The apical surgery with simultaneous root filling consists in the elimination of the local etiologic factors concomitant with the shaping and obturation of the root canal system. This technique is indicated when there is no possibility of obturating the root canal by conventional methods. Some adverse factors may difficult the clinical procedures, such as the radiographic interpretation. Periapical radiographs must not be used as the main diagnostic resource due to their inherent disadvantages, such as distortion and superimposition with other anatomical structures. The aim of this clinical case report is to show the limitations of radiographic examination in an apical surgery in which a root perforation was not visible in the radiographic image, as well as the gutta-percha was not related to its real clinical location.

OBJECTIVES: Pleomorphic adenomas are the most frequent type of epithelial salivary gland neoplasms, and their malignant counterpart, the carcinoma in pleomorphic adenomas, is much less common. Beta-catenin is a cell adhesion molecule associated with the invasion and metastasis of carcinomas of the head and neck, esophagus. The objective of this study was to detect the expression of beta-catenin in pleomorphic adenomas, carcinomas in pleomorphic adenomas and normal salivary glands to discuss its role in the development of these two lesions. STUDY DESIGN: The expression of beta-catenin (BD Transduction Laboratories) was analyzed by immunohistochemistry in formalin-fixed, paraffin embedded specimens by the avidin-biotin-peroxidase complex method in 16 pleomorphic adenomas (12 from minor salivary glands), 3 carcinomas in pleomorphic adenomas (all from palate) and 10 normal salivary glands as control group (5 from major and 5 from minor salivary glands). RESULTS: All cases of glands, adenomas and carcinomas in pleomorphic adenomas have membranous and cytoplasmic immunostaining. Nuclear beta-catenin immunostaining was not observed. The antibody presented a fine granular arrangement in the cytoplasm and cellular membrane of duct and acinic cells. Higher beta-catenin index rates were seen mainly in salivary gland ducts and in ductal structures in the adenomas and carcinomas in pleomorphic adenomas. There was protein loss in pleomorphic adenomas and cytoplasmic accumulation in carcinoma in pleomorphic adenomas. CONCLUSIONS: The present study showed participation of the loss of beta-catenin adhesion molecule in the development of pleomorphic adenoma, and that the cytoplasmic accumulation of the molecule takes part in the malignant transformation of the pleomorphic adenoma into carcinoma in pleomorphic adenoma.

The oral rehabilitation with osseointegrated implants is a well-documented treatment with high rate of success. Nevertheless, demands related to treatment time and surgical technique began to appear. In this context, the procedure of immediate loading in which the dental prosthesis is placed right after implant surgery has become a constant practice. Although immediate loading has been an important advance, minimally invasive procedures, such as implant placement without raising a mucoperiosteal flap (flapless) are increasingly being performed. Association of immediate loading with the flapless technique improves acceptance by patients and professionals, because no suturing is required. Moreover, it reduces swelling, bleeding during and after surgery, postoperative pain, surgery time, discomfort and hematoma, as well as the need for postoperative medication. These characteristics ease the stages of rehabilitation soon after implant placement, cooperating with prosthodontist's work. Thus, the proposal of this study is to present a clinical case of oral rehabilitation with osseointegrated implants and fixed prosthesis in both arches, in which the flapless technique was applied, followed by immediate loading. It will discuss the diagnosis, prosthetic planning, surgical/prosthodontic procedures and follow-up for 20 months.

Many incisions have been described for approaches to zygomatic fractures. Precise repositioning of zygomatic complex fractures is difficult. The traditional approach is through an eyebrow incision, but it can produce a scar that causes aesthetic and psychological problems for the patient. We describe the supratarsal fold approach to expose the frontozygomatic suture and to reduce small displacements of frontal sinus anterior wall; it gives good access and excellent aesthetic results.

Necrotizing ulcerative periodontitis is associated with bacteria and predisposing risk factors. The treatment is centered in elimination and/or control of the bacteria and predisposing risk factors. However the disease acute phase could induce sequelae as periodontal tissue destruction which may be treated by periodontal surgical procedures.

Multiple gingival recessions may cause mainly dentinary sensibility and aesthetic alterations. The subepithelial graft is a mucogingival technique which can be used to treat multiple gingival recessions. In this case report the Erbium YAG laser was applied to promote radicular surface decontamination, an important step to reach reinsertion of the gingival tissues on exposed radicular surface.

INTRODUCTION: In periapical surgery, the absence of standardization between different studies makes it difficult to compare the outcomes. OBJECTIVE: To compare the healing classification of different authors and evaluate the prognostic criteria of periapical surgery at 12 months. MATERIAL AND METHODS: 278 patients (101 men and 177 women) with a mean age of 38.1 years (range 11 to 77) treated with periapical surgery using the ultrasound technique and a 2.6x magnifying glass, and silver amalgam as root-end filling material were included in the study. Evolution was analyzed using the clinical criteria of Mikkonen et al., 1983; radiographic criteria of Rud et al., 1972; the overall combined clinical and radiographic criteria of von Arx and Kurt, 1999; and the Friedman (2005) concept of functional tooth at 12 months of surgery. RESULTS: After 12 months, 87.2% clinical success was obtained according to the Mikkonen et al., 1983 criteria; 73.9% complete radiographic healing using Rud et al. criteria; 62.1% overall success, following the clinical and radiographic parameters of von Arx and Kurt, and 91.9% of teeth were functional. The von Arx and Kurt criteria was found to be the most reliable. CONCLUSION: Overall evolution according to von Arx and Kurt agreed most closely with the other scales.

The diseases that affect the oral cavity are wide and diverse, comprising a broad spectrum of either benign or malignant lesions. However, few histological-based studies were performed for the evaluation of oral cavity lesions, and very few directed to oral soft tissue pathology. The aim of this study was to carry out pioneering research, within a Portuguese population, to determine the frequency and characteristics of oral malignancies, potential malignant disorders, and soft benign tissues pathologies submitted for biopsy in a north Portugal (Oporto) hospital population.

OBJECTIVES The shear bond strength of three glass ionomer cements (GIC) to enamel and dentine was evaluated. STUDY DESIGN Sound permanent human molars (n=12) were grinded perpendicular to their axial axes, exposing smooth, flat enamel and dentine surfaces. The teeth were embedded in resin and conditioned with polyacrylic acid (25%; 10s). Twenty four specimens of each GIC: Fuji IX (FJ-GC), Ketac Molar Easymix (KM-3M ESPE) and Maxxion (MX-FGM) were prepared according to the Atraumatic Restorative Treatment (ART) (12 enamel and 12 dentine), in a bonding area of 4.91 mm² and immersed in water (37°C, 24h). The shear bond strength was tested in a universal testing machine. Non-parametric statistical tests (Friedman and post-hoc Wilcoxon Signed Ranks) were carried out (p=0.05). RESULTS The mean (±sd) of shear bond strength (MPa), on enamel and dentine, were: KM (6.4±1.4 and 7.6±1.5), FJ (5.9±1.5 and 6.0±1.9) and MX (4.2±1.5 and 4.9±1.5), respectively. There was a statistically significant difference between the GICs in both groups: enamel (p=0.004) and dentine (p=0.002). The lowest shear bond value for enamel was with MX and the highest for dentine was KM (p<0.05). CONCLUSION It is concluded that KM has the best adhesion to both enamel and dentine, followed by FJ and MX.

INTRODUCTION Fear and anxiety are part of all human experiences and they may contribute directly to a patient's behavior. The Atraumatic Restorative Treatment (ART) is a technique that may be an alternative approach in treating special care patients or those who suffer fear or anxiety. OBJECTIVE the aim of this paper is to review the ART technique as an alternative to reduce pain and fear during dental treatment. MATERIAL AND METHODS A search for the term "atraumatic restorative treatment" was carried out in the MEDLINE search engine. References, from the last 10 years, containing at least one of the terms: "psychological aspects", "discomfort", "fear", "anxiety" or "pain", were selected. RESULTS A total of 120 references were found, from which only 17 fit the criteria. Discussion: All authors agreed that the ART promotes less discomfort for patients, contributing to a reduction of anxiety and fear during the dental treatment. Results also indicated that ART minimizes pain reported by patients. CONCLUSIONS The ART approach can be considered as having favorable characteristics for the patient, promoting an "atraumatic" treatment. This technique may be indicated for patients who suffer from fear or anxiety towards dental treatments and whose behavior may cause the treatment to become unfeasible or even impossible altogether.

El objetivo del presente estudio fue determinar el efecto del protocolo de exodoncias de premolares sobre el perfil blando durante el tratamiento de ortodoncia en pacientes atendidos en un consultorio particular de la provincia de Chiclayo, durante el periodo 2014 – 2015. El diseño del presente estudio fue básico, analítico y retrospectivo, donde la unidad de análisis serán las radiografías cefalométricas tomadas antes y después del tratamiento de ortodoncia en pacientes que acudieron a un consultorio particular. Se analizarán 21 pacientes pre y post tratamiento divididas en 2 grupos: grupo casos en los cuales se le realizó las exodoncias y grupo control donde no se realizaron exodoncias contando con 20 y 22 placas radiográficas respectivamente. Se emplearon 8 mediciones para la evaluación del perfil blando: ángulo nasofacial, Línea E de Ricketts, ángulo nasolabial, ángulo facial de tejidos blandos, profundidad del surco labial superior, distancia Subnasal a la línea H, ángulo H y labio inferior a la línea H. En el análisis de datos se presentó la información descriptiva mediante tablas (frecuencias absolutas y porcentuales) y gráficos. Así mismo, para el análisis inferencial se usó prueba T para muestras independientes y prueba T para muestras relacionadas a un nivel de confiabilidad de 95%. El presente estudio concluyó que el protocolo de exodoncias de premolares tiene el efecto de modificar el perfil blando con una diferencia significativa en los ángulo nasofacial, Línea E de Ricketts-labio superior y ángulo H . y con mayor porcentaje en el sexo femenino y en pacientes de 17 a 19 años de edad después el tratamiento de ortodoncia en pacientes atendidos en un consultorio particular.

RESUMEN Introducción: La cirugía extracorpórea es el procedimiento quirúrgico torácico en la que se accede a las cavidades del corazón para realizar recambios valvulares, puentes cardiopulmonares y otras intervenciones. Se accede al mediastino mediante esternotomía. Una maquina asumirá la función del corazón, realizando la circulación extracorpórea mientras este es intervenido. El paciente será derivado a las unidades de cuidados intensivos, donde deberá ser controlado por el personal sanitario controlando los dispositivos y catéteres, disminuyendo las posibles complicaciones postoperatorias. Objetivo: Conocer las complicaciones postoperatorias de la cirugía extracorpórea, describir las complicaciones postoperatorias más habituales de la cirugía, analizar las variables con mayor influencia en esta cirugía y comparar los beneficios de la cirugía extracorpórea frente a la revascularización miocárdica sin cirugía extracorpórea, mediante el análisis de fuentes de datos. Material y métodos: Se ha realizado una búsqueda bibliográfica informatizada en las bases de datos Cuiden, Pubmed, Elservier, CSIC, Dialnet y Google Académico de fuentes de datos actuales sobre la cirugía extracorpórea, mediante unos criterios de inclusión y exclusión determinados. Está dividida en 3 partes: investigación, selección y extracción de datos. Resultados: Se han analizado 8 estudios. Se han dividido en diferentes grupos, la muestra de los estudios analizados va desde 11 pacientes hasta 18.252, midiendo variables de dolor, sangrado, reintervención y otras. Conclusiones: La complicación más frecuente en el postoperatorio es el dolor. Muchas de las complicaciones se dan habitualmente en las primeras 24 horas posteriores a la intervención. Tiene un índice de mortalidad bajo, pero es mayor que en la revascularización miocárdica sin cirugía extracorpórea.

Esta tese tem por objeto o processo de regionalização das ações de média complexidade e da oferta de próteses dentárias no âmbito da Política Nacional de Saúde Bucal (PNSB). Foram realizadas duas pesquisas fundamentais para o entendimento desse processo: a primeira esteve voltada para o levantamento da descrição da oferta de ações especializadas e de próteses dentárias nos Planos Diretores de Regionalização (PDR) nas 27 unidades federadas do país. A segunda pesquisa verificou a cobertura por Equipes de Saúde Bucal (ESBs) na Estratégia de Saúde da Família (ESF) e a distribuição dos 844 Centros de Especialidades Odontológicas (CEOs) e dos 526 Laboratórios Regionais de Próteses Dentárias (LRPDs) implantados até setembro de 2010 nas regiões de saúde do Brasil dos 27 estados da federação. Essas pesquisas permitiram concluir que: os PDRs, na grande maioria dos estados brasileiros, não contribuíram para a organização regionalizada da distribuição de CEOs e LRPDs no Brasil. A cobertura por Equipes de Saúde Bucal é heterogênea, com predominância da Região Nordeste e dos municípios de pequeno porte, em detrimento das capitais e dos estados das regiões Sul e Sudeste. No tocante à distribuição das unidades CEO e LRPD pelas regiões de saúde, a pesquisa mostrou que os critérios normativos para a seleção dos municípios a sediarem essas unidades vêm sendo cumpridos de forma precária na maior parte do país. Além disso, a distribuição dessas unidades não apresenta coerência com os princípios da regionalização prevista pelo Pacto de Gestão do SUS.
