[Therapy-resistant pneumonia]

We report the case of a 72 year old patient with B-symptoms and a persistent pulmonary infiltrate despite an antibiotic therapy. Buds of granulation tissue were found by transbronchial biopsy proving an organizing pneumonia. B-Symptoms and pulmonary infiltrate were improved immediately by a therapy with steroids. Even though there were reasons for a secondary organizing pneumonia due to a known chronic lymphocytic leukemia and a pneumonia treated four months before, we consider a cryptogenic organizing pneumonia as most probable.

Wir berichten über eine 72-jährige Patientin mit B-Symptomatik und einem persistierenden pulmonalen Infiltrat trotz antibiotischer Behandlung. In der transbronchialen Biopsie fand sich intraalveoläres Granulationsgewebe vereinbar mit einer organisierenden Pneumonie. Eine Steroid-Therapie führte zu einer prompten Besserung der B-Symptomatik sowie zu einer Regredienz des Infiltrates. Auch wenn aufgrund einer chronisch-lymphatischen Leukämie (CLL) sowie einer vier Monate früher behandelten Pneumonie eine mögliche Ursache für eine sekundär organisierende Pneumonie vorlag, handelte es sich am ehesten trotzdem um eine kryptogen organisierende Pneumonie (COP).
