Reports on the clinical course of mycophenolic acid (MPA)-related colitis in kidney transplant recipients are scarce. This study aimed at assessing MPA-related colitis incidence, risk factors, and progression after kidney transplantation. All kidney transplant patients taking MPA who had colonic biopsies for persistent chronic diarrhea, between 2000 and 2012, at the Kidney Transplantation Unit of Botucatu Medical School Hospital, Brazil, were included. Cytomegalovirus (CMV) immunohistochemistry was performed in all biopsy specimens. Data on presenting symptoms, medications, immunosuppressive drugs, colonoscopic findings, and follow-up were obtained. Of 580 kidney transplant patients on MPA, 34 underwent colonoscopy. Colonoscopic findings were associated with MPA usage in 16 patients. The most frequent histologic patterns were non-specific colitis (31.3%), inflammatory bowel disease (IBD)-like colitis (25%), normal/near normal (18.8%), graft-versus-host disease-like (18.8%), and ischemia-like colitis (12.5%). All patients had persistent acute diarrhea and weight loss. Six of the 16 MPA-related diarrhea patients (37.5%) showed acute dehydration requiring hospitalization. Diarrhea resolved when MPA was switched to sirolimus (50%), discontinued (18.75%), switched to azathioprine (12.5%), or reduced by 50% (18.75%). No graft loss occurred. Four patients died during the study period. Late-onset MPA was more frequent, and no correlation with MPA dose or formulation was found.

Background: Glypican 3 (GPC3) is a member of the family of glypican heparan sulfate proteoglycans (HSPGs). The GPC3 gene may play a role in controlling cell migration, negatively regulating cell growth and inducing apoptosis. GPC3 is downregulated in several cancers, which can result in uncontrolled cell growth and can also contribute to the malignant phenotype of some tumors. The purpose of this study was to analyze the mechanism of action of the GPC3 gene in clear cell renal cell carcinoma.Methods: Five clear cell renal cell carcinoma cell lines and carcinoma samples were used to analyze GPC3 mRNA expression (qRT-PCR). Then, representative cell lines, one primary renal carcinoma (786-O) and one metastatic renal carcinoma (ACHN), were chosen to carry out functional studies. We constructed a GPC3 expression vector and transfected the renal carcinoma cell lines, 786-O and ACHN. GPC3 overexpression was analyzed using qRT-PCR and immunocytochemistry. We evaluated cell proliferation using MTT and colony formation assays. Flow cytometry was used to evaluate apoptosis and perform cell cycle analyses.Results: We observed that GPC3 is downregulated in clear cell renal cell carcinoma samples and cell lines compared with normal renal samples. GPC3 mRNA expression and protein levels in 786-O and ACHN cell lines increased after transfection with the GPC3 expression construct, and the cell proliferation rate decreased in both cell lines following overexpression of GPC3. Further, apoptosis was not induced in the renal cell carcinoma cell lines overexpressing GPC3, and there was an increase in the cell population during the G1 phase in the cell cycle.Conclusion: We suggest that the GPC3 gene reduces the rate of cell proliferation through cell cycle arrest during the G1 phase in renal cell carcinoma.

With the improvement in quality of life of animals, it is increasingly frequent clinical care of elderly patients, which present renal disorders, including chronic renal failure. Recent studies report the use of stem cells to treat renal failure, which would improve the levels of urea and creatinine, and in renal ultrasound evaluation. With the present work, the idea is to report a case of ultrasonographic evaluation in a patient with chronic renal failure, liver disease and splenic nodule, which underwent stem cell therapy, where there was an improvement in the sonographic evaluation of part of the liver.

An 8-month-old Lhasa Apso female dog was referred to the Veterinary Hospital, FMVZ, Unesp, Botucatu Campus, with suspected congenital renal disorder. Ultrasound images revealed higher renal echogenicity, loss of corticomedullary demarcation and diverticular mineralization on the right kidney. The mucosal lining of the stomach was hyperechoic and radiographic examination demonstrated mineralization of wall and folds, which is consistent with uremic gastropathy.

Renal abscesses are rare in dogs and, although its pathogenesis is uncertain, may occur in diabetic patients with pyelonephritis, nephrolithiasis, kidney injuries or after abdominal surgery or kidney biopsy. Because of nonspecific clinical signs, laboratory tests associated with ultrasonography are important to confirm the diagnosis and treatment. Generally, kidney abscesses have sonographic aspects with different degrees of echogenicity such as hypo or hyperechoic, homogeneous or heterogeneous, and may or may not have thickened and hyperechoic walls, depending on its chronicity. Our objective was to report the use of ultrasound as a complementary method in the evaluation of disorders of the urinary tract, focusing on a case of renal abscess in a dog.

A área septal medial (ASM), situada no prosencéfalo, está envolvida na regulação cardiovascular e no controle do balanço hidroeletrolítico. Esta área é rica em receptores colinérgicos e a ativação dos mesmos induz ingestão de água, natriurese e anti-diurese. Existem evidências experimentais de que as espécies reativas de oxigênio possam participar do controle de respostas fisiológicas. Resultados recentes de nosso laboratório demonstraram que uma espécie reativa de oxigênio, o peróxido de hidrogênio (H2O2), injetada na ASM reduz a ingestão de água induzida por carbacol (agonista colinérgico) também injetado na ASM. No presente projeto propomos estudar os efeitos do aumento da produção endógena de H2O2 na ASM nos efeitos dipsogênico, natriurético e anti-diurético do carbacol injetado na ASM. O inibidor da catalase, 3 amino 1,2,4 triazole (ATZ) será injetado na ASM para aumentar os níveis endógenos de H2O2

Maternal undernutrition affects the foetal development, promoting renal alterations and adult hypertension. The present study investigates, in adult male rats, the effect of food restriction in utero on arterial blood pressure changes (AP), and its possible association with the number of nephrons, renal function and angiotensin II (AT1R/AT2R), glucocorticoid (GR) and mineralocorticoid (MCR) receptors expression. The daily food supply to pregnant rats was measured and one group (n=5) received normal quantity of food (NF) while the other group received 50% of that (FR50) (n=5). The AP was measured weekly. At 16 weeks of life, fractionator’s method was used to estimate glomeruli number in histological slices. The renal function was estimate by creatinine and lithium clearances. Blood and urine samples were collected to biochemical determination of creatinine, sodium, potassium and lithium. At 90th and 23rd days of life, kidneys were also processed to AT1R, AT2R, GR and MCR immunolocalization and for western blotting analysis. FR50 offspring shows a significant reduction in BW (FR50: 5.67 ± 0.16 vs. 6.84 ± 0.13g in NF, P<0.001) and increased AP from 6th to 12nd week (6thwk FR50: 149.1 ± 3.4 vs. 125.1 ± 3.2mmHg in NF, P<0.001and, 12ndwk FR50: 164.4 ± 4.9 vs. 144.0 ± 3.3 mmHg in NF, P=0.02). Expression of AT1R and AT2R were significantly decreased in FR50 (AT1, 59080 ± 2709 vs. 77000 ± 3591 in NF, P=0.05; AT2, 27500 ± 95.50 vs. 67870 ± 1509 in NF, P=0.001) while the expression of GR increased in FR50 (36090 ± 781.5 vs. 4446 ± 364.5 in NF, P=0.0007). The expression of MCR did not change significantly. We also verified a pronounced decrease in fractional urinary sodium excretion in FR50 offspring (0.03 ± 0.02 vs. 0.06 ± 0.04 in NF, p=0.03). Uremic syndrome, arising from kidney malfunction, consists of a set of systemic changes caused by the accumulation of toxic substances to the body. Since, with the advance of medicine, the animals lived more reaching advanced age and entering thus on track of senility, chronic renal disease, became a common complaint in the routine of the ambulatory. This high rate of morbidity generates an increased need for clarification of pathophysiology involved in this disease. The components of the uremic syndrome include water and sodium imbalance, anemia, intolerance to carbohydrate, neurological disorders, disorders of the gastrointestinal tract, osteoarthritis, immunological incompetence and metabolic acidosis. The clinical manifestations occur in isolation or in combination. In most cases canines patients are subject to an assessment when the kidney disease has evolved to the final stage with uremic syndrome and installed already, under these conditions, the prognosis is reserved

A Doença Renal Crônica (DRC) é considerada um problema mundial de saúde pública. O paciente com DRC apresenta baixa tolerância ao exercício físico e como conseqüência apresenta limitações na capacidade funcional e força muscular respiratória. O paciente com DRC em programa de hemodiálise pode sofrer impactos negativos na sua capacidade funcional. Objetivo: Avaliar a capacidade funcional e a força muscular respiratória associada ao exercício físico em pacientes com doença renal crônica que realizam hemodiálise. Metodologia: 28 pacientes de ambos os sexos com idade entre 40 e 60 anos participaram do estudo, em programa de hemodiálise no Instituto do Rim da Santa Casa de Misericórdia de Presidente Prudente-SP. Os pacientes foram avaliados pela força muscular inspiratória (PImax) e força muscular expiratória (PEmax) pela manovacuometria e Teste de Caminhada de Seis Minutos (TC6’) para avaliar a capacidade funcional. Após as avaliações, os pacientes iniciaram o programa de atividade física, com duração de oito semanas, realizado 3 vezes por semana durante 40 minutos na hemodiálise. Ao final do programa de exercícios físicos, os pacientes foram reavaliados. Resultados: Na avaliação da força muscular respiratória o valor da PImax pós foi significativamente maior que o valor obtido na avaliação pré programa (p<0,05), para a variável PEmax não foi encontrada diferença significativa entre os momentos pré e pós intervenção (p<0,05). As avaliações da capacidade funcional ou Teste de Caminhada de Seis Minutos (TC6’) inicial e após o programa de exercícios, não apresentaram diferenças significativas (p<0,05). Dados da Escala de Borg indicaram redução significativa do cansaço e dispnéia (p<0,05), após o programa de exercícios. A área septal medial (ASM), situada no prosencéfalo, está envolvida na regulação cardiovascular e no controle do balanço hidroeletrolítico. Esta área é rica em receptores colinérgicos e a ativação dos mesmos induz ingestão de água, natriurese e antidiurese. A ASM também envia projeções aos núcleos paraventricular (NPV) supra-óptico (NSO), os quais contêm os neurônios que secretam vasopressina e ocitocina. Existem evidências experimentais demonstrando que as espécies reativas de oxigênio podem participar do controle de respostas fisiológicas. Resultados recentes de nosso laboratório demonstraram que uma espécie reativa de oxigênio, o peróxido de hidrogênio (H2O2), injetado no ventrículo lateral (VL) reduz a ingestão de água e a resposta pressora induzida por ANG II e carbacol (agonista colinérgico) também injetados no VL. Por isso, o presente estudo teve como objetivo estudar os efeitos da injeção de H2O2 na ASM sobre a ingestão de água, sobre a excreção renal de água e eletrólitos e sobre a expressão da proteína c-Fos no NSO produzidos pela injeção de carbacol também na ASM. Para realizar este trabalho, foram utilizados ratos com cânulas de aço inoxidável implantadas na ASM. A ingestão de água e a excreção renal de água e eletrólitos foram estudadas em ratos que receberam injeções de H2O2 (5 mol/0,5 μl) ou PBS (veículo, 0,5 l) na ASM e, após um minuto, injeção de carbacol (4 nmol/0,5 l) ou salina (NaCl 0,15 M / 0,5 l) também na ASM. A ingestão de água induzida pelo carbacol, através da estimulação colinérgica, foi menor nos ratos que receberam a injeção prévia de peróxido de hidrogênio (8 ± 2,0 ml / 1 h, p<0,05) comparado àqueles que receberam veículo, também na ASM (16,6 ± 1,9 ml / 1 h, p<0,05).
