This article argues that the precarisation of employment that has taken place in Brazil since the 1990s has been fundamentally different in kind from earlier forms of precariousness, which took place outside the formal economy. The new forms of precariousness are taking place within the sphere of the economy controlled by transnational corporations. Although they have only reached critical mass during the 2000s, the ground was prepared by ‘post neoliberal’ restructuring, including labour law reforms, that took place in Brazil during the 1990s and introduced new forms of flexible working. The article argues that the new condition of labour now emerging in Brazil, which is a structural feature of labour under global capitalism, is characterised by psychosocial dynamics that cause: first, class desubjectivation; second, a ‘seizure’ of the waged worker's subjectivity; and third, the reduction of living labour to the status of a workforce treated as goods. Comprehending these changes necessitates a related change in the theoretical and methodological framework in which the precariousness of work is studied, one that incorporates within its scope the issues of workers' health and the quality of working life.

Objective - For patients with medication refractory medial temporal lobe epilepsy (MTLE), surgery offers the hope of a cure. However, up to 30% of patients with MTLE continue to experience disabling seizures after surgery. The reasons why some patients do not achieve seizure freedom are poorly understood. A promising theory suggests that epileptogenic networks are broadly distributed in surgically refractory MTLE, involving regions beyond the medial temporal lobe. In this retrospective study, we aimed to investigate the distribution of epileptogenic networks in MTLE using Bayesian distributed EEG source analysis from preoperative ictal onset recordings. This analysis has the advantage of generating maps of source probability, which can be subjected to voxel-based statistical analyses.Methods - We compared 10 patients who achieved post-surgical seizure freedom with 10 patients who continued experiencing seizures after surgery. Voxel-based Wilcoxon tests were employed with correction for multiple comparisons.Results - We observed that ictal EEG source intensities were significantly more likely to occur in lateral temporal and posterior medial temporal regions in patients with continued seizures post-surgery.Conclusions - Our findings support the theory of broader spatial distribution of epileptogenic networks at seizure onset in patients with surgically refractory MTLE.

The hypoparathyroidism is a rare endocrinopathy reported in dogs, caused by a deficiency in the synthesis of parathyroid hormone (PTH). The lack of PTH causes hypocalcemia and hyperphosphatemia, resulting in a series of neurological and neuromuscular disorders. Unlike most endocrinopathies, hypoparathyroidism is a disease in which the exogenous hormone replacement is not being viable, becoming the treatment a challenge. The present report aims to describe a case of primary hypoparathyroidism in a Schnauzer dog with seizures and neuromuscular disorders, and successful treatment employed, this being the first case, according to the literature, of hypoparathyroidism diagnosed in Brazil. The hypoparathyroidism should be considered as a differential diagnosis in cases of seizure. A complete neurological evaluation and determination of serum ionized calcium and parathyroid hormone are essential for the diagnosis of this disease. Early diagnosis may improve the quality of life of affected animals, since after the initiation of therapy, there is complete remission of clinical signs.

The use of multimodal neuroimaging techniques has been helpful in the investigation of epileptogenic zone in patients with refractory epilepsies. This work aims to describe an ictal event during EEG-fMRI performed simultaneously in a 39-year-old man with refractory epilepsy. The EEG data were recorded at a sampling rate of 5 kHz, using a BrainAmp (BrainProducts, München, Germany) amplifier, with 64 MR (magnetic resonance) compatible Ag/AgCl electrodes. MR images were acquired using a 3T scanner in 3 sequences of 6 minutes of echo-planar images (EPIs), with TR = 2s, being the last sequence stopped after the ictal event. The EEG was corrected for gradient and pulse artifacts using the Brain Vision Analyzer2 software (BrainProducts), and the functional images were realigned, slice-timing corrected, normalized and smoothed. The start of the ictal changes was used for the evaluation of the BOLD response in MR images, using a t-test with a minimum cluster of 5 voxels, p <0.005 (T>2.5). The patient had a partial complex seizure, as noted by neurologist. The fMRI data showed positive BOLD responses (activation) in dysplastic areas, but showed the most significant activation outside the lesion, in areas compatible with secondary spread of the epileptic focus, probably caused by motor reaction also observed during the seizure. As a conclusion, we note that the technique of EEG-fMRI can detect the epileptogenic zone in patients with refractory epilepsy, but areas of dissemination of primary epileptogenic focus may show significant activation, introducing additional difficulties to the interpretation of the results

Seizure is a clinical manifestation of abnormal neuronal hyperactivity of the cerebral cortex, presentation clinical varies according to the location and extent of the brain. The classic seizure begins with an inicial period, named prodrome, followed by the aura, the ictus and finally the post-ictus phase. Seizures can be generalized or focal/partial types. Focal seizures with secondary generalization are the most common type in dogs. The seizure can be divided etiologically in intracraneal (idiopathic epilepsy; sintomatic; probably sintomatic epilepsy) and extracraneal disorders. After determining the etiology of seizures and critical analysis of the frequency, severity, and worry they cause to the owner, you should make the decision whether to launch an anticonvulsant drug therapy. Although the ideal is the elimination of seizures, the real goal should be to decrease the frequency and intensity of crises, without affecting the quality of life of patients taking the drugs used side effects acceptable. The veterinary should consider the bioavailability of the drug, contraindications, toxicity, drug interactions, the amount of daily administration, the availability of the owner and the cost of therapy. Seizures can be controlled in about 70- 80% of the dogs and in the majority of the cats when treated with Phenobarbital alone. Addition of a second drug (usually potassium bromide), decreases the number of seizures in more that 50% in about 70-80% of dogs. However, approximately 20-30% of dogs are refractory to this treatment. In these cases, it will be necessary to think in other pharmacological alternatives, like felbamate, gabapentin, levetiracetam, zonisamid, among others. It has been concluded that... Epilepsy is a chronic disorder that impairs life quality (LQ) and interferes in emotional, personal, social and family relationship aspects. This study evaluated 165 people with epilepsy diagnosis, ranging from 18 to 75 years old (M=41,28; SD= 13,26), by means of the QQV-65 (Life Quality Questionnaire with 65 questions) and investigated the association between the questionnaire scores and the disease features, comparing patients who showed refrectory seizures with patients with partially controled seizures. There was no difference in relation to the two groups regarding the LQ factor. The QQV-65 emotional factor was the most affected in both groups, since the estimated mean z-escore was 51,0±14,4 for Group I and 49,9±14,7 for Group II. Regarding the type of crisis, the analysis of variance (ANOVA) revealed that Health was the only factor in QQV-65 that showed a statistically significant difference between the z-scores (P = 0.024). However, for the frequency of epileptic seizures, except Health (P = 0.185), other factors QQV-65 proved to be related in a statistically significant way (P> 0.05). The research demonstrated that seizure control perception is quite important in the evaluation of life quality in all its aspects (health, body, society, emotion, locus of control, self-concept and cognition).

The author, based on the work Change of Revolution, analyzes what Jacques Ellul understand as structural and existential in a Marxist revolution. Analyzing the main experiences of this type, the French thinker finds its by technification of the society. However, it also notes the validity of Marx’s objectives of overcoming the proletariat and the alienation. Thus, a revolutionary and libertarian socialism would be possible under certain conditions. On an existential level, Ellul questions in each one of us the permanence of efficiency values, of power and the pursuit of money as the main purpose of life, offering an authentic and radical transformation before the seizure of power. Thus, the author develops the Ellulian concept of God’s revelation in Jesus Christ as a lever and fulcrum of Revolution.

Two daughters of a nonconsanguineous couple are described. Both present mental retardation, epileptic seizures, congenital atrichia, histologically anomalous skin and abnormal EEG pattern. From a discussion of the literature on atrichia, the forms without involvement of teeth, nails and hidrosis, among which recessive inheritance prevails, are distinguished from each other. None of them coincide with the syndrome described here.

The status epilepticus (SE) is characterized by a series of crises which occur without recovery of consciousness or a single seizure lasting more than 30 minutes and can damage the central nervous system and systemic. The duration and frequency of attacks are directly related to patient prognosis. Whether seizures occur often and are longer, increase the risk of neurological and systemic complications. Child population has a higher frequency of seizures and it is associated with a lower threshold of immature brain to trigger these episodes. Aim: To determine the safest drugs prescribed for children in SE, their doses and schedules. A bibliographic survey was performed in electronic databases. Methods: The scientific health descriptors used for search was: “status epilecticus” and “anticonvulsivants” and “child”. Results: This strategy identified 396 manuscripts, of whom four were considered eligible for the study, after the assessment by floating reading and criteria. Of these, two were randomized trials and two descriptive. The studies address the use of oral midazolam, rectal diazepam and intravenous lorazepam. Conclusions: All investigated drugs were effective in treat in status epilepticus. Lorazepam is highlight, since it has fewer secondary effects and, as an alternative to the intravenous, oral midazolam. However, there is a need for further studies to demonstrate the efficacy and safety in the use of drugs in children.
