We evaluated the expression of Toll-like receptors 2 and 4 (TLR-2 and TLR-4) in circulating monocytes from peripheral blood of critical care patients treated with and without glutamine. Because no research has been published to date on the effect of glutamine on TLR receptors in critical patients, it was determined in an initial sample of 30 patients.

<p>Porcine circovirus type 2 (PCV2) nucleic acid and/or antigens are consistently observed in cells of monocytic morphology in lesions of pigs affected by post-weaning multisystemic wasting syndrome (PMWS). In this study, PCV2 antigen was detected in the cytoplasm of monocytes, pulmonary macrophages (PMs) and monocyte-derived macrophages exposed to the virus in vitro, by immunofluorescence analysis (IFA) and the phenotype of these cells confirmed by detection of monocytic cell surface markers using flow cytometry. Viral antigen was not observed in lymphocytic cells. Replication of the virus in PMs was investigated further by comparison to that observed in the continuous pig kidney cell line (PK15A) using quantitative virus titration, quantitative PCR and by the detection of double stranded DNA intermediates of viral replication by Southern blotting analyses. Although increases in viral DNA and levels of infectious virus progeny and the presence of replicative intermediates, indicative of viral replication, were observed in PK15A cells, no such changes were observed in PMs in spite of the fact that infectious virus, viral antigen and viral DNA persisted in the cells for at least the duration of the experiment. These results suggest that in vivo, monocytic cells may not represent the primary target for PCV2 replication. (C) 2003 Elsevier B.V. All rights reserved.</p>

A fully functioning immune system is essential in order to maintain good health. However, the immune system deteriorates with advancing age, and this contributes to increased susceptibility to infection, autoimmunity, and cancer in the older population. Progress has been made in identifying age-related defects in the adaptive immune system. In contrast, relatively little research has been carried out on the impact of ageing on the innate immune response. This area requires further research as the innate immune system plays a crucial role in protection against infection and represents a first line of defence. Macrophages are central effector cells of the innate immune system and have many diverse functions. As a result, age-related impairments in macrophage function are likely to have important consequences for the health of the older population. It has been reported that ageing in macrophages impacts on many processes including toll-like receptor signalling, polarisation, phagocytosis, and wound repair. A detailed understanding of the impact of ageing on macrophages is required in order to develop therapeutics that will boost immune responses in the older population.<br/>

<p>BACKGROUND: The chronic myeloproliferative disorders (MPD) are clonal haemopoietic stem cell disorders.</p><p>AIMS: The incidence of JAK2 V617F mutation was sought in a population of patients with MPD.</p><p>METHODS: The JAK2 V617 mutation status was determined in 79 patients with known MPD and 59 patients with features suggestive of MPD.</p><p>RESULTS: The mutation was found in patients with polycythaemia vera, essential thrombocythaemia, idiopathic myelofibrosis and in patients with other chronic myeloproliferative disorders. Eight JAK2 V617F positive cases were identified amongst those patients with features suggestive of MPD.</p><p>CONCLUSIONS: The incidence of the JAK2 V617F mutation in MPD patients is similar to that reported by other groups. The assay confirmed and refined the diagnosis of several patients with features indicative of MPD. We suggest screening for this mutation in all patients with known and suspected MPD as identification is valuable in classification and is a potential target for signal transduction therapy.</p>

Plasmacytoid dendritic cells (pDCs) were first described as interferon-producing cells and, for many years, their overlapping characteristics with both lymphocytes and classical dendritic cells (cDCs) created confusion over their exact ontogeny. In this Viewpoint article, Nature Reviews Immunology asks five leaders in the field to discuss their thoughts on the development and functions of pDCs--do these cells serve mainly as a major source of type I interferons or do they also make other important contributions to immune responses?

AIM: Sclerotherapy is the targeted chemical ablation of varicose veins by intravenous injection of a liquid or foamed sclerosing drug. The treated veins may be intradermal, subcutaneous, and/or transfascial as well as superficial and deep in venous malformations. The aim of this guideline is to give evidence-based recommendations for liquid and foam sclerotherapy. METHODS: This guideline was drafted on behalf of 23 European Phlebological Societies during a Guideline Conference on 7-10 May 2012 in Mainz. The conference was organized by the German Society of Phlebology. These guidelines review the present state of knowledge as reflected in published medical literature. The regulatory situation of sclerosant drugs differs from country to country but this has not been considered in this document. The recommendations of this guideline are graded according to the American College of Chest Physicians Task Force recommendations on Grading Strength of Recommendations and Quality of Evidence in Clinical Guidelines. RESULTS: This guideline focuses on the two sclerosing drugs which are licensed in the majority of the European countries, polidocanol and sodium tetradecyl sulphate. Other sclerosants are not discussed in detail. The guideline gives recommendations concerning indications, contraindications, side-effects, concentrations, volumes, technique and efficacy of liquid and foam sclerotherapy of varicose veins and venous malformations.

LâŸinfection par le VIH-1, chez les patients, affecte principalement le système immunitaire et conduit à une destruction graduelle des lymphocytes T CD4 et, par conséquent, entraîne un état dâŸimmunodéficience. Cette immunodéficience permet l'établissement dâŸinfections opportunistes qui sont responsables de manifestations cliniques associées au Sida. Ces patients peuvent aussi développer des lymphomes, lésions du système nerveux central et une atteinte rénale. L'ampleur et la sévérité des conditions associées observées chez les patients infectés par le VIH-1 ne peuvent être imputées seulement au processus infectieux et à la déplétion des cellules T CD4+. Ceci suggère que les produits des gènes de régulation pourraient avoir des effets cytopathogènes. Cependant, les études sur la physiopathogenèse induite par le VIH ou ses différents gènes ont été difficiles à mener en raison de l'absence d'animaux de laboratoire infectés par ce virus. Ceux-ci auraient pu aider à disséquer le rôle des différents composants du génome viral et les mécanismes pathogénétiques impliqués. Pour pallier cette contrainte, nous avons produit le premier modèle de souris transgéniques pour le gène vpu. Vpu code pour une phosphoprotéine membranaire avec plusieurs fonctions connues. Elle participe au relargage des virions à la surface cellulaire, induit la dégradation des CD4, induit la régulation négative des CMH-1, augmente la susceptibilité à la mort cellulaire des lymphocytes T infectés par le VIH et favorise la réplication virale en empêchant les mécanismes antiviraux cellulaires. Dans ce travail, nous avons caractérisé pathologiquement un modèle de souris transgéniques porteuses du gène vpu du VIH-1. Nos résultats démontrent que lâŸexpression de vpu chez les souris transgéniques induit le développement spontané dâŸune hyperplasie lymphoïde pansystémique, une splénomégalie avec une hyperplasie lymphoïde folliculaire évoluant en lésions prémalignes et malignes qui présentent certaines similarités avec la maladie de Castleman et une iv glomérulonéphrite mesangioproliférative qui rappelle certaines altérations de néphropathie associée au VIH chez les patients infectés. LâŸensemble des altérations démontre que les souris Tg/vpu développent une activation chronique et non spécifique du système immunitaire. Dans cette activation immunitaire, une dérégulation de lâŸIL-6 et une hyperplasie du réseau de cellules métallophiliques pourraient être impliquées. DâŸautres résultats obtenus sur les évaluations du fonctionnement du système immunitaire de la rate et du thymus mettent en évidence une susceptibilité augmentée des lymphocytes des tissus lymphoïdes aux effets apoptotiques de la dexaméthasone et des lipopolysaccharides et un retard dans le repeuplement par les cellules dâŸorganes lymphoïdes ainsi quâŸune réaction inflammatoire (Schwartzman) exacerbée et des anomalies dans la réaction dâŸhypersensibilité retardée expérimentale. Ce modèle transgénique reproduit plusieurs anomalies rencontrées chez les patients infectés par le VIH et ouvre de nouvelles hypothèses sur la pathogenèse de lâŸinfection par le VIH.

Introducción: El tratamiento con antagonistas del factor de necrosis tumoral alfa (anti TNF) ha impactado el pronóstico y la calidad de vida de los pacientes con artritis reumatoide (AR) positivamente, sin embargo, se interroga un incremento en el riesgo de desarrollar melanoma. Objetivo: Conocer la asociación entre el uso de anti TNF y el desarrollo de melanoma maligno en pacientes con AR. Metodología: Se realizó una búsqueda sistemática en MEDLINE, EMBASE, COCHRANE LIBRARY y LILACS para ensayos clínicos, estudios observacionales, revisiones y meta-análisis en pacientes adultos con diagnóstico de AR y manejo con anti TNF (Certolizumab pegol, Adalimumab, Etanercept, Infliximab y Golimumab). Resultados: 37 estudios clínicos cumplieron los criterios de inclusión para el meta-análisis, con una población de 16567 pacientes. El análisis de heterogeneidad no fue significativo (p=1), no se encontró diferencia en el riesgo entre los grupos comparados DR -0.00 (IC 95% -0.001; -0.001). Un análisis adicional de los estudios en los que se reportó al menos 1 caso de melanoma (4222 pacientes) tampoco mostró diferencia en el riesgo DR -0.00 (IC 95% -0.004 ; -0.003). Conclusión: En la evidencia disponible a la fecha no encontramos asociación significativa entre el tratamiento con anti TNF en pacientes con diagnóstico de AR y el desarrollo de melanoma cutáneo.

Over the last 25 years, the effects of fatty acids on the immune system have been characterized using in vitro, animal and human studies. Advances in fatty acid biochemistry and molecular techniques have recently suggested new mechanisms by which fatty acids could potentially modify immune responses, including modification of the organization of cellular lipids and interaction with nuclear receptors. Possibilities for the clinical applications of n-3 PUFA are now developing. The present review focuses on the hypothesis that the anti-inflammatory properties of n-3 PUFA in the arterial wall may contribute to the protective effects of n-3 PUFA in CVD, as suggested by epidemiological and secondary prevention studies. Studies are just beginning to show that dietary n-3 PUFA can be incorporated into plaque lipid in human subjects, where they may influence the morphology and stability of the atherosclerotic lesion.
