Os factores de risco nos adultos jovens são fortes preditores da incidência de doença cardiovascular e mortalidade na idade mais avançada. Em Portugal, desconhecem-se estudos que avaliem os factores de risco para as doenças crónicas, em jovens adultos, na transição do ensino secundário para o universitário. Este estudo pretendeu contribuir para a promoção do conhecimento dos determinantes sócioculturais e ambientais no diagnóstico e detecção de factores de risco para as doenças crónicas, nomeadamente as doenças cardiovasculares, em estudantes universitários. Teve como objectivo principal a investigação do efeito da vida académica nos factores de risco modificáveis, estilos de vida e determinantes da saúde. Teve como objectivos específicos identificar a(s) prevalência(s) dos factores de risco cardiovascular numa população universitária, a identificação dos intervalos de referência para a homocisteína total no soro de adultos jovens portugueses, a determinação do perfil lípidico, comportamentos de saúde e dieta alimentar de tipo mediterrânico entre os estudantes universitários de acordo com o género e a área científica de frequência e a avaliação longitudinal do impacto da exposição à vida académica no estado de saúde dos estudantes universitários Participaram no estudo 781 estudantes sendo a média de idades de 20,6. Os factores de risco estudados para as doenças crónicas, foram o hábito tabágico, a pressão arterial, o índice de massa corporal, a composição do sangue (lípidos, homocisteina e glicose), a alimentação e a actividade física. O estudo mostra que a prevalência de: sedentarismo é significativamente mais elevada nos rapazes (p<0,001); dislipidemia e a hipertrigliceridemia é significativamente mais elevada nas raparigas. Mais de um quarto dos estudantes tem colesterol elevado sendo a hipercolesterolemia significativamente mais elevada nas raparigas (p<0,001); a hipertensão verificou-se em ambos os sexos (6,0%) mas foi significativamente mais elevada nos rapazes (p=0,001). O estudo identificou o intervalo de referência para a homocisteína em adultos jovens portugueses independentemente do sexo (6,2 a 11,6 μmol/) sendo que, acima de 11,6 μmol/l é condição para vigilância médica em populações jovens adultas. Quando se estudou a exposição à vida académica comparada com aqueles que acabaram de entrar na universidade, verificou-se uma associação significativa no que respeita às concentrações de lípidos no sangue, à pressão arterial sistólica e à actividade física, tendo sido as raparigas aquelas que mais se afastavam dos padrões saudáveis (p<0,001). No que respeita à adesão à dieta mediterranica, não foram encontradas associações entre este tipo de alimentação e os vários factores de risco independentemente do género. Os resultados forneceram, evidências empíricas acerca da importância da detecção dos principais factores de risco na idade adulta (jovem) na prevenção das doenças cardiovasculares e vieram corroborar as orientações do Plano de Desenvolvimento Estratégico do Instituto Nacional de Saúde Português para as doenças crónicas, nomeadamente o estabelecimento de valores de referência nacionais para análises biológicas e as orientações do Plano de Acção Estratégica Global para a Prevenção e Controle das Doenças Não-Transmissíveis-2008/2013 da Organização Mundial de Saúde.

BACKGROUND: Digoxin intoxication results in predominantly digestive, cardiac and neurological symptoms. This case is outstanding in that the intoxication occurred in a nonagenarian and induced severe, extensively documented visual symptoms as well as dysphagia and proprioceptive illusions. Moreover, it went undiagnosed for a whole month despite close medical follow-up, illustrating the difficulty in recognizing drug-induced effects in a polymorbid patient. CASE PRESENTATION: Digoxin 0.25 mg qd for atrial fibrillation was prescribed to a 91-year-old woman with an estimated creatinine clearance of 18 ml/min. Over the following 2-3 weeks she developed nausea, vomiting and dysphagia, snowy and blurry vision, photopsia, dyschromatopsia, aggravated pre-existing formed visual hallucinations and proprioceptive illusions. She saw her family doctor twice and visited the eye clinic once until, 1 month after starting digoxin, she was admitted to the emergency room. Intoxication was confirmed by a serum digoxin level of 5.7 ng/ml (reference range 0.8-2 ng/ml). After stopping digoxin, general symptoms resolved in a few days, but visual complaints persisted. Examination by the ophthalmologist revealed decreased visual acuity in both eyes, 4/10 in the right eye (OD) and 5/10 in the left eye (OS), decreased color vision as demonstrated by a score of 1/13 in both eyes (OU) on Ishihara pseudoisochromatic plates, OS cataract, and dry age-related macular degeneration (ARMD). Computerized static perimetry showed non-specific diffuse alterations suggestive of either bilateral retinopathy or optic neuropathy. Full-field electroretinography (ERG) disclosed moderate diffuse rod and cone dysfunction and multifocal ERG revealed central loss of function OU. Visual symptoms progressively improved over the next 2 months, but multifocal ERG did not. The patient was finally discharged home after a 5 week hospital stay. CONCLUSION: This case is a reminder of a complication of digoxin treatment to be considered by any treating physician. If digoxin is prescribed in a vulnerable patient, close monitoring is mandatory. In general, when facing a new health problem in a polymorbid patient, it is crucial to elicit a complete history, with all recent drug changes and detailed complaints, and to include a drug adverse reaction in the differential diagnosis.

In 2003, prostate cancer (PCa) is estimated to be the most commonly diagnosed cancer and third leading cause of cancer death in Canada. During PCa population screening, approximately 25% of patients with a normal digital rectal examination (DRE) and intermediate serum prostate specific antigen (PSA) level have PCa. Since all patients typically undergo biopsy, it is expected that approximately 75% of these procedures are unnecessary. The purpose of this study was to compare the degree of efficacy of clinical tests and algorithms in stage II screening for PCa while preventing unnecessary biopsies from occurring. The sample consisted of 201 consecutive men who were suspected of PCa based on the results of a DRE and serum PSA. These men were referred for venipuncture and transrectal ultrasound (TRUS). Clinical tests included TRUS, agespecific reference range PSA (Age-PSA), prostate specific antigen density (PSAD), and free-to-total prostate specific antigen ratio (%fPSA). Clinical results were evaluated individually and within algorithms. Cutoffs of 0.12 and 0.15 ng/ml/cc were employed for PSAD. Cutoffs that would provide a minimum sensitivity of 0.90 and 0.95, respectively were utilized for %fPSA. Statistical analysis included ROC curve analysis, calculated sensitivity (Sens), specificity (Spec), and positive likelihood ratio (LR), with corresponding confidence intervals (Cl). The %fPSA, at a 23% cutoff ({ Sens=0.92; CI, 0.06}, {Spec=0.4l; CI, 0.09}, {LR=1.56; CI, O.ll}), proved to be the most efficacious independent clinical test. The combination of PSAD (cutoff 0.15 ng/ml/cc) and %fPSA (cutoff 23%) ({Sens=0.93; CI, 0.06}, {Spec=0.38; CI, 0.08}, {LR=1.50; CI, 0.10}) was the most efficacious clinical algorithm. This study advocates the use of %fPSA at a cutoff of 23% when screening patients with an intermediate serum PSA and benign DRE.

Mit ansteigenden Jahresmilchleistungen werden die Kühe einer hohen metabolischen Belastung ausgesetzt, die bei einer suboptimalen Gestaltung der Lebensbedingungen mit einer Erhöhung der Inzidenzrate von Faktorenkrankheiten einhergehen kann. Im Vordergrund der Untersuchungen stand die Frage, inwieweit metabolische Belastungszustände von Milchkühen in der Phase des Puerperiums und in Abhängigkeit von der Milchleistung sowie Belastungen durch eine klinische Erkrankung zu Beginn der Laktation mit Hilfe von Parametern der immunologischen Abwehr und der Reaktion des Immunsystems auf eine Challenge dargestellt werden können. In die Untersuchung wurden insgesamt 68 klinisch gesunde sowie 20 klinisch erkrankte Milchkühe der Rasse Holstein Friesian einbezogen. Die Untersuchungen erfolgten in den Zeiträumen 10-21 Tage sowie 14-15 Wochen nach der Kalbung. Dazu wurden an jeweils drei Untersuchungstagen (Tag 0, 2, 7) Blutproben aus der Vena jugularis entnommen. Zur Stimulierung des unspezifischen Immunsystems wurde in beiden Versuchszeiträumen an den Untersuchungstagen 0 und 2 der Paramunitätsinducer Zylexis® appliziert. Ferner wurde den Kühen zu Beginn der Laktation der Tollwutimpfstoff Rabisin® verabreicht. Für die Auswertung wurden die untersuchten Milchkühe in Gruppen unterteilt. Die klinisch gesunden Kühe wurden anhand der nach Fett und Eiweiß korrigierten 305-Tage-Leistung in die Gruppe M = mittleres Leistungsniveau (Milchmengenleistung 6.500-8.990 kg) und die Gruppe H = hohes Leistungsniveau (Milchmengenleistung 9.000-12.500 kg) aufgeteilt. Die klinisch erkrankten Kühe wurden in der Gruppe K zusammengefasst. Zur Beurteilung der Immunabwehr wurden die Parameter Phagozytoseaktivität der isolierten Neutrophilen Granulozyten, die Vollblutbakterizidie, die Lymphozytenproliferation mit den Mitogenen ConA, PHA und PWM sowie die spezifische Antikörperbildung gegen Tollwut untersucht. Anhand der Parameter Phagozytoseaktivität, Vollblutbakterizidie und Lymphozytenproliferation wurde zusätzlich die Reaktion auf die Challenge mit dem Paramunitätsinducer Zylexis® überprüft. Zur Erfassung der metabolischen Belastungszustände wurden verschiedene Stoffwechselparameter und hämatologische Parameter erfasst. Zu Beginn der Laktation konnten bei den Kühen in der mittleren und hohen Leistungsgruppe signifikant höhere Konzentrationen an ß-Hydroxybuttersäure und Freien Fettsäuren im Vergleich zu der Laktationsmitte festgestellt werden. Die Kühe mit einem hohen Leistungsniveau zeigten darüber hinaus eine signifikant höhere Bilirubinkonzentration am Laktationsanfang gegenüber der Laktationsmitte. Die mittlere Konzentration der freien Fettsäuren lag bei den Kühen in beiden Leistungsgruppen oberhalb des Referenzbereiches und wies auf eine negative Energiebilanz zu Laktationsbeginn hin. Die mittleren Konzentrationen bzw. Enzymaktivitäten der Parameter Cholesterol, Harnstoff, AST, GLDH, γ-GT, Gesamtprotein, Kalzium und Phosphor wiesen in der Laktationsmitte signifikant höhere Werte als am Laktationsanfang auf. Dabei lagen die mittleren Enzymaktivitäten von AST und GLDH sowie die Konzentrationen von Harnstoff und Gesamtprotein in der Laktationsmitte oberhalb des Referenzbereiches. Die untersuchten Stoffwechselparameter wurden durch die Challenge mit einem Paramunitätsinducer nicht signifikant beeinflusst. Hinsichtlich der Immunparameter zeigten die Milchkühe mit einem mittleren Leistungsniveau in Abhängigkeit vom Laktationszeitpunkt zu Beginn der Laktation eine signifikant höhere Phagozytoseaktivität gegenüber der Laktationsmitte (p < 0,05). Die Gruppe mit einem hohen Leistungsniveau wies dagegen keine signifikanten Unterschiede in der Phagozytoseaktivität zwischen den beiden Untersuchungszeiträumen auf. Bei den Immunparametern Vollblutbakterizidie und Lymphozytenproliferation konnten bei den Kühen beider Gruppen keine signifikanten Unterschiede in Abhängigkeit vom Laktationszeitpunkt festgestellt werden. Im Hinblick auf die Milchleistung wiesen die Kühe in den beiden Leistungsgruppen am Untersuchungstag 0 weder zu Beginn noch in der Mitte der Laktation signifikante Unterschiede bezüglich der untersuchten Immunparameter auf. Nach der Challenge durch den Paramunitätsinducer reagierten einzig die Kühe mit einer mittleren Leistung zu Beginn der Laktation auf die Challenge mit einer signifikanten Reduzierung der mittleren Phagozytoseaktivität von Tag 0 zu Tag 2 (p < 0,05). Bei den weiteren untersuchten Immunparametern konnten weder bei den Kühen mit einer mittleren noch mit einer hohen Leistung eine signifikante Reaktion auf die Challenge nachgewiesen werden. Die klinisch erkrankten Kühe zeigten im Vergleich zu den klinisch gesunden Kühen keine unterschiedlichen Konzentrationen bzw. Aktivitäten der untersuchten Stoffwechselparameter. Auch konnten anhand der immunologischen Parameter und der Reaktion auf die Challenge keine Unterschiede zwischen den klinisch gesunden Kühen festgestellt werden. Die in der vorliegenden Untersuchung geprüften metabolischen Belastungen konnten weder anhand der Immunparameter noch durch die Reaktion der Immunparameter auf eine Challenge dargestellt werden. Es wird geschlussfolgert, dass das immunologische Reaktionsvermögen nicht geeignet ist, um als Indikator für Belastungszustände bei der Milchkuh herangezogen zu werden.

Objetivo: Describir los niveles de vitamina D y calcular prevalencia de déficit de vitamina D en mujeres postmenopáusicas mayores de 50 años que asisten a consulta de medicina general en una muestra en de la ciudad de Bogotá durante el periodo 2013-2014. Pacientes y métodos: Realizamos un estudio descriptivo para estimar el déficit de VD en mujeres postmenopáusicas entre 50 y 80 años en la ciudad de Bogotá y la prevalencia de hipovitaminosis D. Durante el periodo octubre 2013 a octubre 2014 se analizaron 320 muestras, se identificaron las características socio-demográficas, patológicas y la exposición a radiación ultravioleta de cada participante. Se realizó un análisis descriptivo de las variables, se estableció su asociación con el déficit de vitamina D su aporte estadístico con al mismo. Resultados :El promedio de edad en la muestra fue de 61 años y el promedio de edad en la que reportaron la ultima menstruación fue a los 43 años. Con respecto a las características socio-demográficas 50% de las mujeres pertenecieron a los estratos 3 y 4, únicamente 11% de los pacientes realizaron una carrera universitaria y 73% de la mujeres fueron amas de casa. Con respecto a los niveles de vitamina D, 81% de los pacientes presentaron niveles anormales y el promedio de 25(OH)D en la población estudio fue de 19,81ng/ml. Conclusión: La prevalencia de déficit de vitamina D en mujeres bogotanas postmenopáusicas es significativo. Se requiere incrementar la exposición solar o dar suplencia con vitamina D para disminuir el riesgo de fracturas.

A pancreatite, apesar de relativamente comum na medicina canina, continua a constituir um desafio devido à sua elevada complexidade patogénica, sinais clínicos não patognomónicos, diagnóstico por vezes difícil e tratamento inespecífico. De forma a realizar um estudo retrospectivo foram avaliados registos médicos de 41 canídeos diagnosticados com pancreatite, apresentados à consulta entre 2007 e 2013, de forma a caracterizar o doente pancreático. As características do animal, história clinica, sinais clínicos apresentados, métodos de diagnóstico e tratamento instituído foram analisados. Como critérios de inclusão neste estudo encontram-se a medição de cPLI com resultados acima do valor de referência e a realização de ecografia abdominal. A ecografia demonstrou evidências de pancreatite em 81% dos casos, sendo este valor superior aos 68% encontrados na literatura. Os sinais clínicos apresentados mais comuns foram vómito (39), prostração (38), dor abdominal (36), perda de peso e anorexia (26), desidratação (25), diarreia (21) e febre (5). Todos os animais foram submetidos a tratamento médico, tendo sido apenas 4 deles sujeitos também a cirurgia. A taxa de sobrevivência nos animais com tratamento médico foi de 95% e de 50 % nos animais com tratamento médico e cirúrgico.

The purpose of this study was to determine the incidence of prostate cancer in patients who have an elevated referral prostate-specific antigen (PSA), which subsequently falls to within their normal age-specific reference range prior to prostate biopsy. The study demonstrated that of the 160 patients recruited, 21 (13%) had a repeat PSA level which had fallen back to within their normal range. Five of these 21 patients (24%) were diagnosed with prostate cancer following biopsy, two of whom had a benign prostate examination. The study, therefore, demonstrates that normalisation of the PSA level prior to biopsy does not exclude the presence of prostate cancer even when the prostate feels benign.

One third of older people in nursing and/or residential homes have significant symptoms of depression. In younger people, deficiencies in selenium, vitamin C and folate are associated with depression. This study examines the association between micronutrient status and mood before and after supplementation. The objective was to determine whether the administration of selenium, vitamin C and folate improved mood in frail elderly nursing home residents. Mood was assessed using the Hospital Anxiety and Depression rating scale (HAD), and Montgomery-Asberg Depression Rating Scale (MADRS). Micronutrient supplementation was provided for 8 weeks in a double-blinded randomised controlled trial. Significant symptoms of depression (29%) and anxiety (24%) were found at baseline. 67% of patients had low serum concentrations of vitamin C, but no-one was below the reference range for selenium. Depression was significantly associated with selenium levels, but not with folate or vitamin C levels. No individual with a HAD depression score of >= 8, had selenium levels >1.2 mu M. In those patients with higher HAD depression scores, there was a significant reduction in the score and a significant increase in serum selenium levels after 8 weeks of micronutrient supplementation. Placebo group scores were unchanged. This small study concluded that depression was associated with low levels of selenium in frail older individuals. Following 8 weeks of micronutrient supplementation, there was a significant increase in selenium levels and improved symptoms of depression occurred in a subgroup. Copyright (C) 2008 S. Objectives: To collect baseline data on the fat content of hot chips, quality (degradation) of cooking fat, deep-frying practices and related attitudes in fast food outlets in New Zealand. To identify the key determinants of the fat content of chips and quality of cooking fat. Methods: A nationally representative sample of fast food outlets (n=150, response rate 80%) was surveyed between September 1998 and March 1999. Data collected included a questionnaire, observation of cooking practices and analysis of cooked chips and frying fat. Results: Only 8% of independent operators had formal training in deep frying practices compared with 93% of chain operators. There was a wide range of fat content of chips (5%-20%, mean 11.5%). The use of thinner chips, crinkle cut chips and lower fryer fat temperature were associated with higher chip fat content. Eighty-nine per cent of chain outlets used 6–10 mm chips compared with 83% of independent outlets that used chips ≥12 mm. A wide range of frying temperatures was recorded (136–233°C) with 58% of outlets frying outside the reference range (175–190°C). As indices of fat degradation, fat acid and polar compound values above the recommended levels occurred in 54% and 5% of outlets respectively. Operators seemed willing to learn more about best practice techniques, with lack of knowledge being the main barrier to change. Conclusions and implications: Deep frying practices could be improved through operator training and certification options. Even a small decrease in the mean fat content of chips would reduce the obesogenic impact of this popular food.

Fracture risk is determined by bone mineral density (BMD). The T-score, a measure of fracture risk, is the position of an individual's BMD in relation to a reference range. The aim of this study was to determine the magnitude of change in the T-score when different sampling techniques were used to produce the reference range. Reference ranges were derived from three samples, drawn from the same region: (1) an age-stratified population-based random sample, (2) unselected volunteers, and (3) a selected healthy subset of the population-based sample with no diseases or drugs known to affect bone. T-scores were calculated using the three reference ranges for a cohort of women who had sustained a fracture and as a group had a low mean BMD (ages 35-72 yr; n = 484). For most comparisons, the T-scores for the fracture cohort were more negative using the population reference range. The difference in T-scores reached 1.0 SD. The proportion of the fracture cohort classified as having osteoporosis at the spine was 26, 14, and 23% when the population, volunteer, and healthy reference ranges were applied, respectively. The use of inappropriate reference ranges results in substantial changes to T-scores and may lead to inappropriate management.

Many children with autistic spectrum disorders (ASDs) suffer from gastrointestinal problems such as diarrhoea, constipation and abdominal pain. Such symptoms may be due to a disruption of the indigenous gut microbiota promoting the overgrowth of potentially pathogenic micro-organisms. These observations have stimulated investigations into possible abnormalities of intestinal microbiota in autistic patients. The purpose of the present study was to determine if a relationship exists between ASD severity (mild – severe) and GI microbial populations. The faecal microbiota of 22 male and 6 female participants with ASDs (aged 7 ± 6 years) were analyzed by standard microbial culture methods and compared within-group (based on ASD severity) and with a standard laboratory reference range. Comparisons between children with mild ASD and those with moderate to severe ASD, as well as comparisons to a neurotypical control group previously reported, revealed that no significant differences appear to exist in the composition of the gut microbiota. Nevertheless, examination of each individual’s gut microbial composition showed 10 cases of unusual findings witch means 1out of 3 cases have unusual microbiota. Our data do not support consistent GI microbial abnormalities in ASD children, but the findings do suggest that aberrations may be found in a minority subset of ASD children. Further studies are required to determine the possible association between the microbiota and gastrointestinal dysfunctions in a subset of children with both ASD and gastro-intestinal problems.

Introduction

We aimed to make recommendations concerning the use of total IgA in saliva (s-IgA) as an aid for monitoring athletic and military training.

Methods:
Unstimulated whole saliva was collected from 16 subjects (11 women and 5 men ages 18–57) during nonconsecutive days of fasting and non-fasting. Seven samples were collected from each subject at 0700, 0900, 1200, 1400, 1600, 1800, and 2030 on each day and a further three samples were collected 30 min after three meals on the non-fasting day (at 0730, 1230, and 1830). Strenuous activity was avoided and subjects did not drink caffeine or alcohol-containing beverages. Albumin and s-IgA were measured by commercial nephelometric immunoassays with intra-analytical coefficient of variance (CVA) of 1.8% and 2.9%, respectively. Individual and group variations were determined. Diurnal variation was determined by use of repeated-measures analysis of variance.

Results:
CV-individual (CVI) was 48% for s-IgA concentration and 43% for s-IgA secretion and s-IgA:albumin. CV-group (CVG) for these same measures was 68%, 75%, and 68%, respectively. When measurements were adjusted for saliva flow rates there was no evidence that s-IgA is subject to diurnal variation. There was strong evidence for a postprandial decrease in s-IgA for all measures.

Conclusion:
The high degree of individuality in s-IgA precludes the use of population reference ranges for identifying individual abnormal results. For the purpose of monitoring individuals we recommend using the individual's calculated biological variance (determined from previous serial measurements over a period of days to weeks). Individual abnormal results can then be identified.

Introdução: a obtenção de um bom controle metabólico é essencial para a prevenção das complicações crônicas do Diabetes Melito (DM). O tratamento é complexo e depende da implementação efetiva das diferentes estratégias terapêuticas disponíveis. Para que isso seja possível, é necessário que o paciente entenda os princípios terapêuticos e consiga executá-los. A precária educação em diabetes é percebida como um dos obstáculos para o alcance das metas terapêuticas. Objetivo: analisar, os fatores associados ao controle metabólico, em pacientes com DM tipo 2 (DM2) não usuários de insulina. Métodos: foi realizado um estudo transversal em pacientes com DM2 não usuários de insulina, selecionados ao acaso entre aqueles que consultavam nos ambulatórios de Medicina Interna, Endocrinologia e Enfermagem do Hospital de Clínicas de Porto Alegre. Os pacientes foram submetidos à avaliação clínica, laboratorial e responderam um questionário que incluía o tipo de tratamento realizado para DM, outros medicamentos e co-morbidades, pesquisa de complicações em ano prévio e avaliação do conhecimento sobre DM. Os pacientes foram classificados em dois grupos, com bom ou mau controle glicêmico, de acordo com o valor da glico-hemoglobina de 1 ponto % acima do limite superior do método utilizado. As comparações entre variáveis contínuas, com distribuição normal, foram analisadas pelo teste t de Student para amostras não-pareadas e para as variáveis de distribuição assimétrica ou com variância heterogênea o teste U de Mann-Whitney. A comparação entre percentagem foi feita pelo teste de qui-quadrado ou exato de Fisher. Foi realizada uma análise logística múltipla para identificar os fatores mais relevantes associados ao controle metabólico (variável dependente). As variáveis independentes com um nível de significância de P < 0,1 na análise bivariada, foram incluídas no modelo. Resultados: foram avaliados 143 pacientes com DM2, idade de 59,3 ± 10,1 anos, duração conhecida do DM 7,5 ± 6,3 anos, índice de massa corporal (IMC) de 29,7 ± 5,2 kg/m².Destes, 94 pacientes (65,73%) apresentavam bom controle glicêmico. Os pacientes com mau controle glicêmico usavam mais anti-hiperglicemiantes orais como monoterapia (OR = 9,37; IC = 2,60-33,81; P=0,004) ou associados (OR = 31,08; IC = 7,42-130,15; P < 0,001). Da mesma maneira, não fizeram dieta em dias de festa (OR = 3,29; IC = 1,51-7,16; P = 0,012). A inclusão do conhecimento sobre diabetes não foi diferente entre os pacientes com bom ou mau controle glicêmico (OR = 1,08; IC = 0,97 - 1,21; P = 0,219). A análise multivariada demonstrou que a consulta com a enfermeira educadora (OR = 0,24; IC = 0,108-0,534; P = 0,003), com o endocrinologista (OR = 0,15 ; IC = 0,063-0,373; P = 0,001) e o uso de hipolipemiantes (OR = 0,10; IC = 0,016 - 0,72; P = 0,054) foram associados ao bom controle glicêmico, ajustados para a não realização de dieta em festas, uso de anti-hiperglicemiantes orais e conhecimento sobre diabetes. Conclusão: o controle metabólico em pacientes DM2 é influenciado pelas atividades de educação com enfermeira e endocrinologista. O tratamento do DM2 deve incluir atividades de educação de forma sistemática.

Objectives To investigate the effects of levomepromazine and different desflurane concentrations upon electrocardiographic variables.Animals Twenty adult mongrel dogs of both sexes weighing 6-28 kg.Methods Dogs were divided into two groups of 10 animals. Group I received 1 mg kg(-1) lV of levomepromazine and 15 minutes later anesthesia was induced with propofol (3 mg kg(-1) IV). Desflurane end-tidal concentration was set at 1.6 MAC. After 30 minutes at this concentration, measurements were taken and the end-tidal concentration was reduced to 1.4 MAC. Thereafter, it was reduced to 1.2 and then 1.0 MAC at 1.5-minute intervals. The same procedure was followed for group 2, except that levomepromazine was replaced with 0.2 mL kg(-1) of 0.9% saline solution and more propofol was needed for induction (7 mg kg(-1)). The animals' body temperature was maintained between 38.3 and 39 degreesC using a heating pad. The electrocardiographic tracing was obtained from lead II throughout the experimental period. The measurements were taken immediately before the administration of levomepromazine or placebo (T-1), 15 minutes after pre-medication (T-2) and 30 minutes after the establishment of 1.6 MAC (T-3)The other measurements were made at the concentrations of 1.4, 1.2, and 1.0 MAC, respectively (T4-6). The numerical data were submitted to analysis of variance plus F-test (p < 0.05).Results the dogs that received levomepromazine had a decrease in heart rate. However, in both groups it increased with desflurane administration. Levomepromazine, in association with desflurane, did not induce significant electrocardiographic changes, and all mean values (except P-wave duration) were within the reference range for this species.Conclusions and clinical relevance This study documented that levomepromazine, in association with desflurane, does not induce significant changes in electrocardiographic variables, suggesting that this drug combination has minimal effect on myocardial conduction.

Mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT) platelet parameters can be important to improve the initial identification of platelet disorders. The purpose of this study is to establish reference values for the platelet count (PLT), MPV, PDW and PCT in a population of 1346 apparently healthy adults, from both sexes, as well as to evaluate the correlations among these platelet parameters and to build the necessary nomograms for the clinical interpretations of the platelet indices. The platelet parameters were analyzed in blood samples collected into dipotassium ethylenediaminetetra-acetic acid (K 2 EDTA) by an electrical aperture-impedance method. There were non-linear and an inverse correlations between MPV and PLT, and between PDW and PLT, throughout the reference range of platelet count: the change in MPV and PDW was most accentuated at the lower platelet counts. Because of this non-linear inverse correlation, MPV versus PLT and PDW versus PLT nomograms were built. These correlations among the platelet parameters on a normal population provide a better understanding of these indices and may contribute to establish the real clinical significance of these platelet parameters in many diseases.
