Quality of life has been shown to be poor among people living with chronic hepatitis C However, it is not clear how this relates to the presence of symptoms and their severity. The aim of this study was to describe the typology of a broad array of symptoms that were attributed to hepatitis C virus (HCV) infection. Phase I used qualitative methods to identify symptoms. In Phase 2, 188 treatment-naive people living with HCV participated in a quantitative survey. The most prevalent symptom was physical tiredness (86%) followed by irritability (75%), depression (70%), mental tiredness (70%), and abdominal pain (68%). Temporal clustering of symptoms was reported in 62% of participants. Principal components analysis identified four symptom clusters: neuropsychiatric (mental tiredness, poor concentration, forgetfulness, depression, irritability, physical tiredness, and sleep problems); gastrointestinal (day sweats, nausea, food intolerance, night sweats, abdominal pain, poor appetite, and diarrhea); algesic (joint pain, muscle pain, and general body pain); and dysesthetic (noise sensitivity, light sensitivity, skin. problems, and headaches). These data demonstrate that symptoms are prevalent in treatment-naive people with HCV and support the hypothesis that symptom clustering occurs.

Objetivo: determinar la prevalencia en los últimos 6 meses de los síntomas de cuello y miembro superior además de sus factores asociados, en trabajadores de una entidad financiera call center en el periodo comprendido de abril a octubre del año 2009. Métodos: se realizó un análisis descriptivo trasversal, a través de la aplicación de un cuestionario de morbilidad sentida que abarcó aspectos demográficos, antecedentes personales y antecedentes laborales. La presencia de los síntomas se documentó en una tabla donde se confrontaron los síntomas osteomusculares y los segmentos afectados en los últimos 6 meses. Adicionalmente se les pidió a los sujetos identificar la postura más frecuente durante su trabajo mediante un diagrama. Resultados: los síntomas más prevalentes fueron dolor en la muñeca derecha (0,44; IC 95% 0,37 – 0,51), dolor en el cuello (0,43; IC95% 0,36 – 0,50), rigidez en el cuello (0,33; IC95% 0,26 – 0,40) y dolor en la mano derecha (0,36; IC95% 0,29 – 0,43). Se encontraron diferencias estadísticamente significativas en cuanto al género en la presencia de dolor en muñeca derecha (26,1% hombres contra 73,9% mujeres; p=0,005), dolor en mano derecha (25% hombres versus 75% mujeres; p=0,008), síntomas neurológicos en mano derecha (19,4% versus 80,6%; p=0,001) y dolor en hombro derecho (26,3% hombres versus 73,7% mujeres; p=0,048). También se evidencio una diferencia estadísticamente significativa en la prevalencia del síntoma dolor en muñeca derecha según el auto reporte de mayor exigencia en el desempeño (85,2% con la percepción de mayor exigencias, versus 14,8% en los sujetos que no; p=0,020). Además una diferencia estadísticamente significativa con mayor presencia de síntomas en muñecas y manos en sujetos con postura en dorsiflexión de de las mismas (muñeca derecha 72,8%, p=0,001; muñeca izquierda 43,5%, p=0,020; mano derecha 62%, p=0,003). Conclusión: después de realizar el estudio se encontró como principal síntoma el dolor, localizado en: la muñeca derecha, el cuello, la mano derecha y el hombro derecho, con diferencias mayores para el género femenino según la postura de las muñecas, lo que es compatible con las condiciones de trabajo y la respuesta fisiológica a estas condiciones.

La industria de las centrales de llamadas es uno de los sectores de más rápido crecimiento en el mundo desarrollado, gracias a los avances tecnológicos han permitido su uso cada vez más generalizado desarrollando servicios integrales que son accesibles las 24 horas del día. Los operadores telefónicos o tele-operadores de esta industria se ven enfrentados a jornadas laborales en las que se exponen al uso constante de la voz, utilización permanente de auriculares de comunicación, confinamiento en estaciones de trabajo delimitadas pero no aisladas; aumentando así la prevalencia de síntomas como los otorrinolaringológicos. Este estudio tiene como objeto identificar la prevalencia de síntomas otorrinolaringológicos dados por alteraciones de la voz, compromiso auditivo y síntomas de la vía respiratoria superior durante la jornada laboral de los trabajadores de una central de llamadas de una prestigiosa empresa aseguradora de la ciudad de Bogotá Colombia, así como también identificar la asociación de factores demográficos organizacionales y biológicos con los síntomas otorrinolaringológicos y analizar el medio ambiente laboral de dicha empresa y la relación de los síntomas otorrinolaringológicos con mediciones de ruido, temperatura y humedad. La población estudiada fue de 81 tele operadores de los cuales 61 (75.3%) fueron mujeres, se evidencio que las enfermedades respiratorias altas tienen una prevalencia del 36%, también se reporto una prevalencia del 85% (69) tele operadores reportaron por lo menos un síntoma de voz y solo 12 tele operadores 15% no reportaron ningún síntoma. En cuanto a la hipoacusia solo 5 (6.2%) reportaron disminución de la agudeza auditiva

The ease with which we perform tasks such as opening the lid of a jar, in which the two hands execute quite different actions, belies the fact that there is a strong tendency for the movements of the upper limbs to be drawn systematically towards one another. Mirror movements, involuntary contractions during intended unilateral engagement of the opposite limb, are considered pathological, as they occur in association with specific disorders of the CNS. Yet they are also observed frequently in normally developing children, and motor irradiation, an increase in the excitability of the (opposite) homologous motor pathways when unimanual movements are performed, is a robust feature of the mature motor system. The systematic nature of the interactions that occur between the upper limbs has also given rise to the expectation that functional improvements in the control of a paretic limb may occur when movements are performed in a bimanual context. In spite of the ubiquitous nature of these phenomena, there is remarkably little consensus concerning the neural basis of their mediation. In the present review, consideration is given to the putative roles of uncrossed corticofugal fibers, branched bilateral corticomotoroneuronal projections, and segmental networks. The potential for bilateral interactions to occur in various brain regions including the primary motor cortex, the supplementary motor area, non-primary motor areas, the basal ganglia, and the cerebellum is also explored. This information may provide principled bases upon which to evaluate and develop task and deficit-specific programs of movement rehabilitation and therapy. (c) 2005 Elsevier B.V. A presente dissertação apresenta a análise da eficácia de regimes de proteção, nomeadamente fisioterapia e alterações no posto de trabalho, aplicados a operadores que sofrem ou sofreram de lesões músculo-esqueléticas relacionadas com o trabalho, do membro superior, numa amostra de operadores de uma empresa da indústria, composta por 41 homens e 14 mulheres, com idades compreendidas entre os 28 e os 53 anos. Ao longo do estudo foi aplicada uma compilação de questionários que avaliavam a perceção dos operadores no que diz respeito à incapacidade do membro superior e à prática de atividade física. No final da análise verificou-se que houve uma melhoria na capacidade funcional dos operadores após terem sido sujeitos a regimes de proteção, no entanto não se verificaram diferenças estatisticamente significativas quando comparada com as variáveis sociodemográficas. Foi recolhido o score de risco dos postos avaliados e na sua maioria, o nível de risco identificado era baixo, com algumas exceções. Ao longo do estudo, a adesão dos operadores foi muito positiva, mas alguns mostraram desânimo pois sentiam que os seus problemas não estavam a ser tratados convenientemente, segundo os seus pontos de vista. No final da dissertação são apresentadas as limitações do estudo e algumas propostas futuras.

Objective: Autism spectrum disorders are now recognized to occur in up to 1% of the population and to be a major public health concern because of their early onset, lifelong persistence, and high levels of associated impairment. Little is known about the associated psychiatric disorders that may contribute to impairment. We identify the rates and type of psychiatric comorbidity associated with ASDs and explore the associations with variables identified as risk factors for child psychiatric disorders. Method: A subgroup of 112 ten- to 14-year old children from a population-derived cohort was assessed for other child psychiatric disorders (3 months' prevalence) through parent interview using the Child and Adolescent Psychiatric Assessment. DSM-IV diagnoses for childhood anxiety disorders, depressive disorders, oppositional defiant and conduct disorders, attention-deficit/hyperactivity disorder, tic disorders, trichotillomania, enuresis, and encopresis were identified. Results: Seventy percent of participants had at least one comorbid disorder and 41% had two or more. The most common diagnoses were social anxiety disorder (29.2%, 95% confidence interval [CI)] 13.2-45.1), attention-deficit/hyperactivity disorder (28.2%, 95% CI 13.3-43.0), and oppositional defiant disorder (28.1%, 95% CI 13.9-42.2). Of those with attention/deficit/hyperactivity disorder, 84% received a second comorbid diagnosis. There were few associations between putative risk factors and psychiatric disorder. Conclusions: Psychiatric disorders are common and frequently multiple in children with autism spectrum disorders. They may provide targets for intervention and should be routinely evaluated in the clinical assessment of this group.

This study aimed at demonstrating the asymmetry in volume between the dominant and nondominant upper limbs in tennis players, controlled for maturity status. Upper limb volumes on both sides were calculated in 72 tennis players and 84 control subjects, using the truncated cone method. The participants’ maturity status was determined using the predicted age at peak height velocity (PHV). The results showed significant larger side-to-side asymmetry in volume in tennis groups than
in control groups. These findings suggested that, even before PHV, specific-sport adaptations occurred in the dominant upper limb in tennis players.

Background: The influence of adiposity on upper-limb bone strength has rarely been studied in children, despite the high incidence of forearm fractures in this population.

Objective: The objective was to compare the influence of muscle and fat tissues on bone strength between the upper and lower limbs in prepubertal children.

Design:
Bone mineral content, total bone cross-sectional area, cortical bone area (CoA), cortical thickness (CoTh) at the radius and tibia (4% and 66%, respectively), trabecular density (TrD), bone strength index (4% sites), cortical density (CoD), stress-strain index, and muscle and fat areas (66% sites) were measured by using peripheral quantitative computed tomography in 427 children (206 boys) aged 7–10 y.

Results: Overweight children (n = 93) had greater values for bone variables (0.3–1.3 SD; P < 0.0001) than did their normal-weight peers, except for CoD 66% and CoTh 4%. The between-group differences were 21–87% greater at the tibia than at the radius. After adjustment for muscle cross-sectional area, TrD 4%, bone mineral content, CoA, and CoTh 66% at the tibia remained greater in overweight children, whereas at the distal radius total bone cross-sectional area and CoTh were smaller in overweight children (P < 0.05). Overweight children had a greater fat-muscle ratio than did normal-weight children, particularly in the forearm (92 ± 28% compared with 57 ± 17%). Fat-muscle ratio correlated negatively with all bone variables, except for TrD and CoD, after adjustment for body weight (r = −0.17 to −0.54; P < 0.0001).

Conclusions:
Overweight children had stronger bones than did their normal-weight peers, largely because of greater muscle size. However, the overweight children had a high proportion of fat relative to muscle in the forearm, which is associated with reduced bone strength.
