Objective: The purpose of this study was to examine the effect of maternal type 1 diabetes on the structure and function of the embryonic and neonatal mouse heart.

Methods: Type 1 diabetes was induced in female C57BL6/J mice using streptozotocin. Embryonic (n = 105) and neonatal hearts (n = 46) were examined using high-frequency ultrasound (US) and a cohort of E18.5 (n = 34) and 1-day-old pup hearts (n = 27) underwent histological examination.

Results: Global cardiac hypertrophy in late gestation (E18.5) was evident on US in the diabetic group compared to controls with increased interventricular septal (IVS) thickness (0.44 ± 0.08 mm vs 0.36 ± 0.08 mm, P < .05) and increased left ventricular wall thickness (0.38 ± 0.04 mm vs 0.29 mm ± 0.05, P < .01). Isovolumetric relaxation time was initially prolonged in the diabetic group but resolved by E18.5 to control values. Histological examination at E18.5 demonstrated increased transverse measurements (2.42 ± 0.72 mm/g vs 1.86 ± 0.55 mm/g, P < .05) and increased IVS thickness (0.64 ± 0.20 mm/g vs 0.43 ± 0.15 mm/g, P < .05) in diabetic embryos compared to control embryos.

Conclusion: Maternal hyperglycemia has severe effects on offspring with evidence of cardiac impairment and cardiac hypertrophy in the embryo. These effects persisted in the 1-day old but attenuated in the 1-week old suggesting cardiac remodeling after the hyperglycemic milieu of pregnancy is removed

BACKGROUND: Dilated cardiomyopathy (DCM) is a leading cause of chronic morbidity and mortality in muscular dystrophy (MD) patients. Current pharmacological treatments are not yet able to counteract chronic myocardial wastage, thus novel therapies are being intensely explored. MicroRNAs have been implicated as fine regulators of cardiomyopathic progression. Previously, miR-669a downregulation has been linked to the severe DCM progression displayed by Sgcb-null dystrophic mice. However, the impact of long-term overexpression of miR-669a on muscle structure and functionality of the dystrophic heart is yet unknown. METHODS AND RESULTS: Here, we demonstrate that intraventricular delivery of adeno-associated viral (AAV) vectors induces long-term (18 months) miR-669a overexpression and improves survival of Sgcb-null mice. Treated hearts display significant decrease in hypertrophic remodeling, fibrosis, and cardiomyocyte apoptosis. Moreover, miR-669a treatment increases sarcomere organization, reduces ventricular atrial natriuretic peptide (ANP) levels, and ameliorates gene/miRNA profile of DCM markers. Furthermore, long-term miR-669a overexpression significantly reduces adverse remodeling and enhances systolic fractional shortening of the left ventricle in treated dystrophic mice, without significant detrimental consequences on skeletal muscle wastage. CONCLUSIONS: Our findings provide the first evidence of long-term beneficial impact of AAV-mediated miRNA therapy in a transgenic model of severe, chronic MD-associated DCM.

FGF-2 has been implicated in the cardiac response to hypertrophic stimuli. Angiotensin II (Ang II) contributes to maintain elevated blood pressure in hypertensive individuals and exerts direct trophic effects on cardiac cells. However, the role of FGF-2 in Ang II-induced cardiac hypertrophy has not been established. Therefore, mice deficient in FGF-2 expression were studied using a model of Ang II-dependent hypertension and cardiac hypertrophy. Echocardiographic measurements show the presence of dilated cardiomyopathy in normotensive mice lacking FGF-2. Moreover, hypertensive mice without FGF-2 developed no compensatory cardiac hypertrophy. In wild-type mice, hypertrophy was associated with a stimulation of the c-Jun N-terminal kinase, the extracellular signal regulated kinase, and the p38 kinase pathways. In contrast, mitogen-activated protein kinase (MAPK) activation was markedly attenuated in FGF-2-deficient mice. In vitro, FGF-2 of fibroblast origin was demonstrated to be essential in the paracrine stimulation of MAPK activation in cardiomyocytes. Indeed, fibroblasts lacking FGF-2 expression have a defective capacity for releasing growth factors to induce hypertrophic responses in cardiomyocytes. Therefore, these results identify the cardiac fibroblast population as a primary integrator of hypertrophic stimuli in the heart, and suggest that FGF-2 is a crucial mediator of cardiac hypertrophy via autocrine/paracrine actions on cardiac cells.

De déterminer si une stratégie de contrôle du rythme améliore la qualité de vie et / ou la capacité fonctionnelle par rapport à une stratégie de contrôle de la fréquence cardiaque chez les patients atteints de fibrillation auriculaire et d'insuffisance cardiaque congestive. Méthode: Pour évaluer la qualité de vie, le questionnaire SF-36 a été administré à l'inclusion et à 4 mois chez 749 patients de l’étude AF-CHF. Les paramètres de capacité fonctionnelle évalués ont été la classe fonctionnelle NYHA (1376 patients) et la distance de marche de six minutes (1099 patients). Résultats: Le type du traitement assigné n'a pas eu un impact significatif sur la qualité de vie ou la capacité fonctionnelle. Conclusion: La qualité de vie et la capacité fonctionnelle sont similaires chez les patients randomisés au contrôle du rythme par rapport au contrôle de la fréquence. Les hommes non-obèses avec moins de comorbidités semblent plus susceptibles de s'améliorer.

INTRODUCTION : L’insuffisance cardiaque congestive (ICC) induit remodelage pulmonaire et dysfonction ventriculaire droite (VD) qui contribuent de façon importante à la morbidité/mortalité. Malgré l’efficacité prouvée, l’antagonisme des récepteurs minéralocorticoïdes est sous-utilisé en ICC et ses mécanismes d’actions demeurent incompris. Nous avons évalué si l’Aldostérone contribue au remodelage pulmonaire et à la dysfonction VD en stimulant la prolifération des myofibroblastes (MYFs) pulmonaires. MÉTHODE ET RÉSULTATS : L’étude a été réalisée chez des rats avec infarctus du myocarde (IM) de taille modérée à grande permettant le développement de l’ICC. Deux semaines après l’IM, les rats ont été traités avec 100mg/kg/jour d’Aldactone ou non, pendant trois semaines et comparé à un groupe témoin (N=21;24;8). Comparativement au groupe témoin, les rats IM ont développé une ICC caractérisée par une réduction de la fraction de raccourcissement du VG (53±1%vs.16±2%, moyenne±ESM, P<0.0001), une hypertension pulmonaire (PSVD:27±1vs.40±3mmHg, P<0.01) et une hypertrophie VD (VD/(VG+Septum):24±1%vs.38±3%, P<0.05). L’Aldactone n’a eu aucun effet sur ces paramètres. Les rats IM ont développé un syndrome pulmonaire caractérisé par un abaissement de la courbe respiratoire pression-volume, un remodelage structurel pulmonaire avec doublement du poids poumon sec (P<0.01) et de la fibrose pulmonaire avec augmentation du taux de collagène dans les poumons (P<0.05). L’Aldactone n’a pas restauré la fonction pulmonaire. Enfin, les MYFs pulmonaires isolés n’ont pas proliféré avec l’exposition de 48h aux deux traitements d’Aldostérone (10-7M, 10-6M). CONCLUSION : L’Aldostérone ne contribue pas au remodelage pulmonaire et à la dysfonction VD associés à l’ICC. D’autres mécanismes d’actions sont responsables des effets bénéfiques de l’Aldactone.

Introducción: El Aislamiento de Venas Pulmonares (AVP) es un procedimiento de alto costo al cual son sometidos pacientes con riesgo cardiovascular elevado. Requiere un alto grado de especialización en el personal médico y paramédico que lo ejecuta, con curvas de aprendizaje que sobrepasan los dos años de formación académica y entrenamiento específico. Metodología: Se realizó un estudio de cohorte retrospectivo, donde se incluyeron 88 sujetos sometidos al procedimiento en el lapso comprendido entre el 1º de enero y el 31 de diciembre de 2013, con el objetivo de evaluar su proceso de atención en el Centro Internacional de Arritmias ‘Andrea Natale’ de la FCI – Instituto de Cardiología. Se realizó análisis de regresión lineal y logística múltiple. Resultados: Se encontró que en el 97,73%% de los pacientes el diagnóstico principal era algún tipo de Fibrilación Auricular (FA); a su vez, la comorbilidad más frecuente fue HTA en el 30,68% y ningún paciente presentaba enfermedad coronaria, no hubo diferencias significativas por sexo. La complicación peri operatoria tuvo una incidencia del 3,41%, el 22,73% requirió ingreso a UCI con un promedio de días estancia 0,25+0,51. El 98,86% de la población estudiada recibió educación pos procedimiento acerca de sus cuidados y signos de alarma. Los factores encontrados en el estudio que afectan la duración del procedimiento y la estancia hospitalaria son las interconsultas pre procedimiento, el manejo médico de la cardiomiopatía de base y el uso de anti agregantes plaquetarios pre procedimiento; los cuales, son puntos por mejorar previo al ingreso o programación del paciente para ser llevado a AVP. Discusión: Como recomendaciones específicas se destacan: La necesidad de incluir en el protocolo de preparación para ablación de venas pulmonares la realización de interconsultas a las especialidades requeridas, antes de su ingreso para la realización del procedimiento. Es importante que el paciente que lo amerite haga parte de un programa de falla cardiaca previamente al procedimiento

In hypertension, left ventricular (LV) hypertrophy develops as an adaptive mechanism to compensate for increased afterload and thus preserve systolic function. Associated structural changes such as microvascular disease might potentially interfere with this mechanism, producing pathological hypertrophy. A poorer outcome is expected to occur when LV function is put in jeopardy by impaired coronary reserve. The aim of this study was to evaluate the role of coronary reserve in the long-term outcome of patients with hypertensive dilated cardiomyopathy. Between 1996 and 2000, 45 patients, 30 of them male, with 52 +/- 11 years and LV fractional shortening <30% were enrolled and followed until 2006. Coronary flow velocity reserve was assessed by transesophageal Doppler of the left anterior descending coronary artery. Sixteen patients showed >= 10% improvement in LV fractional shortening after 17 +/- 6 months. Coronary reserve was the only variable independently related to this improvement. Total mortality was 38% in 10 years. The Cox model identified coronary reserve (hazard ratio = 0.814; 95% CI = 0.72-0.92), LV mass, low diastolic blood pressure, and male gender as independent predictors of mortality. In hypertensive dilated cardiomyopathy, coronary reserve impairment adversely affects survival, possibly by interfering with the improvement of LV dysfunction. J Am Soc Hypertens 2010;4(1):14-21. Older adults with congestive heart failure [CHF] are likely to experience multiple readmissions to hospital. There have been several studies conducted on hospital readmissions; however, generalising the findings is problematic due to the use of variable definitions of what constitutes a readmission. This paper addresses the absence of Australian research comparing groups of older patients with CHF who are readmitted to hospital with those who are not readmitted. It also adopts one of the more frequently used definitions of readmission to aid in future comparability of research. Using a comparative cohort design, a multivariate logistic regression model was used to compare readmitted patients with non-readmitted patients and identify risk factors associated with readmission. Significant risk factors identified were male gender, numerous diagnoses, lengths of stay of 3 days or longer and admission from acute, subacute or aged care facilities. The increased likelihood of readmission among patients from acute, subacute and aged care services warrants further investigation.
