Traditionally, the apicoectomies are carried out with the vestibular approach for visibility and easiness of access. The aim of this study was to present a case report of a patient with extensive injury endo-periodontal opted for the surgical access through a palatal incision. The patient presented with abscess drainage via periodontal ligament. The periodontal probing coincident with the radiographic apex in mesial tooth on dental element 22. Radiographically it was noted that the periapical lesion extended from the distal of the tooth 11 to the mesial of tooth 23. The tooth 22 had undergone endodontic treatment and showed signs of shutter material extravasation. It was decide to carry out an approach by the Palatine of the tooth to prevent gingival. After the flap elevation, the injury was debrided and apicoectomy was performed. The patient reported no pain or discomfort after surgery. Furthermore, as follow-up of 30 months there was total remission of signs and symptoms presented initially and absence of gingival recession. Therefore, according to the results showed in this case report, it is suggested that the Palatine access is an alternative approach that can be successfully employed in cases of apicoectomies in order to avoid the occurrence of gingival recessions.

Root canal and furcal perforations are causes of endodontic therapy failure and different materials that stimulate tissue mineralization have been proposed for perforation treatment. In the first case, a patient presented tooth 46 with unsatisfactory endodontic treatment and a periapical radiographic lesion. A radiolucent area compatible with a perforating internal resorption cavity was found in the mesial root. The granulation tissue was removed, and root canals were prepared. The intracanal medication was composed of calcium hydroxide and the perforation cavity was filled with Portland cement. The 11-year followup showed radiographic repair of the tissue adjacent to the perforation and absence of clinical signs and symptoms or periapical lesion. In the second case, a patient presented with edema on the buccal surface of tooth 46. The examination showed a radiolucent area in the furcation region compatible with an iatrogenic perforation cavity. The mesial root canals were calcified, and only the distal root canal was prepared. The cavity was filled with a calcium hydroxide-based paste and the distal root canal was obturated. In sequence, the perforation cavity was filled with Portland cement. The 9-year followup showed the tooth in masticatory function with radiographic and clinical aspects compatible with normality.

Malignant syphilis is an uncommon manifestation of secondary syphilis, in which necrotic lesions may be associated with systemic signs and symptoms. Generally it occurs in an immunosuppressed patient, mainly HIV-infected, but might be observed on those who have normal immune response. Since there is an exponential increase in the number of syphilis cases, more diagnoses of malignant syphilis must be expected. We report a case in an immunocompetent female patient.

We report on two patients with a history of chronic exposure to organochlorine insecticides who developed clinical and electromyographic signs and symptoms of chronic motor neuron disease. Measurements of aldrin, lindane and heptachlor confirmed the intoxication. We emphasize the importance of searching for toxic and environmental factors in cases of motor neuron disease especially in Third World countries, where workers usually wear no adequate protective equipment.

The Temporomandibular Joint is a noble structure of the complex mandibular, a lot of research was conducted on the to signs and symptoms of the alterations that attack those structures. ln spite of the high incidence of the DTM in children, there's little knowledge about it, wich makes difficult the treatment Desorders Craniomandibulares (DCM) or Desorders Temporomandibulars (DTM). The Temporomandibular Joint is composed basically by three elements: bones, muscles and disk, in relation to bony part, we have the fossae mandibular that is part of the temporary bone and wich houses the condyle mandibular, accomplishing the articulation among the cranium and the jaw (it leaves piece of furniture of the articulation). Our intention in that work was of verifying a possible asymmetry of the fossae mandibular on the left side and of the right side in relation to two straight line: a straight line that coincided with the plane medium sagittal and another perpendicular straight line to the plan medium sagittal. Analyzing, the fossae mandibular in 91 dry craniums of children, with age varying between four months of life intrauterina and five years, in x-rays in that the incidence was cranium-flow, we could end that: in spite of we find statistical significance in relation to that asymmetry, clinic cannot affirm that interferences on occlusion exists for that asymmetry

Introduction: A variety of hypotheses have been proposed to explain the association between temporomandibular disorders and hearing symptoms, including the many anatomical and/or neurological ways, as of a muscle or joint dysfunction, can cause such symptoms. Objective: The purpose of this study is to discuss, through a systematic review of literature, the interrelation between hearing symptoms and temporomandibular disorders. Material and Methods: We consulted the Medline/PubMed, ISI and Scielo in the period 1918 to 2008. The terms related to the research were: “temporomandibular disorders”, “signs and symptoms of temporomandibular disorders”, “otologic symptomsand “aural symptoms”, where 19 papers were included using the pre-selected criteria for the manufacture of a meta-analysis. Results: According to the findings, the hearing symptoms most often associated with temporomandibular disorders were otalgia and tinnitus, followed by vertigo and hearing loss, respectively. Conclusions: It’s clear the correlation between hearing symptoms and temporomandibular disorders, however there is a need clinical and scientific research more complex to prove the specific cause-effect relationships between them.

The orofacial pain, also known by the generic term dysfunction, is highly prevalent in the general population, causing great discomfort to the patients. For this reason, there is great importance of dentist’s participation in practice and in the process of proper diagnosis of this disease, which in modern times has become increasingly important as guides the best treatment. Thus, this study was aimed to review the main causes, diagnosis, main symptoms and treatment modalities, to serve as a guide to the dentist. Moreover, it also aims to review the main etiologic factors commonly associated, as emotional stress and muscular hyperactivity, to clearly identify signs and symptoms, so the dentist is able to realize simple treatment in your particular office or recognizes complex cases that could be send for treatment to multidisciplinary teams.

Costen’s syndrome is defined as a set of auditory signs and symptoms in patients with TMD. It is characterized by ear fullness, hearing loss, tinnitus, ear pain and vertigo with nystagmus.It has predilection for females and fourth decade of life. The multidisciplinary monitoring of these patients is imperative in its recognition and the establishment of effective therapy. In this paper we report a case of Costen’s Syndrome patient whose treatment consisted of advice on the etiology of the disorder and weekly sessions of acupuncture. The clinical features that led to the diagnosis and treatment plan, and the effectiveness of acupuncture as supportive therapy were also discussed.

This paper presents a case report of a patient with idiopathic thrombocytopenic purpura (ITP) diagnosed in routine dental care. ITP is defined as a decrease in the number of circulating platelets and generally shows oral manifestations. In this light, we show that the general dental practitioner must be ready to detect ITP signs and symptoms, especially oral changes that are not of dental origin, so as to immediately prescribe the proper treatment. Without this treatment, this pathology may well be irreversible and fatal.
