Gains or amplifications involving chromosome arm 8q are one of the most recurrent chromosomal alterations in head and neck tumors. To characterize previously reported gains, we performed fluorescence in situ hybridization (FISH) using the sequences BAC RP1179E1 and 8-centromere PMJ 128 as probes. Gains and/or amplifications were detected in all 19 cases evaluated by FISH. The FISH analysis, but not G-banding, revealed homogeneously staining region in three cases. We conclude that gains of one or more genes on chromosome arm 8q may be important for the early stages of head and neck carcinomas. (C) 2003 Elsevier B.V. All rights reserved.

OBJECTIVE: Pneumoperitoneum during laparoscopy results in transient oliguria and decreased glomerular filtration and renal blood flow. The presence of oliguria and elevated serum creatinine is suggestive of acute renal injury. Serum cystatin C has been described as a new marker for the detection of this type of injury. In this study, our aim was to compare the glomerular filtration rate estimated using cystatin C levels with the rate estimated using serum creatinine in patients with normal renal function who were undergoing laparoscopic surgery. METHODS: In total, 41 patients undergoing laparoscopic cholecystectomy or hiatoplasty were recruited for the study. Blood samples were collected at three time intervals: first, before intubation (T1); second, 30 minutes after the establishment of pneumoperitoneum (T2); and third, 30 minutes after deflation of the pneumoperitoneum (T3). These blood samples were then analyzed for serum cystatin C, creatinine, and vasopressin. The Larsson formula was used to calculate the glomerular filtration rate based on the serum cystatin C levels, and the Cockcroft-Gault formula was used to calculate the glomerular filtration rate according to the serum creatinine levels. RESULTS: Serum cystatin C levels increased during the study (T1 = T2T3; p<0.05). The calculated eGlomerular filtration rate-Larsson decreased, whereas the eGlomerular filtration rate-Cockcroft-Gault increased. There was no correlation between cystatin C and serum creatinine. Additionally, Pearson's analysis showed a better correlation between serum cystatin C and the eGlomerular filtration rate than between serum creatinine and the eGlomerular filtration rate. CONCLUSION: This study demonstrates that serum cystatin C is a more sensitive indicator of changes in the glomerular filtration rate than serum creatinine is in patients with normal renal function who are undergoing laparoscopic procedures.

The present study investigated the early presence of inflammatory response in renal tissue of young offspring from diabetic mothers. The effect of L-arginine (L-arg) supplementation was also investigated. The offspring was divided into four groups: group CO (controls); group DO (diabetic offspring); group CA (CO receiving 2% L-arg solution) and group DA (DO receiving the 2% L-arg solution). Glycemia, arterial pressure and renal function were evaluated; gene and protein expression of pro-inflammatory cytokines were also measured. Blood pressure levels were significantly increased in 2 and 6 month-old DO rats, whereas L-arg administration caused a significant decrease in the DA group, at both ages. DO rats showed a significantly blunted glycemic response to exogenous insulin. In 2 month-old DO animals, renal protein expression of pro-inflammatory molecules was significantly increased. At six months of age, we also observed an increase in gene expression of pro-inflammatory molecules, whereas L-arg supplementation prevented this increase at both ages. Our data suggest that activation of inflammatory pathways is present early in the kidney of DO rats, and that L-arg can attenuate the expression of these markers of tissue inflammation. Our results also reinforce the concept that intrauterine environmental factors are a fundamental determinant in the development of metabolic and vascular diseases later in life. OBJECTIVE: The aim of this study was to assess the microcirculatory and metabolic consequences of reduced mesenteric blood flow. DESIGN: Prospective, controlled animal study. SETTING: The surgical research unit of a university hospital. SUBJECTS: A total of 13 anesthetized and mechanically ventilated pigs. INTERVENTIONS: Pigs were subjected to stepwise mesenteric blood flow reduction (15% in each step, n = 8) or served as controls (n = 5). Superior mesenteric arterial blood flow was measured with ultrasonic transit time flowmetry, and mucosal and muscularis microcirculatory perfusion in the small bowel were each measured with three laser Doppler flow probes. Small-bowel intramucosal Pco2 was measured by tonometry, and glucose, lactate (L), and pyruvate (P) were measured by microdialysis. MEASUREMENTS AND MAIN RESULTS: In control animals, superior mesenteric arterial blood flow, mucosal microcirculatory blood flow, intramucosal Pco2, and the lactate/pyruvate ratio remained unchanged. In both groups, mucosal blood flow was better preserved than muscularis blood flow. During stepwise mesenteric blood flow reduction, heterogeneous microcirculatory blood flow remained a prominent feature (coefficient of variation, approximately 45%). A 30% flow reduction from baseline was associated with a decrease in microdialysis glucose concentration from 2.37 (2.10-2.70) mmol/L to 0.57 (0.22-1.60) mmol/L (p < .05). After 75% flow reduction, the microdialysis lactate/pyruvate ratio increased from 8.6 (8.0-14.1) to 27.6 (15.5-37.4, p < .05), and arterial-intramucosal Pco2 gradients increased from 1.3 (0.4-3.5) kPa to 10.8 (8.0-16.0) kPa (p < .05). CONCLUSIONS: Blood flow redistribution and heterogeneous microcirculatory perfusion can explain apparently maintained regional oxidative metabolism during mesenteric hypoperfusion, despite local signs of anaerobic metabolism. Early decreasing glucose concentrations suggest that substrate supply may become crucial before oxygen consumption decreases.

BACKGROUND Docetaxel is one of the most frequently used drugs to treat breast cancer. However, resistance or incomplete response to docetaxel is a major challenge. The aim of this study was to utilize MR metabolomics to identify potential biomarkers of docetaxel resistance in a mouse model for BRCA1-mutated breast cancer. METHODOLOGY High resolution magic angle spinning (HRMAS) (1)H MR spectroscopy was performed on tissue samples obtained from docetaxel-sensitive or -resistant BRCA1-mutated mammary tumors in mice. Measurements were performed on samples obtained before treatment and at 1-2, 3-5 and 6-7 days after a 25 mg/kg dose of docetaxel. The MR spectra were analyzed by multivariate analysis, followed by analysis of the signals of individual compounds by peak fitting and integration with normalization to the integral of the creatine signal and of all signals between 2.9 and 3.6 ppm. RESULTS The HRMAS spectra revealed significant metabolic differences between sensitive and resistant tissue samples. In particular choline metabolites were higher in resistant tumors by more than 50% with respect to creatine and by more than 30% with respect to all signals between 2.9 and 3.6 ppm. Shortly after treatment (1-2 days) the normalized choline metabolite levels were significantly increased by more than 30% in the sensitive group coinciding with the time of highest apoptotic activity induced by docetaxel. Thereafter, choline metabolites in these tumors returned towards pre-treatment levels. No change in choline compounds was observed in the resistant tumors over the whole time of investigation. CONCLUSIONS Relative tissue concentrations of choline compounds are higher in docetaxel resistant than in sensitive BRCA1-mutated mouse mammary tumors, but in the first days after docetaxel treatment only in the sensitive tumors an increase of these compounds is observed. Thus both pre- and post-treatment tissue levels of choline compounds have potential to predict response to docetaxel treatment.

Early Diagnosis of Miocardial Dysfunction in Patients with Hematological Malignancies Submitted to Chemotherapy. Preliminary Background: Considering the current diagnostic improvements and tl1erapeutic approaches, patients witl 1 cancer can now be healed or keep the disease under control, still, the chemotherapy may cause heart damage, evolving to Congestive Heart Failure. Recognition of those changes increases the chances of control the endpoints; hence, new parameters of cardiac and fluid mechanics analysis have been used to assess the myocardial function, pursuing an earlier diagnosis of the cardiac alterations. This study aimed to detect early cardiac dysfunction consequently to chemotherapy in patients with hematological malignancies (HM). Methods: Patients with leukemia and lymphoma, submitted to chemotherapy, without knowing heart diseases were studied. Healthy volunteers served as the control group. Conventional 2DE parameters of myocardial function were analyzed. The peak global longitudinal, circumferential and radial left ventricular (LV) strain were deternined by 2D and 3D speckle tracking (STE); peak area strain measured by 3D STE and LV torsionn, twisting rate, recoil / recoil rate assessed by 2D STE. The LV vortex formation time (VFT) during the rapid diastolic filling was estimated by the 2D mitral valve (MV) planimetry and Pulsed Doppler LV inflow by: VFT- 4(1-β) / π x α3 x LVEF Where 1- β is the E wave contribution to the LV stroke volume and α3 is a volumetric variable related to the MV area. The statistical level was settled on 5%. Results: See Table. Conclusion: Despite the differences between the two groups concerning the LVESV, LVEF and E´, those parameters still are in the normal range when considering the patients submitted to chemotherapy; thus, in the clinical setting, they are not so noticeable. The 3D GLS was smaller among the patients, oppositely to the 2D GLS, suggesting that the former variable is more accurate to assess tlhe LV systolic function. The VFT is a dimensionless measure of the optimal vortex development inside the LV chamber; reflecting the efficiency of the diastolic filling and, consequently, blood ejection. This index showed to be diminished in patients with HM submitted to chemotherapy, indicating an impairment of the in1pulse and thrust, hence appearing to be a very early marker of diastolic and systolic dysfunction in this group.

Background Late stage Ovarian Cancer is essentially incurable primarily due to late diagnosis and its inherent heterogeneity. Single agent treatments are inadequate and generally lead to severe side effects at therapeutic doses. It is crucial to develop clinically relevant novel combination regimens involving synergistic modalities that target a wider repertoire of cells and lead to lowered individual doses. Stemming from this premise, this is the first report of two- and three-way synergies between Adenovirus-mediated Purine Nucleoside Phosphorylase based gene directed enzyme prodrug therapy (PNP-GDEPT), docetaxel and/or carboplatin in multidrug-resistant ovarian cancer cells. Methods The effects of PNP-GDEPT on different cellular processes were determined using Shotgun Proteomics analyses. The in vitro cell growth inhibition in differentially treated drug resistant human ovarian cancer cell lines was established using a cell-viability assay. The extent of synergy, additivity, or antagonism between treatments was evaluated using CalcuSyn statistical analyses. The involvement of apoptosis and implicated proteins in effects of different treatments was established using flow cytometry based detection of M30 (an early marker of apoptosis), cell cycle analyses and finally western blot based analyses. Results Efficacy of the trimodal treatment was significantly greater than that achieved with bimodal- or individual treatments with potential for 10-50 fold dose reduction compared to that required for individual treatments. Of note was the marked enhancement in apoptosis that specifically accompanied the combinations that included PNP-GDEPT and accordingly correlated with a shift in the expression of anti- and pro-apoptotic proteins. PNP-GDEPT mediated enhancement of apoptosis was reinforced by cell cycle analyses. Proteomic analyses of PNP-GDEPT treated cells indicated a dowregulation of proteins involved in oncogenesis or cancer drug resistance in treated cells with accompanying upregulation of apoptotic- and tumour- suppressor proteins. Conclusion Inclusion of PNP-GDEPT in regular chemotherapy regimens can lead to significant enhancement of the cancer cell susceptibility to the combined treatment. Overall, these data will underpin the development of regimens that can benefit patients with late stage ovarian cancer leading to significantly improved efficacy and increased quality of life.

Chronic kidney disease (CKD) is a worldwide health problem, with adverse outcomes of cardiovascular disease and premature death. The ageing of populations along with the growing prevalence of chronic diseases such as diabetes and hypertension is leading to worldwide increase in the number of CKD patients. It has become evident that inflammation plays an important role in the pathogenesis of atherosclerosis complications. CKD patients also have an increased risk of atherosclerosis complications (including myocardial infarction, sudden death to cardiac arrhythmia, cerebrovascular accidents, and peripheral vascular disease). In line with this, oral and dental problems can be an important source of systemic inflammation. A decline in oral health may potentially act as an early marker of systemic disease progression. This series of studies examined oral health of CKD patients from predialysis, to dialysis and kidney transplantation in a 10-year follow-up study and in a cross-sectional study of predialysis CKD patients. Patients had clinical and radiographic oral and dental examination, resting and stimulated saliva flow rates were measured, whilst the biochemical and microbiological composition of saliva was analyzed. Lifestyle and oral symptoms were recorded using a questionnaire, and blood parameters were collected from the hospital records. The hypothesis was that the oral health status, symptoms, sensations, salivary flow rates and salivary composition vary in different renal failure stages and depend on the etiology of the kidney disease. No statistically significant difference were seen in the longitudinal study in the clinical parameters. However, some saliva parameters after renal transplantation were significantly improved compared to levels at the predialysis stage. The urea concentration of saliva was high in all stages. The salivary and plasma urea concentrations followed a similar trend, showing the lowest values in kidney transplant patients. Levels of immunoglobulin (Ig) A, G and M all decreased significantly after kidney transplantation. Increased concentrations of IgA, IgG and IgM may reflect disintegration of the oral epithelium and are usually markers of poor general oral condition. In the cross-sectional investigation of predialysis CKD patients we compared oral health findings of diabetic nephropathy patients to those with other kidney disease than diabetes. The results showed eg. more dental caries and lower stimulated salivary flow rates in the diabetic patients. HbA1C values of the diabetic patients were significantly higher than those in the other kidney disease group. A statistically significant difference was observed in the number of drugs used daily in the diabetic nephropathy group than in the other kidney disease group. In the logistic regression analyses, age was the principal explanatory factor for high salivary total protein concentration, and for low unstimulated salivary flow. Poor dental health, severity of periodontal disease seemed to be an explanatory factor for high salivary albumin concentrations. Salivary urea levels were significantly linked with diabetic nephropathy and with serum urea concentrations. Contrary to our expectation, however, diabetic nephropathy did not seem to affect periodontal health more severely than the other kidney diseases. Although diabetes is known to associate with xerostomia and other oral symptoms, it did not seem to increase the prevalence of oral discomfort. In summary, this series of studies has provided new information regarding the oral health of CKD patients. As expected, the commencement of renal disease reflects in oral symptoms and signs. Diabetic nephropathy, in particular, appears to impart a requirement for special attention in the oral health care of patients suffering from this disease.

A prevalência da obesidade e da síndrome metabólica (SM) vem aumentando dramaticamente em jovens e está se tornando um problema de saúde pública na maioria dos países desenvolvidos e em desenvolvimento. Tanto a obesidade quanto a SM aumentam o número de pacientes expostos ao risco de doença cardiovascular. Estudos recentes mostram que uma redução na biodisponipilidade de óxido nítrico (NO) é um dos principais fatores que contribui para a ação deletéria da insulina nos vasos de pacientes adultos com obesidade e SM. O NO, potente vasodilatador e anti-agregante plaquetário, tem como precursor o aminoácido catiônico L-arginina que é transportado para o interior das plaquetas através do carreador y+L. Uma família de enzimas denominadas NO sintases (NOS) catalisa a oxidação da L-arginina em NO e L-citrulina e é composta de três isoformas: neuronal (nNOS), induzível (iNOS) e endotelial (eNOS). Os objetivos principais do presente estudo são de investigar diferentes etapas da via L-arginina-NO em plaquetas associando agregação plaquetária, concentração plasmática de L-arginina, estresse oxidativo, marcadores metabólicos, hormonais, clínicos e inflamatórios em pacientes adolescentes com obesidade e SM. Foram incluídos no estudo trinta adolescentes, sendo dez com obesidade, dez com SM, e dez controles saudáveis pareados por idade, sexo e classificação de Tanner (controles: n= 10, 15.6 0.7 anos; obesos: n= 10, 15 0.9 anos; SM: n= 10, 14.9 0.8 anos). O transporte de L-arginina (pmol/109céls/min) através do sistema y+L estava diminuído nos pacientes com SM (18.4 3.8) e obesidade (20.8 4.7), comparados aos controles (52.3 14.8). Houve uma correlação positiva do influxo de L-arginina via sistema y+L com os níveis de HDL-Colesterol. Por outro lado, foi encontrada uma correlação negativa do influxo de L-arginina com os níveis de insulina, os índices Homa IR, relacionado a RI, Homa Beta, relacionado a função da célula beta e também com os índices de Leptina. Em relação a produção de NO, a obesidade e a SM não afetaram a atividade e expressão das enzimas NOS. A atividade da superóxido dismutase (SOD), através da mensuração da inibição da auto-oxidação da adrenalina, mostrou diferença significativa nas plaquetas de pacientes com obesidade (4235 613,2 nMol/mg de proteína), quando comparada aos controles (1011 123,6 nmol/mg de proteína) e SM (1713 267,7 nmol/mg de proteína). A nível sistêmico, foi também evidenciada uma ativação desta enzima anti-oxidante no soro de pacientes obesos, em relação aos controles. A peroxidação lipídica avaliada pelas substâncias reativas ao ácido tiobarbitúrico (TBARS) estava inalterada no soro dos pacientes e controles. Estes resultados sugerem que o transporte de L-arginina diminuído nas plaquetas de adolescentes obesos e com SM pode ser um marcador precoce de disfunção plaquetária. A alteração desta via correlaciona-se com a resistência à insulina e hiperinsulinemia. A contribuição deste estudo e de fatores que possam ser precocemente identificados pode diminuir o risco cardiovascular na vida adulta desta população de pacientes.

Malgré les avancées médicales, la prédiction précoce du devenir développemental des enfants nés prématurément demeure un défi. Ces enfants sont à risque de séquelles plus ou moins sévères telles l'infirmité motrice d'origine cérébrale, les déficiences intellectuelles et sensorielles ainsi que les difficultés d'apprentissage. Afin de diminuer l’impact fonctionnel de ces séquelles, l’identification de marqueurs précoces devient un enjeu important. Dans le contexte actuel de ressources financières et humaines limitées, seuls les enfants nés avant 29 semaines de gestation ou avec un poids de naissance (PN) <1250g sont systématiquement suivis, laissant pour compte 95% des enfants prématurés. L’identification de marqueurs précoces permettrait de cibler les enfants nés après 28 semaines de gestation porteurs de séquelles. Le principal objectif des présents travaux visait à évaluer l’utilité de l’Évaluation neurologique d’Amiel-Tison (ENAT) dans l’identification et le suivi des enfants nés entre 29 et 37 semaines de gestation et qui présenteront des problèmes neurodéveloppementaux à l’âge corrigé (AC) de 24 mois. Plus précisément, la fidélité inter-examinateurs, la stabilité ainsi que la validité prédictive de l’ENAT ont été évaluées. La cohorte était composée initialement de 173 enfants nés entre 290/7 et 370/7 semaines de gestation, avec un PN<2500g et ayant passé au moins 24 heures à l’unité de soins néonatals du CHU Sainte-Justine. Les enfants étaient évalués avec l’ENAT à terme et aux AC de 4, 8, 12 et 24 mois. À l’AC de 24 mois, leur développement était évalué à l’aide du Bayley Scales of Infant Development–II. Les principaux résultats révèlent une excellente fidélité inter-examinateurs ainsi qu’une bonne stabilité au cours des deux premières années de vie du statut et des signes neurologiques. Des différences significatives à l’AC de deux ans ont été relevées aux performances développementales en fonction du statut neurologique à terme, qui constitue l’un des meilleurs facteurs prédictifs de ces performances. Les résultats encouragent l’intégration du statut neurologique tel que mesuré par l’ENAT comme marqueur précoce dans le cours d’une surveillance neurodéveloppementale des enfants les plus à risque.

La dysfonction endothéliale vasculaire constitue un marqueur précoce des maladies cardiovasculaires car l’endothélium est l’une des premières cibles des facteurs de risque cardiovasculaire. La présence d'un stress chronique engendré par les facteurs de risque cardiovasculaire sollicite les mécanismes de défense endogènes, tels que les enzymes antioxydantes, qui servent au maintien de la fonction endothéliale. L’environnement vasculaire auquel l’endothélium est exposé a un effet direct sur son fonctionnement à long terme. Certaines habitudes de vie sont ainsi associées à une bonne santé cardiovasculaire. Par exemple, la diète méditerranéenne et/ou la pratique régulière de l’exercice physique aident à maintenir une fonction endothéliale adéquate et à réduire l’incidence des maladies cardiovasculaires. D'autre part, certains gènes clés, comme le gène suppresseur de tumeurs p53, régulent plusieurs voies métaboliques importantes pour préserver l’intégrité des cellules endothéliales. Nous posons l’hypothèse que l’environnement vasculaire post-natal influence la mise en place de mécanismes de défenses endogènes tels que les enzymes antioxydantes afin de faire face à des stress plus tard dans la vie. Notre objectif global était d’évaluer les impacts d’interventions post-natales bénéfiques et d’une diminution endogène du gène suppresseur de tumeurs p53, sur la fonction endothéliale vasculaire et sur sa capacité à faire face à un stress métabolique. Dans une première étude, nous avons soumis des souris saines C57Bl/6 dès leur sevrage et jusqu’à l’âge de 9 mois, à un programme d’exercice physique volontaire (course dans une roue) ou à un antioxydant (catéchine), comparé à un groupe de souris sédentaires et sans antioxydant. Puis les interventions ont été stoppées et une diète riche en gras a été introduite, ou non, pour une période de 3 mois; les souris ont été sacrifiées à l'âge de 9 ou 12 mois. Nous avons observé que l’exercice a protégé les cellules endothéliales des effets délétères induits par la diète riche en gras en préservant la fonction endothéliale par le maintien d’un profil rédox sain et en évitant la hausse de l’inflammation. La catéchine a maintenu la fonction endothéliale aortique, mais n’a pas prévenu le profil inflammatoire en présence de la diète riche en gras. Finalement, chez les souris sédentaires, la fonction endothéliale a été détériorée en présence de la diète riche en gras, sans indice d’inflammation vasculaire. Dans une seconde étude, des souris partiellement déficientes en p53 (p53+/-) et contrôles C57Bl/6 ont été exposées à la même diète riche en gras à partir de 3 mois et ce jusqu’à l’âge de 6 mois. Notre raisonnement était basé sur la démonstration que p53 est un régulateur de l’expression des enzymes antioxydantes in vitro. Chez les souris p53+/-, les cellules endothéliales ont été protégées du stress induit par l’hypercholestérolémie engendrée par la diète riche en gras. Cependant, chez les souris p53+/- cette protection pourrait être secondaire à un métabolisme accru des acides biliaires, qui en prévenant la hausse de cholestérol, protègerait indirectement l'endothélium. Nous avons donc pu démontrer l’importance de l’environnement vasculaire sur la fonction endothéliale. La diète riche en gras a stimulé certains mécanismes de défense vasculaires tels que la voie des EDHF et la superoxyde dismutase afin de maintenir la fonction endothéliale malgré les conditions pro-athérosclérotiques. Nous avons observé que l’exercice et la catéchine influencent différemment l’endothélium malgré leurs capacités antioxydantes. Ces études soulignent la sensibilité de l’endothélium aux changements dans l’environnement vasculaire. En accord avec le vieillissement de la population et la progression des maladies cardiovasculaires, la proportion de personnes ayant une dysfonction endothéliale augmente. Ainsi, une meilleure compréhension des mécanismes ou d’interventions qui permettent le maintien de la fonction endothéliale à long terme s’avère utile.

Introducción: La Preeclampsia ocurre entre el 2-7% de los embarazos. Previos estudios han sugerido la asociación entre los niveles alterados de PAPP-A y la β-hCG libre con el desarrollo de Preeclampsia (PE) y/o Bajo Peso al Nacer (BPN). Metodología: El diseño del estudio es de Prueba Diagnóstica con enfoque de casos y controles. Las mediciones séricas de PAPP-A y la β-hCG libre, fueron realizadas entre la semana 11-13.6 días durante 2 años. Resultados: La cohorte incluyó 399 pacientes, la incidencia de PE fue de 2,26% y de BPN fue de 14.54%. El punto de corte del percentil 10 fue MoM PAPP-A: 0,368293 y MoM β-hCG libre: 0,412268; la especificidad en PE leve fue de 90,5 y para BPN de 90. Los MoM de la β-hCG libre, la edad y el peso materno se comportan como factores de riesgo, mientras que mayores valores de MoM de la PAPP-A y mayor número de partos factores de protección. Para el BPEG severo la edad materna y la paridad se comportan como factores de riesgo, mientras que un aumento promedio de los valores de los MoM de la PAPP-A y la β-hCG libre, como factores de protección en el desarrollo de BPEG Severo. Conclusiones: Existe una relación significativa entre los valores alterados de PAPP-A y de β-hCG libre, valorados a la semana 11 a 13 con la incidencia de Preeclampsia y de Bajo Peso al nacer en fetos cromosómicamente normales, mostrando unos niveles significativamente más bajos a medida que aumentaba la severidad de la enfermedad.

Objetivo: Describir los factores relacionados con la toma de decisión de manejo quirúrgico en pacientes con hidronefrosis secundaria a estrechez de la unión pieloureteral en el servicio de Urología Pediátrica de una institución de IV nivel. Materiales y Métodos: Se realizó un estudio descriptivo retrospectivo. Se seleccionaron por conveniencia a 100 pacientes con diagnóstico antenatal de hidronefrosis, 37 fueron llevados a manejo quirúrgico por estrechez de la unión pieloureteral (EUPU) entre los años 2009 y 2012. Se evaluaron los factores que llevaron a la toma| de esta decisión. Resultados: Los pacientes con diagnóstico postnatal de EUPU representaron el 37% de la población, la indicación de manejo quirúrgico en 13 pacientes (35,1%) fue dilatación caliceal (SFU 3), en 21 pacientes (56,8%) de deterioro de la función renal y en los restantes (8,1%) infección urinaria recurrente. Se encontró una progresión de 30% en la severidad de la dilatación en el periodo postnatal, habían 9 pacientes (24% de la muestra) SFU de 3 y 4 en el periodo prenatal y 20 (54%) en el periodo postnatal que fueron llevados a manejo quirúrgico. De los pacientes que disponíamos de datos precisos de valores de variación porcentual de gammagrafía 16% de la muestra, se encontró que había una variación del 50% en deterioro de la función renal. Conclusión: En el grupo de pacientes colombianos de la consulta externa del servicio de urología pediátrica estudiado se encontró que la decisión de manejo quirúrgico en pacientes con EUPU, está en concordancia con lo encontrado en la literatura mundial, siendo estos la presencia de dilatación caliceal deterioro de la función renal en gammagrafía DMSA.

Examines the interrelationships between body build and body composition and carbohydrate and lipid metabolism in healthy young men and women. Results indicate that in women a low relative sitting height may be an early marker of susceptibility to abdominal obesity and its consequences, non insulin dependent diabetes and coronary heart disease.
