Desde a década de 1970 não se notificavam casos autóctones de doença de Chagas aguda em São Paulo. em março de 2006 a Vigilância Epidemiológica registrou óbito por doença de Chagas aguda, em Itaporanga, de paciente de seis anos de idade. Exame histopatológico post mortem realizado no Hospital das Clínicas da Faculdade de Medicina de Botucatu confirmou o diagnóstico. Consultamos prontuários de hospitais e entrevistamos profissionais de saúde envolvidos além de familiares do paciente. Descrevemos medidas adotadas in loco para identificar a via de transmissão, reservatórios e vetores. Discutimos as possíveis fontes de infecção. Na região não foram identificados outros casos humanos, vetores ou reservatórios vertebrados infectados por Trypanosoma cruzi. Salientamos a importância de manter a vigilância, mesmo em áreas onde a transmissão de doença de Chagas está interrompida e naquelas ainda infestadas por triatomíneos. Deve-se admitir a hipótese diagnóstica de doença de Chagas quando observados: edema palpebral (uni ou bilateral), insuficiência cardíaca, miocardite, pericardite, anasarca, quadros similares aos de síndrome nefrótica ou glomerulonefrite sem causas outras aparentes, em pacientes com dados epidemiológicos positivos. Encontro, mesmo em raras ocasiões, de triatomíneos na região ou ainda contato com alimento contaminável com formas infectantes de T. cruzi.

Objective: To determine the prevalence and severity of caries in 3-12-year-old children in three districts of the city of Araraquara, Design: An epidemiological survey was carried out by trained and calibrated examiners in 1995 and 1996, using the WHO diagnostic criteria. Setting: Two districts, Araraquara and Vila Xavier had been fluoridated since 1963 and one, Gavião Peixoto, since 1994. Subjects: The study included 1,191 children from Araraquara, 653 from Vila Xavier and 652 from Gavião Peixoto. Outcome measures: Prevalence of caries, dmft, dmfs, DMFT and DMFS indices. Results: Results showed moderate caries experience in all three districts. Differences between districts in relation to fluoridation history were particularly obvious in primary teeth. In 3-4-year-old children, one third of those in Araraquara and Vila Xavier had some caries experience compared to 58% in Gavião Peixoto. In permanent teeth, 20% or less of the mean DMFT was made up of untreated decay in Araraquara and Vila Xavier whereas in Gaviao Peixoto it made up between 50 and 57% of values in 7-12-year-old children. Conclusions: The prevalence and severity of caries was lower in dentitions of children from the districts fluoridated since 1963. Improvements are likely in the future in Gaviao Peixoto as the benefit of fluoridation continues but additional means of promoting oral health are needed in all three districts.

Objectives: The objectives of this study were to assess the fluoride concentration in the public water supply and the prevalence of dental fluorosis in schoolchildren between 7 and 15 years old, living in a peripheral district of the municipality of Bauru. Material and Methods: For this, fifty two water samples were collected on three different days of one week. These samples were analyzed for fluoride by means of the ion-sensitive electrode method (Orion 9609) coupled to a potentiometer (Procyon, model 720). In this method, 1.0 mL of TISAB II (Orion) was added to 1.0 mL of the sample. For the epidemiological survey of fluorosis, 52 schoolchildren of both genders, aged between 7 and 15 were assessed, with prior authorization from their caretakers. Only one person examined the children, after supervised toothbrushing and drying with cotton wool rolls. The TF index was used. Results: The fluoride concentrations in the water samples ranged from 0.62 to 1.20 mg/L, with a mean of 0.9 mg/L. The prevalence of dental fluorosis was 33%, with severity ranging from TF1 to TF4 (Kappa of 0.73 and concordance of 83.33%). Conclusions: The results from the analysis of water samples indicated a fluoride concentration greater than recommended for Bauru. The fluorosis levels found were higher than expected for a peripheral district, in which water is one of the few sources of fluoride.

Background: Leptospirosis is a zoonosis which is spread through contamined running water. This contaminations is seriously affected by the flooding which occurs in the area surrounding the Aricanduva river. The transmission of the disease results mainly from the contact of water with soil contaminated by the urine of infected animals. We aimed to conduct an epidemiological survey on Leptospirosis cases in Sao Paulo East Zone area. Method. The analysis conducted in this study was based on data collected from the health authorities of that region close the Aricanduva river between 2007 and 2008 years, which give the rates of confirmed cases, mortality and death from human Leptospirosis. Other information concerned with the relationships among rainfall index, points of flooding and incidence of Leptospirosis. Results: We observed a direct and important water contamination. Records of flooding points and dates of the reported cases in the region showed a direct relationship from which the period of higher rainfall also recorded an increase in cases. The annual record of the city and the region and rainfall regions also presented correlation. Conclusion: The association between the indices of flooding and Leptospirosis cases indicates that preventive measures are necessary to avoid exposing the community. © 2013 Miyazato et al.; licensee BioMed Central Ltd.

Este estudo avalia a contaminação mercurial em comunidades de pescadores em quatro localidades nas margens do rio Tapajós: Rainha, Barreiras, São Luís do Tapajós e Paraná-Mirim. Análises toxocológicas das amostras de cabelo foram realizadas por espectofotometria de absorção atômica. Os níveis de mercúrio total em amostras de cabelo variaram entre 2,9µg/g e 71,5µg/g. Os valores mais baixos foram encontrados na comunidade de Paraná-Mirim. Os mais elevados, em São Luís do Tapajós e Barreiras, cerca de seis a sete vezes superiores ao valor estabelecido. As diferenças entre as concentrações médias de mercúrio total nas amostras, coletadas em populações ribeirinhas, a montante e a jusante do rio Tapajós em Itaituba, não apresentaram significância estatística (p > 0,05). Conclui-se que a exposição humana ao mercúrio por ingestão de peixes contaminados constitui risco potencial para o aparecimento de sintomas e sinais da doença de Minamata, o que recomenda a manutenção de um programa de vigilância epidemiológica.

ABSTRACT: Human T-lymphotropic virus tipe 1 is recognized as the etiologic agent of tropical spastic paraparesis/HTLV-1 associated myelopathy (TSP/HAM). A very similar clinical disease has been increasingly associated to HTLV-2, whose pathogenicity still requires further assessments. This transversal, retrospective epidemiological survey aimed to determine the prevalence of HTLV among individuals with neurological disturbances and further evaluate cases of inconclusive serology using molecular biology methods. The present study involved patients inhabitants of Pará State and/or admitted at health institutions of the and who were referred to the Virology Section of Instituto Evandro Chagas (IEC) by local doctors between January of 1996 and December 2005, to search for the presence of HTLV-1/2 serum antibodies. Of these patients 353 were selected, with age between 9 months and 79 years, who presented at least one signal or symptom of the Marsh’s Complex (1996), as well as had HTLV-1/2 positive serology at screening and confirmatory ELISA. The overall prevalence of HTLV antibodies by ELISA as 8,8% (31/353), with rates of 10,6% (19/179) and 6,9% (12/174) for female and male patients, respectively. Among HTLV-1/2 the 31 ELISA-positive patients it was noted that 15 (48.4%) of 31 had paresis (n = 8), parestesis (n = 5), and paraplegia (n = 3). Of these 31 HTLV ELISA positive patients, 25 could be submitted to WB for assessment of viral types, which were distributed as follow: 80% (20/25) were HTLV-1, 12% (3/25) were HTLV-2, one case was of HTLV-1+HTLV-2 infection (4%), and serum from one patient yielded an indeterminate profile (4%). Only 14 of these 25 patients could be re-localised for collection of an additional sample for molecular analysis. It was observed that 78.6% of samples typed by WB had the proviral TAX region successfully amplified by nested-PCR. In addition, types were confirmed as based on results obtained from the amplification of the POL region using real-time PCR; this denoted good specificity and sensitivity of the WB used in this study. The sample defined as HTLV-1+HTLV-2 infection by WB was amplified in its TAX region but real time PCR confirmed HTLV-1 infection only. The patient with WB indeterminate profile and one of samples typed as HTLV-2 by WB were amplified by nested-PCR but the real time PCR was negative for HTLV-1 and HTLV-2 in both samples. One patient presenting clinical manifestations of crural myalgia and parestesia with duration of about 7 years reacted HTLV-2-positive by both WB and real-time PCR, a denoting a clear HTLV-2- related chronic myelopathy. This study has identified a case of possible vertical transmission in two distinct situations: a patient whose mother presented antibodies for HTLV-1 by WB and two sisters who reacted HTLV-1-positive by WB and real-time PCR. Although of epidemiological relevance, results from this study warrant further and broader analyses concerning the molecular epidemiology of HTLV types and subtypes HTLV. In addition, a more complete clinical assessment of neurological symptoms should be further performed, in order to better characterise cases of HTLV-related chronic myelopathy in our region.

O tracoma como principal causa de cegueira prevenível no mundo, é uma doença negligenciada relacionada a baixas condições socioeconômicas e locais sem saneamento básico. Presente principalmente nos países subdesenvolvidos traz grandes prejuízos aos cofres públicos com a perda de produtividade e a deficiência visual. Com a criação da Aliança para Eliminação Global de Tracoma em 1997 (GET2020), o Estado do Pará, com apoio do Ministério da Saúde do Brasil, realizaram em 2006 o inquérito epidemiológico do tracoma em escolares de 1ª a 4ª série da rede oficial de ensino, nos municípios com índice de desenvolvimento humano inferior a média nacional, para conhecer a prevalência da doença. Os dados obtidos no inquérito comprovaram que a doença não foi erradicada, revelando 35 municípios paraenses prioritários e prevalências acima de 5%. Uma sub-amostra da conjuntiva de escolares clinicamente positivos foi coletada para a confirmação diagnóstica por Imunofluorescência direta (IFD). O presente estudo utilizou 52 amostras crio conservadas obtidas durante o inquérito, para serem analisadas pelos métodos de IFD e de biologia molecular, na identificação laboratorial da Chlamydia trachomatis. Foram encontradas as frequências de 26,92% (14/52) e 49% (24/49) de resultados positivos pelas técnicas de IFD e reação em cadeia da polimerase (nested-PCR), respectivamente. Considerando as 49 amostras analisadas pelas duas metodologias, as sensibilidades para a detecção do agente etiológico, por IFD e PCR foram de 28,57% (14/49) e 48,98% (24/49), respectivamente (p = 0,0127). As duas técnicas juntas confirmaram a infecção em 57,14% (n=28) das amostras, onde 50% (n=14) foram positivas apenas pela PCR, 35,72% (n=10) para ambas as técnicas e 14,28% (n=4) somente pela IFD. A análise de sete sequências nucleotídicas demonstrou homologia para isolados de C. trachomatis genótipo L1. Este estudo é pioneiro no Brasil, pois além de confirmar a presença de C. trachomatis em amostras oculares de escolares clínicamente positivos para tracoma, validou protocolo de obtenção de DNA a partir de lâminas de IFD crioconservadas, demonstrou a maior sensibilidade do método molecular frente à IFD e identificou o genótipo L1 presente nas amostras.

A disseminação das bactérias do gênero Chlamydia no Brasil, inclusive na região Amazônica, é pouco conhecida. Este estudo soroepidemiológico incluiu 2.086 amostras de soro de populações indígenas da Amazônia brasileira, empregando metodologia de triagem pela imunofluorescência indireta para pesquisa de anticorpos. Usou-se o sorotipo L2 da C. trachomatis como substrato; a seguir, para os quinze sorotipos de C. trachomatis e para a C. pneumoniae, discriminou-se a sororreatividade pela microimunofluorescência específica. A prevalência média de anticorpos para Chlamydia foi de 48,6%. Sua variação entre as comunidades indicou as que não tiveram contato com as bactérias e aquelas em que quase todos os testados tiveram. Por meio da titulação dos anticorpos IgG e a presença de IgM específica nas amostras com títulos altos viu-se que 6,1% dos infectados persistiam com a infecção, servindo de reservatórios à disseminação das espécies de Chlamydia. Pela resposta à C. trachomatis, evidenciou-se a circulação dos sorotipos A, B, Ba, D, E, G, H, I e L1. Ademais, constatou-se que há C. pneumoniae na região. As duas espécies causariam impacto significativo no hospedeiro humano.

To evaluate the oral health indicators by determining the experience of dental caries and periodontal disease and identification of self-perceived oral health status of pregnant adolescents and to assess the association between the studied variables. A transversal study, survey type and survey of the oral health status of 127 adolescents, 10-19 years of age, pregnant, accompanied by the Unified Health System of Araçatuba-SP was performed. The self-perceived oral health and socio-demographic data were recorded using an adapted questionnaire. The oral health statuses were analyzed using the DMFT and CPI indices in accordance with the criteria established by the WHO for epidemiological survey. Descriptive statistical analyzes and the Fisher's exact test with a significance level of 5% was performed, as well as a logistic regression analysis to verify the association between the variables. Of the total, 41.0% reported having satisfactory oral health, while 63.0% believed they had problems with their teeth and gums. The DMFT index was 12.51 (SD = 4.21). The percentage of caries-free was 6.3%, and 91.3% had periodontal problems. A statistically significant association was found between the variables: self-perceived oral health and periodontal disease p = 0.0166 and self-reported gum disease and periodontal disease p = 0.0039. Most patients considered their oral health as poor and reported having dental and gum problems, which can also be observed in the clinical examination since the caries experience of the pregnant women examined was considered high and the symptoms of periodontal disease were observed in most of the volunteers.

To improve mental health services, the World Health Organization proposes an “epidemiological approach” based on the constant screening of existing research, and aimed at continuous improvement of psychological treatment rather than strict application of prescribed techniques. This study provides an epidemiological survey conducted at the psychology ward of the municipal Ambulatório de Saúde Mental in Birigui, São Paulo, Brazil. Data from 180 patients in psychotherapeutic care were collected, and subsequent descriptive analysis showed that the population consisted predominantly of adults (82.8% of total) and females (81.0%). Depressive disorder was the most common symptom (61.1%), and the majority of the participants (72.2%) received psychological treatment for the first time. The data presented in this paper can assist mental health professionals in selecting appropriate treatment by creating a profile of patients.
