) response between CHB and CHC. We studied the cytokine levels of 38 patients with CHB, 40 patients with CHC and 9 patients with dual hepatitis B and C, and compared them with the baseline levels of 12 healthy controls. The plasma levels of TNF-, interferon-, interleukin (IL)-2, IL-4, IL-10 and soluble TNFR-1 and 2 (sTNFR-1 and 2) were quantified by enzyme-linked immunosorbent assays. The expression of TNFR-1 and 2 in liver tissues was examined in 30 cases of CHB and 15 cases of CHC by semiquantitative reverse transcription polymerase chain reaction. The results showed that sTNFR-1 levels correlated with liver inflammation in all patients, whereas this correlation was not found with sTNFR-2 or other cytokines. Liver inflammation indicators were higher in HCV RNA+ than in HCV RNA– CHC. Most significantly, sTNFR-1 levels correlated with liver inflammation in CHB, but not in CHC. However, the expression of TNFR-1 and 2 in liver was similar between CHB and CHC. These findings suggest that the TNFR signal transduction pathway is modulated differently in HBV and HCV infection.

Heparin, a sulfated polysaccharide, was the first compound used as an anticoagulant and antithrombotic agent. Due to their structural characteristics, also has great potential anti-inflammatory, though such use is limited in inflammation because of their marked effects on coagulation. The occurrence of heparin-like compounds that exhibit anticoagulant activity decreased in aquatic invertebrates, such as crab Goniopsis cruentata, sparked interest for the study of such compounds as anti-inflammatory drugs. Therefore, the objective of this study was to evaluate the potential modulator of heparin-like compound extracted from Goniopsis cruentata in inflammatory events, coagulation, and to evaluate some aspects of its structure. The heparin-type compound had a high degree of N-sulphation in its structure, being able to reduce leukocyte migration into the peritoneal cavity at lower doses compared to heparin and diclofenac sodium (anti-inflammatory commercial). Furthermore, it was also able to inhibit the production of nitric oxide and tumor necrosis factor alpha by activated macrophages, inhibited the activation of the enzyme neutrophil elastase in low concentrations and showed a lower anticoagulant effect in high doses as compared to porcine mucosal heparin. Because of these observations, the compound extracted from crab Goniopsis cruentata can be used as a structural model for future anti-inflammatory agents

Despite advances in antibiotic therapy, bacterial meningitis (BM) remains with high mortality and morbidity rates in worldwide. One important mechanism associated to sequels during disease is the intense inflammatory response which promotes an oxidative burst and release of reactive oxygen species, consequently leading to cell death. Activation of DNA repair enzymes during oxidative stress has been demonstrated in several neurological disorders. APE1/Ref-1 is a multifunctional protein involved in DNA repair and plays a redox function on transcription factors such as NFkB and AP-1.The aim of this study was assess the role of APE1/Ref-1 on inflammatory response and the possibility of its modulation to reduce the sequels of the disease. Firstly it was performed an assay to measure cytokine in cerebrospinal fluid of patients with BM due to Streptococcus pneumoniae and Neisseriae meningitides. Further, a cellular model of inflammation was used to observe the effect of the inhibition of the endonuclease and redox activity of APE1/Ref-1 on cytokine levels. Additionally, APE1/Ref-1 expression in cortex and hippocampus of rat with MB after vitamin B6 treatment was evaluated. Altogether, results showed a similar profile of cytokines in the cerebrospinal fluid of patients from both pathogens, although IFNy showed higher expression in patients with BM caused by S. pneumoniae. On the other hand, inhibitors of APE1/Ref-1 reduced cytokine levels, mainly TNF-α. Reduction of oxidative stress markers was also observed after introduction of inhibitors in the LPS-stimulated cell. In the animal model, BM increased the expression of the protein APE1/Ref-1, while vitamin B6 promoted reduction. Thereby, this data rise important factors to be considered in pathogenesis of BM, e.g., IFNy can be used as prognostic factor during corticosteroid therapy, APE1/Ref-1 can be an important target to modulate the level of inflammation and VIII oxidative stress, and vitamin B6 seems modulates several proteins related to cell death. So, this study highlights a new understanding on the role of APE1/Ref-1 on the inflammation and the oxidative stress during inflammation condition

A obesidade é atualmente um problema de saúde pública que provoca sérias conseqüências sociais, físicas e psicológicas. A etiologia da obesidade não é de fácil identificação, uma vez que a mesma é caracterizada como doença multifatorial de complexa interação entre fatores comportamentais, culturais, genéticos, fisiológicos e psicológicos. Recentes avanços na área de endocrinologia e metabolismo mostram que, diferentemente do que se acreditava há alguns anos, o adipócito sintetiza e libera diversas substâncias, não sendo apenas uma célula armazenadora de energia. Entre as substâncias liberadas pelo adipócito incluem-se a adiponectina, o fator de necrose tumoral-alfa, a interleucina-6 e a leptina. Especificamente, a leptina desempenha importante papel no controle da ingestão alimentar e no controle do peso corporal em mamíferos. Além disso, o hormônio ghrelina, recentemente descoberto, também parece influenciar o metabolismo energético e a obesidade. As alterações que o exercício físico provoca na fisiologia endócrino-metabólica podem contribuir sobremaneira para a prática clínica. Assim, essa revisão abordará os conhecimentos mais recentes sobre a leptina, a ghrelina e o papel dos diferentes tipos de exercício físico sobre estes hormônios. Os trabalhos mostram que a relação entre o exercício físico e a concentração plasmática desses peptídeos ainda não está clara. As razões para isso poderiam ser devidas aos diferentes protocolos de treinamento físico empregados nos estudos. Além disso, diferenças genéticas também podem explicar as discrepâncias entre os resultados obtidos em seres humanos, pois a existência de polimorfismo em alguns genes pode acarretar respostas celulares diferentes frente ao exercício físico.

Este trabalho apresenta os resultados da pesquisa intitulada Níveis de TNF–α e seu Polimorfismo Genético em Pacientes HIV Positivos, Portadores de Lipodistrofia e em Uso de Terapia Anti–retroviral. Objetivou–se, por meio da pesquisa ora apresentada, estudar os níveis séricos de TNF–α e seu Polimorfismo Genético em Pacientes HIV Positivos, Portadores de Lipodistrofia e em Uso de Terapia Anti–retroviral. Justifica esse objetivo a necessidade de melhor conhecer um dos fatores envolvidos no desenvolvimento de uma síndrome clínica conhecida como lipodistrofia, considerada um efeito adverso da terapia anti–retroviral, a fim de que esse conhecimento oportunize a exeqüidade de um regime terapêutico seguro, propiciando, desse modo, uma possível melhoria na qualidade de vida dos pacientes. A metodologia empreendida para a determinação quantitativa dos níveis séricos de TNF–α utilizou-se do método ELISA, enquanto, tendo em vista a pesquisa da ocorrência do polimorfismo, utilizou–se da reação de PCR para a extração do DNA, seguida da digestão pela enzima de restrição Ncol. Participaram da presente pesquisa 40 pacientes, dos quais 26 do sexo masculino e 14 do sexo feminino. Desses pacientes, procedeu–se a dosagem dos níveis de TNF–α, bem como pesquisa da presença ou ausência do polimorfismo genético (–308A).

OBJETIVO: Avaliar o efeito do resveratrol sobre a via de sinalização da insulina e melhora do quadro inflamatório no miocárdio de ratos Wistar obesos induzidos por dieta.MÉTODOS: Ratos Wistar foram divididos em grupos: controle (dieta padrão para roedores), obeso (dieta hiperlipídica) e obeso suplementado com resveratrol (20 mg/kg/dia), por 8 semanas (n=10). Ao final do período experimental, realizou-se o teste de tolerância à insulina, nos tempos 0 (sem insulina), 5, 10, 15, 20, 25 e 30 minutos após injeção intraperitoneal de insulina (2 U/kg). O peso corporal e o tecido adiposo epididimal foram mensurados. Fragmentos do miocárdio foram extraídos para análises da via da insulina e moléculas pró-inflamatórias através de Western blot.RESULTADOS: Os resultados indicam que a intervenção com resveratrol aumenta a constante de decaimento da glicose, fosforilação do receptor de insulina, substrato do receptor de insulina e da proteína quinase B. A suplementação de resveratrol também reduziu os níveis proteicos do fator de necrose tumoral alfa e de moléculas envolvidas com a transdução do sinal pró-inflamatório (quinase indutora do kappa B e fator nuclear kappa B). Os resultados ainda sugerem que a melhora na sensibilidade à insulina e a redução das moléculas pró-inflamatórias ocorreram independentemente da perda de peso corporal e da redução do tecido adiposo epididimal.CONCLUSÃO: A suplementação de resveratrol aumenta a sensibilidade à insulina, o que está relacionado à redução de fatores inflamatórios no miocárdio.
